For most adults, 9mg of melatonin is a high dose. Standard clinical recommendations start at 2mg for sleep problems, and many sleep researchers suggest that doses between 0.5mg and 3mg are enough to improve sleep onset. While 9mg isn’t dangerously toxic, it’s well above what most people need and raises the likelihood of side effects, next-day grogginess, and potential longer-term concerns.

What Doses Are Actually Recommended

The NHS recommends 2mg as the standard adult dose for short-term insomnia, taken one to two hours before bed. For longer-term sleep problems, the starting dose is still 2mg, with gradual increases up to a maximum of 10mg only if lower doses aren’t working and side effects remain manageable. That makes 9mg close to the ceiling, not a starting point.

The reason lower doses often work better comes down to how melatonin functions. Your body naturally produces small amounts of this hormone as darkness falls, signaling your brain that it’s time to sleep. A supplement dose of 0.5mg to 3mg is typically enough to mimic or boost that natural signal. Higher doses don’t necessarily produce deeper or longer sleep. They can actually disrupt your sleep architecture, leaving you feeling less rested despite falling asleep faster.

Side Effects at Higher Doses

The most common side effects of melatonin at any dose are drowsiness and headaches. At higher doses like 9mg, these become more likely and more pronounced. Some people also report feeling confused and disoriented, particularly if the melatonin is still active in their system when they wake up. The Mayo Clinic warns against driving or using machinery within five hours of taking melatonin because of reduced alertness.

The “melatonin hangover” is one of the most frequent complaints from people taking high doses. Because 9mg floods your system with far more melatonin than your body produces naturally (your pineal gland makes roughly 0.1 to 0.8mg per night), the hormone can linger into the morning. This creates the opposite of what you wanted: you took melatonin to sleep better, but you wake up groggy, foggy, and sluggish.

Long-Term Use Raises Serious Questions

A large study presented at the American Heart Association’s 2025 conference found that long-term melatonin users had significantly higher rates of heart failure compared to non-users. Over five years, 5% of melatonin users developed heart failure versus 3% of the control group. Heart failure-related hospitalizations were nearly three times higher in the melatonin group (19% vs. 7%), and all-cause mortality was also elevated (8% vs. 4%). The researchers noted these associations held up even after accounting for other risk factors.

This doesn’t prove melatonin caused these outcomes, and the study didn’t separate doses. But it challenges the widespread assumption that melatonin is harmless because it’s “natural.” If you’re taking 9mg nightly over months or years, these findings are worth considering, especially since lower doses may work just as well for sleep.

Your 9mg Pill May Not Contain 9mg

Melatonin supplements aren’t regulated the same way prescription medications are, and the label on your bottle may not reflect what’s actually inside. A study analyzing commercially available melatonin products found that 40% failed to meet acceptable content standards, with actual melatonin content falling outside the 90% to 110% range of what was advertised. A separate analysis of 25 melatonin brands found contents ranging from nearly zero to 347% of the labeled dose. Only three of the 25 products contained what they claimed.

This means your 9mg tablet could contain anywhere from a few milligrams to over 30mg. That unpredictability makes high-dose melatonin riskier than it appears, because you can’t be confident in what you’re actually taking.

Drug Interactions to Watch For

At 9mg, melatonin’s interactions with other medications become more relevant. If you take blood thinners or anti-clotting supplements, melatonin can increase bleeding risk. It also affects blood sugar levels, which matters if you’re on diabetes medication. These interactions exist at lower doses too, but higher doses amplify the effect.

Children Should Not Take 9mg

If you’re considering 9mg for a child or teenager, that dose is far too high. Children ages 5 to 12 should take between 1 and 3mg, and teens should not exceed 5mg. Higher doses in children can cause daytime grogginess, nausea, vomiting, confusion, short-term depression, and an increased risk of seizures. Children can also develop tolerance to melatonin over time, leading to a cycle of escalating doses. Melatonin is not recommended at all for children under 5.

A Better Approach to Dosing

If you’re currently taking 9mg and sleeping fine with no side effects, you’re likely taking more than you need. Try gradually reducing your dose. Many people find that 1 to 3mg works just as well, sometimes better, because it more closely mirrors the body’s natural melatonin levels. Start by cutting your dose in half, then reduce further over a week or two.

Timing matters as much as dose. Taking melatonin 1 to 2 hours before your intended bedtime gives it time to work with your circadian rhythm rather than overwhelming it. If you find that even low doses of melatonin leave you groggy, the issue may not be dosing at all. It could be that your sleep problems stem from something melatonin can’t fix, like stress, irregular sleep schedules, or light exposure before bed.
