Advil is not the same as aspirin. They are two different drugs with different active ingredients. Advil contains ibuprofen, while aspirin (sold under brands like Bayer and Ecotrin) contains acetylsalicylic acid. Both belong to the broader family of NSAIDs (nonsteroidal anti-inflammatory drugs), which is why they’re often confused, but they work differently in your body and carry distinct risks.
Different Drugs, Same Family
Think of NSAIDs as a category, like “citrus fruit.” Oranges and lemons are both citrus, but they’re not the same thing. Similarly, ibuprofen and aspirin both reduce pain, fever, and inflammation by blocking the same general group of enzymes, but they do so in chemically distinct ways. Aspirin is derived from salicylic acid, placing it in a sub-class called salicylates. Ibuprofen belongs to a different chemical sub-class entirely.
This distinction matters because it affects how long each drug works, what unique benefits it offers, and which side effects you’re more likely to experience.
How They Work Differently in Your Body
Both drugs block enzymes called COX-1 and COX-2, which are involved in producing chemicals that trigger pain and inflammation. The critical difference is how they do it. Aspirin permanently disables these enzymes. Once aspirin binds to an enzyme, that enzyme is shut down for good, and your body has to make new ones. Ibuprofen, on the other hand, temporarily blocks the same enzymes. Its effects wear off as the drug leaves your system.
This is why aspirin has a unique role in heart health that ibuprofen doesn’t share. Because aspirin permanently disables the enzyme in platelets (the blood cells responsible for clotting), those platelets can never clot normally again for the rest of their roughly 10-day lifespan. That’s the basis for low-dose aspirin therapy in people at risk for heart attacks and strokes. Ibuprofen’s clot-blocking effect, by contrast, lasts only 6 to 8 hours before platelet function fully recovers.
Pain Relief: How They Compare
For everyday aches, headaches, and fevers, both drugs are effective. They work on similar timelines, typically kicking in within 30 to 60 minutes. Where they diverge is duration. A standard dose of ibuprofen provides relief for about 4 to 6 hours. Aspirin’s pain-relieving effect is comparable in duration, though its impact on platelet function lasts far longer (72 to 96 hours for a single dose to fully clear).
In practice, most people reaching for an over-the-counter pain reliever will find ibuprofen and aspirin similarly effective for minor pain. The choice between them usually comes down to other health factors rather than which one kills pain better.
Stomach and Kidney Risks
Both drugs can irritate the digestive tract. NSAIDs as a class can cause ulcers, bleeding, or even holes in the stomach or intestinal lining. These problems can develop at any point during use, sometimes without warning symptoms. The risk climbs for people who are older, take NSAIDs for extended periods, smoke, or drink three or more alcoholic beverages a day.
Aspirin carries an additional bleeding concern because of its permanent effect on platelets. Pooled data from large clinical trials shows aspirin use is associated with a 58% increase in major gastrointestinal bleeding and a 31% increase in bleeding inside the skull compared to non-use. This is one reason the U.S. Preventive Services Task Force now recommends against starting daily aspirin for heart disease prevention in adults 60 and older.
Both drugs can also affect kidney function, particularly in people who already have kidney disease or are dehydrated. Signs of kidney trouble include cloudy or bloody urine and painful urination.
Aspirin’s Role in Heart Protection
Low-dose aspirin (typically 81 mg per day) has long been used to prevent heart attacks and strokes in people at elevated cardiovascular risk. Current guidelines from the USPSTF are more cautious than they once were. For adults aged 40 to 59 with a 10% or greater 10-year risk of cardiovascular disease, the decision to start daily aspirin is considered individual, and the net benefit is described as small. For adults 60 and older, the task force recommends against starting aspirin for primary prevention because the bleeding risks tend to outweigh the benefits as you age.
Ibuprofen has no equivalent role in heart protection. Its temporary effect on platelets simply doesn’t provide the sustained anti-clotting action that makes aspirin useful for cardiovascular prevention.
Why You Shouldn’t Take Them Together Casually
If you take daily low-dose aspirin for your heart and then pop an Advil for a headache, the ibuprofen can actually block aspirin’s heart-protective effect. Both drugs compete for the same binding site on the COX enzyme. When ibuprofen gets there first, it physically prevents aspirin from permanently disabling the enzyme. The FDA has flagged this interaction specifically: ibuprofen can interfere with aspirin’s ability to prevent dangerous blood clots.
If you’re on a daily aspirin regimen and occasionally need ibuprofen for pain, timing matters. Taking aspirin at least 30 minutes before ibuprofen, or waiting at least 8 hours after ibuprofen to take aspirin, can help minimize this interference. But this is a conversation worth having with your pharmacist or doctor, since the stakes involve heart protection.
Children Should Not Take Aspirin
This is one of the sharpest safety differences between the two drugs. Aspirin is linked to Reye’s syndrome, a rare but potentially fatal condition affecting the brain and liver, in children and teenagers who have the flu or chickenpox. Because of this risk, aspirin should not be given to anyone under 18 for general pain or fever relief. Ibuprofen (including children’s Advil and Motrin) is considered a safe alternative for pediatric use at appropriate doses, along with acetaminophen (Tylenol).
The only exception is children with specific chronic conditions like Kawasaki disease, who may need aspirin-containing medications under medical supervision.
