Invisalign vs Braces: What Actually Hurts More?

Braces hurt more than Invisalign, but mainly in the first few days. A systematic review of multiple clinical studies found that patients with traditional fixed braces reported higher pain levels than Invisalign users during the initial days of treatment. After that early window, the difference in discomfort largely disappears, with both treatments producing similar pain levels from about two weeks onward.

That said, the two systems cause different kinds of discomfort. Understanding where each one hurts, and for how long, can help you choose the option that fits your pain tolerance and lifestyle.

Pain in the First Few Days

The biggest gap in comfort between braces and Invisalign shows up right at the start. Studies measuring pain 24 hours after treatment began consistently found higher scores for fixed braces. Significant differences were recorded on day one and day two, but by day 14, two separate studies found no measurable difference between the groups. Patients with braces also reached for pain relievers more often during those first few days.

This pattern makes sense when you consider what’s happening mechanically. Braces apply force through a metal wire threaded across every tooth, creating broad, constant pressure. Invisalign trays grip individual teeth with a more targeted, lighter force. Both are moving teeth through bone, which is inherently uncomfortable, but the initial intensity is lower with aligners.

How Each Type of Discomfort Feels

Braces and Invisalign produce qualitatively different pain experiences. With braces, you get two sources of discomfort working at the same time: pressure on the teeth from the wire, and physical irritation of your cheeks, lips, and tongue from the metal hardware. Brackets have sharp edges. Wire ends can poke. Research shows that roughly 51% to 63% of orthodontic patients develop mouth ulcers at some point during treatment, caused by brackets and wires rubbing against soft tissue and breaking down the inner lining of the mouth.

Invisalign largely avoids that second category of pain. The smooth plastic trays sit flush against your teeth without protruding edges. You can still get minor irritation from the small composite bumps (called attachments) bonded to certain teeth to help the trays grip, especially if they’re placed near the sides of your mouth. But this tends to settle quickly as your cheeks adjust, and it’s far less common than the ulcers braces cause.

The pressure-on-teeth feeling, however, is present with both. Invisalign users describe tightness and soreness when switching to a new tray, which happens every one to two weeks. Braces patients feel the same thing after each tightening appointment, typically every four to six weeks. The soreness from either event lasts a couple of days.

Eating and Daily Comfort

One area where Invisalign consistently wins is mealtime comfort. A study assessing quality of life during treatment found that 47% of Invisalign patients rated their eating and chewing experience positively, compared to just 24% of braces patients. This gap exists partly because you remove Invisalign trays to eat, so there’s no hardware interfering with chewing. With braces, biting into firm foods puts direct pressure on brackets and sore teeth, and food getting caught in wires adds its own frustration.

Braces also require periodic adjustments that can leave teeth tender for days afterward. While Invisalign tray changes produce a similar wave of soreness, you control the timing yourself at home rather than leaving an orthodontist’s office with freshly tightened wires.

Long-Term Pain: More Similar Than Different

After the first few months, pain levels between the two treatments converge. The systematic review found no notable differences beyond the three-month mark. Both systems are doing the same fundamental job of remodeling bone to shift teeth, and once your mouth adapts to the presence of hardware or trays, the baseline discomfort is comparable.

The main ongoing difference is soft tissue irritation. Braces patients may deal with ulcers and sore spots throughout treatment, especially after adjustments when a new wire or tighter configuration creates fresh friction points. Invisalign users occasionally feel tray edges against their gums but rarely develop the persistent sores that brackets cause.

Managing Pain With Either Option

Over-the-counter pain relievers work well for orthodontic soreness regardless of which system you choose. A meta-analysis comparing common options found that all three major choices (ibuprofen, acetaminophen, and naproxen) significantly reduced pain compared to placebo. Naproxen showed the strongest effect at both the two-hour and six-hour marks, and its relief lasted a full 24 hours. Ibuprofen peaked around six hours and remained effective through 24 hours. Acetaminophen built up more gradually but provided strong relief by the 24-hour point.

Taking a pain reliever about an hour before a braces adjustment, or before putting in a new Invisalign tray, can get ahead of the soreness before it peaks. For braces-specific irritation, orthodontic wax pressed over a sharp bracket or wire end creates a barrier between the metal and your cheek. Rinsing with warm salt water also helps soothe ulcers when they develop.

Which One Should You Choose Based on Pain?

If minimizing discomfort is a top priority, Invisalign has a clear edge in the early weeks and an ongoing advantage in soft tissue comfort. You’ll experience less intense startup pain, fewer mouth sores, and an easier time eating. But the difference narrows significantly after the first month, and both treatments involve recurring waves of soreness as teeth continue to move.

Pain tolerance also varies widely between individuals, and the complexity of your case matters. Some orthodontic problems require forces that only braces can deliver, which may take the decision out of your hands. For cases where either option works, most patients find Invisalign the more comfortable experience overall, with the caveat that neither treatment is pain-free.
