The Spiriva HandiHaler is a dry powder inhaler that delivers one capsule of medication per dose. You inhale the powder directly into your lungs, where it relaxes the muscles around your airways and keeps them open for a full 24 hours. The device itself is straightforward once you learn the sequence, but small mistakes in technique can mean the medication never reaches your lungs. Here’s exactly how to use it.
Prepare the Device and Capsule
Start by pulling open the dust cap on the HandiHaler, then open the mouthpiece by lifting it upward. You’ll see a small center chamber where the capsule sits.
Take one Spiriva capsule from its blister pack. Separate the blister strip and peel back the foil only to the “STOP” line printed on the packaging. Drop the capsule into the center chamber. Don’t remove capsules ahead of time. Each capsule should stay sealed in its blister until the moment you’re ready to inhale, because exposure to air and moisture degrades the powder inside.
Close the mouthpiece firmly until you hear a click. Then press the green button on the side of the device once. This pushes a small needle through the capsule, creating holes that release the powder when you inhale. Press the button only once. Pressing it multiple times can shatter the capsule into small pieces.
Inhale the Medication
Before putting the mouthpiece to your lips, breathe out fully, away from the device. Never exhale into the mouthpiece, as moisture from your breath can make the powder clump and stick inside.
Keep your head upright. Close your lips tightly around the mouthpiece and breathe in slowly and deeply. You need enough force to make the capsule rattle and vibrate inside the chamber. That vibration is your confirmation that air is flowing through the capsule and carrying the powder into your lungs. If you don’t hear it, your breath isn’t strong enough or the seal around the mouthpiece isn’t tight.
Once your lungs are full, remove the HandiHaler from your mouth and hold your breath for up to 10 seconds, or as long as feels comfortable. Then breathe out normally.
Take a Second Breath From the Same Capsule
One inhalation usually doesn’t empty the capsule completely. To get the full dose, repeat the process: breathe out fully (away from the device), seal your lips around the mouthpiece, inhale slowly and deeply until you hear the capsule vibrate, then hold your breath again for up to 10 seconds. You do not need to press the green button a second time. The capsule is already pierced.
After your second inhalation, open the mouthpiece and tip out the used capsule. You can check whether powder remains inside it. If the capsule still looks full, something went wrong with your technique. Most commonly, this means you either didn’t inhale with enough force or didn’t seal your lips tightly enough around the mouthpiece.
When You’ll Feel It Working
Spiriva is not a rescue inhaler. It won’t provide instant relief during an acute breathing episode. After inhalation, you’ll typically notice your airways opening within about 30 minutes. The peak effect takes 1.5 to 3 hours. From there, a single dose keeps working for at least 24 hours, which is why you only use it once a day, at the same time each day.
Keeping Track of Your Doses
The HandiHaler has no built-in dose counter. Each prescription comes with 30 capsules in individual blisters, so the simplest tracking method is counting your remaining blisters. Some people mark a calendar or set a daily phone alarm. Because the capsule is a separate, visible object you load by hand each time, it’s easier to confirm whether you’ve taken your dose compared to metered-dose inhalers where the count is hidden inside.
One important note that the FDA emphasizes repeatedly: these capsules are for inhalation only. Do not swallow them. Swallowing a Spiriva capsule won’t deliver the medication to your lungs, so it simply won’t work.
Cleaning the HandiHaler
Powder residue builds up inside the chamber over time, which can interfere with how well the capsule releases its contents. Clean the device as needed by rinsing it under warm running water. While the water runs, press the green piercing button a few times so water flows through the center chamber and around the needle. Check that no powder buildup or capsule fragments remain.
Tip excess water onto a paper towel, then leave the device open to air dry with the dust cap, mouthpiece, and base all spread apart. This takes a full 24 hours. Do not use a hair dryer or any heat source to speed up the process, and do not use the device while it’s still wet. If you only have one HandiHaler, plan your cleaning so the device dries overnight and is ready by your next scheduled dose.
Common Mistakes That Reduce Your Dose
Three errors come up most often, and all of them mean less medication actually reaches your lungs:
- Not inhaling deeply enough. If you can’t hear the capsule rattling, the airflow through the device is too weak to pull the powder out. Try a slower but deeper breath rather than a short, forceful puff.
- Exhaling into the mouthpiece. Moisture from your breath makes the powder sticky and harder to inhale. Always breathe out fully before bringing the device to your mouth, and direct your exhale away from it.
- Piercing the capsule incorrectly. Press the green button firmly, once. If you press it multiple times or press it without fully closing the mouthpiece first, the capsule can break into pieces rather than being cleanly pierced.
Storing Your Capsules
Keep Spiriva capsules at room temperature, ideally around 77°F (25°C), though anywhere between 59°F and 86°F is acceptable. Avoid leaving them in a hot car, a steamy bathroom, or anywhere with high humidity. Always store capsules inside their sealed blister packaging and only peel one open right before you use it. Once a blister is opened, the capsule should be used immediately.
