After earning their medical degree, aspiring medical examiners complete at least two years of residency training in anatomic pathology, where they learn to evaluate tissue specimens and perform autopsies. Following residency, they complete a 12-month fellowship in forensic pathology, working directly in a medical examiner’s office to gain hands-on experience collecting evidence, performing external examinations, and conducting autopsies. The full path from undergraduate degree to practicing forensic pathologist typically takes 13 or more years of post-secondary education and training.

To practice, medical examiners must pass a board certification exam administered by the American Board of Pathology. The forensic pathology exam is a full-day, computer-based test covering 275 questions across written, practical, and virtual microscopy sections. It tests knowledge spanning injury pattern interpretation, toxicology, natural disease pathology, molecular biology, forensic odontology, physical anthropology, and legal topics like jurisprudence.

What a Medical Examiner Actually Does

The core job is determining why and how someone died. When a death is sudden, unexplained, violent, or potentially caused by drugs or foul play, the medical examiner takes jurisdiction. They perform autopsies, which remain the most accurate method for establishing cause of death. This involves a detailed external and internal examination of the body, collection of biological specimens (blood, urine, and vitreous humor from the eye), and interpretation of toxicology results alongside the circumstances surrounding the death.

The work is deeply medical but also deeply legal. Medical examiners certify the cause and manner of death on death certificates, listing the specific drugs, injuries, or diseases responsible. In suspected drug deaths, for example, they account for how drugs are metabolized in the body and how concentrations shift after death due to a process called postmortem redistribution. A toxicology result alone doesn’t tell the full story. The medical examiner interprets those results in context: the person’s medical history, the scene where they were found, and the autopsy findings.

Medical examiners also frequently serve as expert witnesses in criminal and civil trials, testifying about their findings and how they reached their conclusions. Their medical credentials are central to that role. In 27 states, expert witnesses in legal proceedings are required to hold a medical license.

Medical Examiners vs. Coroners

This is the distinction that causes the most confusion. A medical examiner is required to be a physician certified in forensic pathology. A coroner, in most places, is not a physician and has no training in medicine, forensic medicine, or forensic science. Coroners are often elected officials whose qualifications vary widely by state and county. In some jurisdictions, there are no medical education requirements at all to serve as coroner.

The system a given area uses depends entirely on the state. According to CDC data, states like Connecticut, Delaware, Maine, Maryland, Massachusetts, New Hampshire, New Mexico, North Carolina, Oklahoma, and Oregon use a centralized statewide medical examiner system. Other states, like Idaho, Indiana, Kansas, Nebraska, and Nevada, rely on county-based coroner systems. Many states, including California, New York, Illinois, and Ohio, use a mixed system where some counties have medical examiners and others have coroners. Some states with coroner systems also maintain a state medical examiner office that can provide oversight or assistance.

The practical difference matters. In a coroner-based system, the elected coroner may need to contract with a forensic pathologist to perform autopsies and interpret medical evidence. In a medical examiner system, the person making those determinations is the physician themselves.

A Profession Facing a Shortage

Despite the critical nature of the work, the United States has a significant shortage of forensic pathologists. As of 2024, roughly 860 forensic pathologists are practicing in the country, while approximately 1,400 are needed to adequately support medicolegal death investigations. That gap means many offices are understaffed, with individual pathologists handling caseloads well above recommended limits. The shortage is driven partly by the lengthy training pipeline and partly by compensation that often lags behind other medical specialties requiring similar levels of education.
