How to Treat Itchy Hemorrhoids: Creams, Baths & More

Itchy hemorrhoids respond well to a combination of home treatments, and most people find relief within a few days. The key is calming the irritated skin around the anus while addressing the underlying cause of the itch, which is usually moisture, friction, or minor inflammation from swollen hemorrhoidal tissue. Here’s what works and how to do it right.

Why Hemorrhoids Itch

The itch isn’t coming from the hemorrhoid itself. External hemorrhoids, the kind that form just outside the anus, make it harder to clean thoroughly after a bowel movement. Internal hemorrhoids, which sit inside the rectum, can cause small amounts of mucus or stool to leak onto the surrounding skin. Both situations leave behind residue that irritates the delicate perianal skin, triggering that persistent, maddening itch.

This matters because effective treatment targets the irritation, not just the hemorrhoid. If you only shrink the swelling but keep irritating the skin with rough wiping or moisture buildup, the itch will come right back.

Sitz Baths for Immediate Relief

A sitz bath is one of the fastest ways to calm itchy, inflamed skin around the anus. Fill a bathtub or a plastic sitz bath basin (they fit over your toilet seat) with a few inches of warm water, around 104°F (40°C). Soak the area for 15 to 20 minutes. You can do this up to three or four times a day when symptoms are active.

The warm water increases blood flow to the area, which helps reduce swelling, and it gently cleans without friction. Pat dry with a soft towel afterward rather than rubbing. Leaving the area damp will make the itch worse.

Over-the-Counter Treatments That Help

Witch Hazel Pads

Witch hazel is a plant-based astringent that tightens irritated tissue and reduces inflammation. Pre-soaked pads are the easiest way to apply it. You can use them to gently clean the area after bowel movements (instead of dry toilet paper, which can scratch inflamed skin) or press one against the area for a few minutes of relief. They’re safe to use up to six times a day.

Hydrocortisone Cream

Low-strength hydrocortisone cream, available without a prescription, reduces itch and inflammation quickly. It’s effective for breaking the itch-scratch cycle that makes hemorrhoid irritation worse. The important limitation: do not use hydrocortisone for more than seven consecutive days. The skin around the anus is thin, and prolonged steroid use can cause it to become even thinner and more fragile, which creates new problems.

Numbing Creams

Lidocaine-based creams at 5% strength are available over the counter specifically for hemorrhoid relief. They temporarily numb the nerve endings in the skin, which stops both itch and pain. These work well for nighttime use when itching tends to feel more intense and can disrupt sleep. Apply a thin layer to the external area only.

Cold Therapy Between Soaks

Ice packs constrict blood vessels and reduce swelling, which takes pressure off irritated nerve endings. Apply an ice pack to the anal area for 10 minutes at a time, several times a day. Always place a thin cloth between the ice and your skin to prevent cold burns. You can alternate between sitz baths and cold packs throughout the day for continuous relief.

Fiber and Softer Stools

Hard stools and straining during bowel movements are the most common reasons hemorrhoids flare up and stay irritated. Increasing your fiber intake softens stool and makes it easier to pass, which reduces the pressure on hemorrhoidal tissue and gives inflamed skin time to heal.

The recommended daily fiber intake is about 14 grams per 1,000 calories you eat. On a standard 2,000-calorie diet, that’s 28 grams a day. Most people fall well short of this. Good sources include beans, lentils, whole grains, berries, pears, and broccoli. If you’re adding fiber quickly, increase your water intake at the same time to prevent bloating and gas. A fiber supplement like psyllium husk can fill the gap if dietary changes alone aren’t enough.

Hygiene Habits That Prevent Flare-Ups

How you clean the area matters as much as what you put on it. Dry toilet paper is abrasive on swollen, itchy skin. Switching to unscented, alcohol-free wet wipes (or witch hazel pads) reduces friction and cleans more thoroughly. Some people find that a gentle rinse with a handheld bidet or peri bottle works even better.

Avoid scented soaps, bubble baths, or any fragranced product near the anus. These contain chemicals that strip the skin’s natural moisture barrier and worsen irritation. Wear breathable cotton underwear, and change it if you’ve been sweating. Moisture trapped against the skin creates the exact environment that feeds itching.

Resist the urge to scratch. Scratching damages the skin surface, triggers more inflammation, and creates a cycle where the itch keeps returning worse than before. If the itch hits at night, a numbing cream or a cool witch hazel pad applied before bed can help you get through without scratching in your sleep.

When the Itch Isn’t From Hemorrhoids

Not all anal itching comes from hemorrhoids, and treating the wrong cause means the itch won’t resolve. Two common mimics are worth knowing about.

Yeast infections caused by candida can affect the skin around the anus, producing itch along with visible redness, swelling, or small cracks in the skin. This is more likely if you’ve recently taken antibiotics or if you also have a vaginal yeast infection. Antifungal cream, not hemorrhoid cream, is the appropriate treatment.

Pinworms are tiny white worms that live in the intestines and lay eggs around the anus at night, causing intense itching that’s worst after bedtime. This is particularly common in children. A simple tape test confirms the diagnosis: press a piece of clear tape near the anus before bed, remove it in the morning, and look for tiny worms or eggs. Pinworms require an antiparasitic medication.

If your itch doesn’t improve after a week or two of consistent home treatment, or if you notice skin changes like discoloration, rashes, or open sores, the cause may be something other than hemorrhoids.

Procedures for Persistent Cases

When hemorrhoids keep causing symptoms despite weeks of fiber changes, sitz baths, and topical treatments, rubber band ligation is the most common next step. A small band is placed around the base of the hemorrhoid during an office visit, cutting off its blood supply. The hemorrhoid shrivels and falls off on its own within about a week.

The procedure is 70% to 80% effective. Recovery is quick, with most people returning to normal activities the same day or within a day or two. Some temporary difficulty with bowel movements or excess gas is normal in the first few days. Because the hemorrhoid is physically removed, the source of the itch disappears along with it.
