Swollen feet and ankles usually respond well to a combination of elevation, movement, and simple lifestyle changes. Most cases stem from fluid pooling in the lower legs after long periods of sitting or standing, excess sodium intake, or hormonal shifts like those during pregnancy. The strategies below work for the majority of people dealing with everyday swelling, but sudden or one-sided swelling needs a different response entirely.
Elevate Your Legs Above Your Heart
Elevation is the fastest way to get visible relief. The key detail most people miss: your legs need to be above the level of your heart, not just propped on an ottoman. Lying on a couch or bed with two or three pillows under your calves does the job. Aim for about 15 minutes per session, three to four times a day. Gravity works in your favor here, helping fluid drain back toward your core where your kidneys can process it.
If you work at a desk, even a partial elevation (feet on a low stool) throughout the day helps slow the rate at which fluid accumulates, though it won’t reverse swelling as effectively as lying down with your legs fully raised.
Use Compression Stockings
Compression stockings apply graduated pressure to your lower legs, squeezing fluid upward and preventing it from settling into your tissues. They come in different pressure levels measured in mmHg, and picking the right one matters. For mild, everyday swelling, 15 to 20 mmHg is usually enough. If you have more persistent or moderate swelling, 20 to 30 mmHg provides firmer support. Avoid any socks or stockings with tight bands at the top, as these can actually trap fluid below the band and make things worse.
Move Your Feet and Ankles Often
Sitting or standing in one position for hours is one of the most common causes of lower leg swelling. Your calf muscles act as a pump that pushes blood and fluid back up toward your heart, and that pump only works when you move. Walking is ideal, but even small movements help significantly when you can’t get up.
Ankle pumps are the simplest exercise you can do from a chair, bed, or couch. Point your toes toward your knees as far as they’ll go, then point them away from you, alternating back and forth for two to three minutes. Repeat this every 30 to 60 minutes throughout the day. Circular ankle rotations work the same muscles from a different angle. These movements are effective enough that hospitals use them routinely to prevent blood clots and fluid buildup in patients who can’t walk.
Cut Back on Sodium
Sodium causes your body to hold onto water. For people dealing with edema, research from Georgetown University’s nephrology department suggests keeping daily sodium intake between 1,375 and 1,800 mg. For context, the average American consumes over 3,400 mg per day, so most people have significant room to cut back.
The biggest sources of sodium are rarely the salt shaker. Processed foods, restaurant meals, canned soups, deli meats, and bread account for most of the sodium in a typical diet. Reading nutrition labels and cooking more meals at home gives you far more control. You’ll often notice a difference in swelling within a few days of reducing sodium, because your kidneys start flushing excess fluid relatively quickly once the sodium load drops.
Stay Hydrated
It sounds counterintuitive to drink more water when your body is already holding too much fluid, but dehydration actually makes swelling worse. When you’re not drinking enough, your kidneys hold onto sodium to maintain blood pressure, and that extra sodium pulls even more water into your tissues. Adequate water intake signals your kidneys to release sodium and reduce the osmotic pull that draws fluid into swollen areas. There’s no magic number for how much to drink, but steady intake throughout the day (rather than large amounts at once) gives your kidneys the best conditions to regulate fluid balance.
Try an Epsom Salt Soak
Epsom salt soaks are a popular home remedy, and there’s some clinical evidence behind them. A study on pregnant women with foot swelling tested a protocol of 30 grams of Epsom salt (about one tablespoon) dissolved in one liter of lukewarm water, applied to the feet for 20 minutes daily over three days. Before the intervention, 55% of participants had severe swelling and 45% had moderate swelling. After three days, none had moderate or severe swelling, with half showing no swelling at all. The study was small (20 women in the Epsom salt group), but the results were notable: a 74% average reduction in swelling scores.
Even without the magnesium sulfate, warm water soaks help by relaxing blood vessels and encouraging circulation. Adding Epsom salt may provide an additional benefit through magnesium absorption, though the exact mechanism isn’t fully settled. It’s a low-risk option worth trying.
Swelling During Pregnancy
Pregnancy swelling is extremely common, especially in the third trimester, and most of the standard strategies apply with a few specific adjustments. Sleep on your left side, which takes pressure off the large vein that returns blood from your lower body to your heart. Raising your legs slightly with pillows while sleeping adds another layer of relief.
Standing or walking in a pool is particularly helpful during pregnancy. The water pressure acts like natural compression on your legs, and the buoyancy takes weight off your joints at the same time. Avoid tight bands around your ankles or calves, wear supportive compression stockings during the day, and keep drinking normal amounts of water. Cutting back on fluids does not reduce pregnancy swelling and can cause other problems.
When Swelling Is a Warning Sign
Most foot and ankle swelling is harmless, but certain patterns signal something serious. The biggest red flag is sudden swelling in only one leg, especially if it comes with pain or cramping in the calf, skin that looks red or purple, or warmth in the affected area. These are the hallmark symptoms of a deep vein thrombosis (DVT), a blood clot in a deep leg vein that can become life-threatening if it travels to the lungs.
Swelling in both legs that develops gradually is far less likely to be a clot. But bilateral swelling that comes on suddenly, or that doesn’t respond to elevation and other home measures over several days, can point to heart, kidney, or liver problems that need medical evaluation. Swelling paired with shortness of breath, chest pain, or a sudden increase during pregnancy (along with headache or vision changes) also warrants prompt attention.
When Home Measures Aren’t Enough
If swelling persists despite consistent elevation, compression, movement, and dietary changes, the next step is usually a medical evaluation to look for an underlying cause. Persistent edema can result from venous insufficiency (where the valves in your leg veins don’t close properly), kidney issues, or medication side effects from drugs like calcium channel blockers or certain anti-inflammatory medications.
For swelling caused by lymphedema, a specialized technique called manual lymphatic drainage can help. This is a specific form of light, rhythmic touch performed by trained physical therapists that redirects excess fluid from swollen limbs into areas where the body can filter and eliminate it. It’s different from regular massage, and MD Anderson Cancer Center specifically notes that trendy “lymphatic massage” services offered at spas are not the same thing and aren’t designed to treat actual swelling conditions.
Prescription water pills (diuretics) are another option for persistent swelling, particularly when it’s tied to heart failure, kidney disease, or high blood pressure. These work by helping your kidneys excrete more sodium and water, reducing the overall fluid volume in your body. They’re effective but require monitoring, since they change your electrolyte balance and can cause dehydration if the dose isn’t right.
