Canine scabies, also called sarcoptic mange, is treated with antiparasitic medications that kill the burrowing mites causing the infection. Most dogs need 30 to 60 days of treatment to fully clear the infestation, since the mite’s life cycle takes about three weeks to complete and treatment must eliminate each generation of hatching mites.
What Canine Scabies Looks Like
Sarcoptic mange is caused by a microscopic mite called Sarcoptes scabiei that burrows into your dog’s skin, lays eggs, and triggers intense itching. The itching is relentless. Dogs with scabies scratch, bite, and rub themselves constantly, often to the point of creating raw, bleeding patches. You’ll typically notice hair loss, crusty skin, and redness starting on the ears, elbows, belly, and hocks before spreading across the body.
One telltale sign vets look for is called the pinnal-pedal reflex: when you gently rub the edge of your dog’s ear between your fingers, the dog reflexively kicks a hind leg. It’s not a definitive test, but combined with visible symptoms, it strongly suggests scabies.
Why Diagnosis Can Be Tricky
Confirming scabies with certainty is harder than you’d expect. The standard method is a skin scraping, where a vet uses a blade to collect cells from the surface of the skin and examines them under a microscope. The problem is that even scraping 15 or more sites often turns up only one or two mites or eggs. Many dogs with active scabies will have completely negative skin scrapings simply because the mites are so few and so deep in the skin.
Because of this low detection rate, most vets will diagnose scabies based on the combination of symptoms, the ear-scratch reflex, and how the dog responds to treatment. If your dog improves after starting antiparasitic medication, that response itself confirms the diagnosis. Some clinics also use blood tests to check for antibodies against the mite, though these can miss early infections.
Antiparasitic Medications That Kill the Mites
Two topical treatments are specifically approved for sarcoptic mange in dogs: selamectin and a combination of moxidectin with imidacloprid. Both are applied as spot-on treatments to the skin between the shoulder blades.
Selamectin is given as a single topical dose, with a second dose one month later if needed. The moxidectin/imidacloprid combination requires two doses applied four weeks apart. These medications absorb through the skin and circulate in your dog’s body, killing mites as they feed.
Many vets also prescribe newer oral flea and tick medications from a drug class called isoxazolines. These chewable tablets were originally designed for fleas and ticks but have proven highly effective against sarcoptic mites as well. They’re often preferred because they’re easier to administer and dogs readily eat them.
Regardless of which medication your vet chooses, expect the full course of treatment to last 30 to 60 days. This extended timeline ensures that mites hatching from eggs laid before treatment started are killed before they can reproduce. Stopping early, even if your dog looks better, risks a relapse.
Managing the Itch During Recovery
The itching from scabies doesn’t stop the moment treatment begins. Even after the mites start dying, your dog’s immune system is still reacting to mite proteins, feces, and eggs embedded in the skin. Your vet may prescribe a short course of anti-itch medication to keep your dog comfortable during the first few weeks of treatment. This helps break the scratch cycle and gives damaged skin a chance to heal without being torn open again.
Treating Secondary Skin Infections
All that scratching often breaks the skin barrier and lets bacteria in. The most common culprit is a type of staph bacteria called Staphylococcus pseudintermedius, which causes red, oozing, or pus-filled bumps on the skin. This secondary bacterial infection, called pyoderma, needs its own treatment on top of the scabies medication.
For mild cases, chlorhexidine-based shampoos used weekly or chlorhexidine sprays applied daily to affected areas can clear the infection topically. More widespread infections may require oral antibiotics. The bacterium involved produces an enzyme that makes certain common antibiotics ineffective, so your vet will choose one that works against resistant strains. If the infection doesn’t respond to the first antibiotic, a skin culture can identify exactly which drug the bacteria are sensitive to.
Cleaning Your Home and Preventing Reinfection
Sarcoptic mites can survive off a host for two to three days, which means your dog’s bedding, blankets, collars, and any fabric they’ve been in contact with can harbor live mites. Machine wash everything in hot water, at least 50°C (122°F), for a minimum of 10 minutes to kill both mites and eggs. Dry on the hottest cycle your dryer offers.
Items that can’t be washed should be sealed in a plastic bag for at least 72 hours, though a full week is safer. Vacuum carpets, furniture, and your car’s interior thoroughly and discard the vacuum bag or empty the canister outside.
If you have multiple dogs, treat all of them at the same time, even those showing no symptoms. Scabies is highly contagious between dogs through direct contact, and an untreated dog in the household will simply reinfect the others.
Can You Catch Scabies From Your Dog?
Yes, but with an important caveat. The canine variety of the scabies mite can transfer to human skin and cause intense itching and a red, bumpy rash, usually on the arms, torso, or waistline. However, the canine mite cannot complete its life cycle on human skin. It can burrow in and cause temporary misery, but it can’t reproduce on you. The infestation in humans is self-limiting and resolves once all the dogs in the household are treated. If your rash persists after your dog’s treatment is complete, see a doctor to rule out human scabies, which is caused by a different variety of the same mite.
What Recovery Looks Like
Within the first two weeks of treatment, most dogs start scratching noticeably less as the mite population drops. Hair regrowth on bald patches typically begins within three to four weeks, though severely affected areas may take longer. Full recovery, meaning completely normal skin and coat, can take two to three months depending on how widespread the infection was and whether secondary infections were involved.
Your vet will likely want a follow-up visit around the four-to-six-week mark to confirm the mites are gone and the skin is healing properly. If itching returns after treatment ends, it could mean some mites survived or your dog was re-exposed, and another round of treatment may be needed.
