The 10 mg tablet is the lowest available strength, often prescribed as a starting dose for people who may be more sensitive to the medication’s effects, including older adults, those with liver or kidney concerns, or anyone new to the drug.

Hydroxyzine HCl has three FDA-approved uses. First, it treats anxiety and tension, both on its own and alongside other conditions where anxiety is a significant symptom. Second, it manages itching caused by allergic skin conditions like chronic hives (urticaria), eczema, and contact allergies. Third, it’s used as a sedative before or after general anesthesia.
The drug works by blocking histamine, one of the chemicals your body releases during allergic reactions. This same mechanism produces a calming, sedative effect in the brain, which is why it pulls double duty for both itching and anxiety. It is not a benzodiazepine, a narcotic, or a controlled substance, which makes it a common choice for short-term anxiety relief in people who have a history of substance misuse or who want to avoid habit-forming medications.

The FDA label specifically recommends that older adults start at the low end of the dosing range because the body clears the drug more slowly with age, and side effects like drowsiness and confusion hit harder. The 10 mg tablet is also commonly used for children, younger adults who are sensitive to sedation, or as an “as needed” dose for mild, situational anxiety. Your prescriber may increase the dose over time depending on how you respond.
How Quickly It Works
Hydroxyzine is absorbed relatively fast after you swallow it. You can expect to feel its effects within 15 to 30 minutes, with peak activity around 2 hours. A Cochrane review of five clinical trials involving 884 participants found that hydroxyzine is more effective than a placebo for generalized anxiety disorder. It also performed comparably to benzodiazepines and buspirone, two other commonly prescribed anti-anxiety drugs. However, the evidence base is small, and the review noted that it can’t be recommended as a reliable first-line treatment for anxiety based on current data alone.
The practical advantage over benzodiazepines is the absence of dependency risk. Benzodiazepines are controlled substances with real potential for physical dependence and withdrawal, especially with regular use. Hydroxyzine doesn’t carry those risks, which is why prescribers often reach for it as a bridge treatment while a longer-term anxiety medication (like an antidepressant) takes effect. That said, neither hydroxyzine nor benzodiazepines are considered first-choice options for long-term anxiety management.
Common Side Effects
The most frequently reported side effect is drowsiness, which is essentially built into how the drug works. The American Geriatrics Society lists hydroxyzine on its Beers Criteria, a widely used guide that flags medications considered potentially inappropriate for adults over 65. The recommendation is to avoid it when possible. The concern is hydroxyzine’s anticholinergic activity, which means it blocks a neurotransmitter involved in memory, digestion, and bladder function. In older adults, this translates to a higher risk of confusion, constipation, dry mouth, falls, delirium, and with cumulative exposure over time, an increased risk of dementia.
This doesn’t mean it’s never used in older adults, but it does explain why the 10 mg dose exists. If you’re over 65 and taking this medication, your prescriber is balancing the benefit against these specific risks, and the low dose is part of that calculation.
