How to Use Phexxi: Application, Timing, and Side Effects

Phexxi is a hormone-free contraceptive gel that you insert vaginally with a pre-filled applicator immediately before or up to one hour before sex. Each applicator delivers 5 grams of gel, and you need a new one for every act of intercourse. The process is similar to inserting a tampon, and the whole thing takes about a minute.

How Phexxi Works

Your vagina naturally maintains an acidic environment, with a pH between 3.5 and 4.5. Semen is alkaline, with a pH between 7.2 and 8.0. When semen enters the vagina, it temporarily raises the pH, creating conditions that allow sperm to survive and swim toward the egg.

Phexxi contains three acid-buffering ingredients that keep the vaginal pH low even after semen is present. Sperm are completely immobilized at a pH of 5 or below. The gel also has a thick consistency that forms a physical barrier over the cervix, adding a second layer of protection by blocking sperm from entering.

Step-by-Step Application

Each dose of Phexxi comes in a single-use, pre-filled applicator, so there’s no measuring or loading involved. To insert it:

  • Get into position. Lie on your back with knees bent, stand with one foot elevated on a chair, or squat. Whatever position you’d use for a tampon works here.
  • Insert the applicator. Gently slide the applicator into your vagina as far as it comfortably goes.
  • Push the plunger. Press the plunger all the way down to release the full 5 grams of gel.
  • Remove and discard. Pull the applicator out and throw it away. Do not reuse it.

That’s it. The gel spreads on its own to coat the vaginal walls and cervix. You don’t need to wait for it to “activate” before having sex.

Timing Rules That Matter

Phexxi has a strict one-hour window. You can apply it anywhere from immediately before sex to up to 60 minutes beforehand. If you insert the gel and then don’t end up having sex within that hour, you’ll need to use a fresh applicator if you still plan to.

If you have sex more than once within the same hour, you need a new applicator for each round. One dose covers one act of intercourse, period. And Phexxi does not work after sex. Applying it after intercourse will not prevent pregnancy.

How Effective Is Phexxi?

In the phase 3 clinical trial (called AMPOWER), the seven-cycle cumulative pregnancy rate with typical use was 13.7%. That means roughly 14 out of 100 people using Phexxi as their only contraception over seven menstrual cycles became pregnant. This reflects real-world use, including times when people didn’t apply the gel correctly or skipped it altogether.

That places Phexxi in a similar effectiveness range as other on-demand, non-hormonal methods like condoms and diaphragms. If higher protection matters to you, pairing Phexxi with condoms significantly lowers the odds of pregnancy compared to using either method alone.

Common Side Effects

In clinical trials involving nearly 2,500 users, the most frequently reported side effects were localized. Vaginal burning was the most common, affecting 18% of users, followed by vaginal itching at 14.5%. These sensations typically occur shortly after application and are usually mild and temporary.

Who Should Avoid Phexxi

Because the UTI rate in trials was 9%, people who are already prone to recurrent urinary tract infections should weigh that risk carefully. The acidic formulation can irritate urethral tissue, potentially increasing susceptibility.

Phexxi is designed specifically for preventing pregnancy through vaginal intercourse. It does not protect against sexually transmitted infections. It’s also not a spermicide, despite working in a somewhat similar on-demand way. Its mechanism is pH regulation rather than chemical destruction of sperm cells, which is a meaningful distinction for people who’ve had reactions to traditional spermicides containing nonoxynol-9.

Using Phexxi With Other Methods

Phexxi is compatible with latex and polyisoprene condoms. Using it alongside condoms gives you both a barrier method and a pH-based method working simultaneously, which improves overall protection.

You should not use Phexxi with a vaginal ring. The gel can affect how the ring releases its hormones. It’s also not designed to be used alongside vaginal diaphragms or cervical caps, as these weren’t studied together in clinical trials. Hormonal methods that don’t sit in the vagina (pills, patches, implants, IUDs) are generally fine to combine with Phexxi, though this would typically make Phexxi redundant for pregnancy prevention.

Practical Tips

Phexxi comes in boxes of 12 pre-filled applicators. Each one is individually wrapped and compact enough to carry in a bag or keep in a nightstand. The gel is clear and mostly odorless, though some users notice a slight tang from the citric acid.

Expect some leakage after sex. The gel doesn’t fully absorb, so wearing a liner afterward is practical if you’re heading out. This is normal and doesn’t mean the product failed to work.

Because the one-hour window is firm, keeping an applicator accessible matters. If spontaneity is important to you, having one nearby (rather than across the house or in a different bag) makes consistent use more realistic. Consistency is the single biggest factor in how well any on-demand contraceptive performs.
