Yes, the common cold is a viral infection. More than 200 different viruses can cause it, with rhinoviruses responsible for roughly half of all cases. Other culprits include common human coronaviruses, parainfluenza viruses, adenoviruses, and enteroviruses. Because a cold is caused by a virus, not bacteria, antibiotics have no effect on it.
Why So Many Different Viruses Cause Colds
The term “cold” describes a cluster of symptoms, not a single disease caused by a single germ. Any of the 200-plus viruses that infect the upper respiratory tract and produce that familiar combination of congestion, sneezing, and sore throat gets lumped under the same name. This is also why you can catch multiple colds per year: immunity to one rhinovirus strain does nothing to protect you from the dozens of others circulating at any given time.
Rhinoviruses are the dominant players in the United States and worldwide. Common human coronaviruses (not to be confused with the one behind COVID-19) are another frequent cause, along with parainfluenza viruses and adenoviruses. RSV can produce cold-like symptoms, particularly in young children, but is classified as a separate illness.
How Cold Viruses Get Inside Your Cells
Most rhinoviruses latch onto a specific protein on the surface of cells lining your nose and throat. This protein normally helps cells communicate with each other, but the virus hijacks it as an entry point. Once the virus locks on, the outer shell of the virus physically changes shape, expanding slightly and opening tiny pores. The virus’s genetic material then slips through those pores and into the cell, where it begins making copies of itself. Within hours, newly made virus particles burst out and spread to neighboring cells, and the infection takes hold.
Your Immune System Causes Most of the Symptoms
Cold viruses do relatively little direct damage to your airways. The misery you feel is largely your own immune system’s doing. When infected cells send out chemical alarm signals, your body launches an inflammatory response that produces the symptoms you recognize.
Sore throat results from inflammatory chemicals called prostaglandins and bradykinin irritating nerve endings in the throat. Sneezing is triggered when histamine activates a nerve network in the nasal passages. Nasal congestion happens because inflammatory signals cause large veins inside the nose to dilate and swell, physically narrowing the airway. This is why decongestants and antihistamines can ease symptoms even though they do nothing to fight the virus itself: they’re counteracting your immune response, not the infection.
Typical Symptom Timeline
Cold symptoms usually appear one to three days after exposure. The first sign is often a scratchy or sore throat, followed within a day by a runny nose and sneezing. Congestion, mild body aches, and low-grade fever may develop over the next couple of days. Most colds peak around day two or three and resolve within seven to ten days. A lingering mild cough or slight congestion past the ten-day mark isn’t unusual, but symptoms that are still worsening after ten days, or that improve and then suddenly get worse, can signal a secondary bacterial infection.
How Colds Spread
Cold viruses travel primarily through respiratory droplets released when an infected person coughs, sneezes, or talks. But hand-to-hand and surface contact play a surprisingly large role. Rhinovirus survives up to three hours on hard surfaces like stainless steel, countertops, and wood. On porous materials like cotton and facial tissue, it lasts about an hour. In nasal mucus, it can remain infectious for up to 24 hours. Touching a contaminated surface and then touching your eyes, nose, or mouth is one of the most common ways to catch a cold.
Handwashing with soap is the single most effective way to remove cold viruses from your skin. If soap and water aren’t available, a hand sanitizer with at least 60 percent alcohol can kill these viruses on contact.
Why Antibiotics Don’t Work
Because colds are viral, antibiotics are useless against them. A large Cochrane review pooling data from over 1,100 participants found that people given antibiotics for a cold recovered at the same rate as those given a placebo. Worse, adults who took antibiotics were more than two and a half times as likely to experience side effects like nausea and diarrhea. Even when a cold produces thick, yellowish-green nasal discharge (which many people assume means bacteria are involved), antibiotics still showed no benefit. The routine use of antibiotics for colds is not recommended by any major medical guideline.
This matters because unnecessary antibiotic use contributes to antibiotic resistance, making these drugs less effective when you actually need them for a genuine bacterial infection.
Cold vs. Bacterial Infection
One of the most useful distinctions: viral colds produce “all over” symptoms. You get a runny nose, cough, mild body aches, fatigue, and a general sense of feeling unwell. Bacterial infections tend to be more localized. Strep throat, for instance, causes a severe sore throat concentrated in one area, often without the runny nose or cough that accompanies a cold. A bacterial ear infection produces sharp pain focused in one ear. A bacterial sinus infection typically causes intense pressure and pain around the cheeks or forehead.
A pattern worth watching for: a viral cold that seems to be improving, then suddenly worsens with a new or higher fever and new pain in a specific area. This “gets better, then gets worse” pattern often signals a secondary bacterial infection that developed on top of the original cold. Colds can sometimes lead to bacterial sinus infections, middle ear infections, or, less commonly, lung infections that do require antibiotic treatment.
What Actually Helps
Since no medication can kill a cold virus once you’re infected, treatment focuses on managing symptoms while your immune system clears the infection on its own. Rest and fluids help your body mount an effective immune response. Over-the-counter pain relievers can reduce sore throat pain and body aches. Decongestants shrink swollen nasal blood vessels to temporarily open your airways, and saline nasal sprays can thin mucus and ease congestion without medication.
Humidity matters more than most people realize. Dry air dries out nasal membranes and can worsen congestion, so a humidifier in your bedroom during a cold can make a noticeable difference, particularly in winter when indoor air is driest. Warm liquids like broth or tea soothe throat irritation and help loosen congestion. None of these measures shorten the cold, but they can make the seven to ten days considerably more bearable.
