A review in npj Digital Medicine examined the landscape of bias in healthcare AI and found serious gaps. One analysis of 555 neuroimaging-based AI models for psychiatric diagnosis discovered that 97.5% were trained exclusively on data from high-income countries, and only 15.5% included any external validation. That means an AI tool trained primarily on data from white, affluent populations may perform poorly, or dangerously, when applied to patients from different racial, ethnic, or socioeconomic backgrounds.

Addressing this requires intervention at every stage: diverse training datasets, routine bias audits during development, and ongoing monitoring after deployment. Researchers have proposed frameworks like PROBAST and PRISMA-based bias assessment to standardize how developers check their models, but adoption remains inconsistent. For patients, the practical concern is real. An algorithm that performs brilliantly in a clinical trial at a major academic hospital may underperform at a rural clinic or in a community with a different demographic profile.
