But there’s an important catch: Original Alka-Seltzer also contains aspirin, which many people don’t realize and which carries real risks for something you might take casually for an upset stomach.

What’s Actually in Alka-Seltzer Original

Each effervescent tablet contains three active ingredients: 1,916 mg of sodium bicarbonate, 1,000 mg of citric acid, and 325 mg of aspirin. There are no inactive ingredients at all. When you drop the tablet in water, the sodium bicarbonate and citric acid react to create that familiar fizz. Once you drink it, the sodium bicarbonate neutralizes hydrochloric acid in your stomach, producing salt, water, and carbon dioxide gas.

That 325 mg of aspirin is the same dose you’d find in a standard pain relief tablet. It’s included because Alka-Seltzer Original was designed to treat headaches and body aches alongside stomach discomfort. If your only symptom is heartburn, you’re taking a pain reliever you don’t need.

How It Compares to Other Antacids

The speed advantage of Alka-Seltzer is significant. Effervescent sodium bicarbonate begins raising stomach pH in about 6 seconds, compared to 30 seconds or more for water alone. Calcium carbonate antacids like Tums typically take 5 to 30 minutes to start working in the stomach.

Duration is where things get more interesting. Chewable tablets and effervescent bicarbonate formulas keep working in the esophagus for 40 to 45 minutes and in the stomach for 100 to 180 minutes. Calcium carbonate lasts about 60 minutes. So Alka-Seltzer not only kicks in faster but also provides longer-lasting relief than a standard Tums. Both, however, are far shorter-acting than acid reducers like famotidine (Pepcid), which can suppress acid for up to 9 hours but takes over an hour to start working.

This matters most if you have high blood pressure, heart failure, or kidney disease, or if you’re on a sodium-restricted diet. Even for people without these conditions, frequent use adds a surprising amount of sodium to your daily intake.

Who Should Avoid It

Children and teenagers should never take Alka-Seltzer Original. The aspirin it contains has been linked to Reye’s syndrome, a rare but serious condition that causes swelling in the liver and brain. The risk is highest when a child has the flu or chickenpox. The Mayo Clinic specifically names Alka-Seltzer as one of the “unexpected products” that contain aspirin.

People with chronic kidney disease face a double problem. Aspirin at doses above 325 mg per day can reduce blood flow to the kidneys and worsen kidney damage. Sodium bicarbonate and other antacid ingredients can also build up in the body when kidney function is impaired. The National Kidney Foundation advises people with kidney disease to avoid NSAIDs like aspirin, especially if they also take blood pressure medications or diuretics.

Aspirin-Free Alternatives for Pure Heartburn

If heartburn is your only complaint, you have better options that skip the aspirin entirely. Alka-Seltzer makes heartburn-specific products that contain sodium bicarbonate and citric acid without aspirin. These give you the same fast-acting acid neutralization without the bleeding risk.

Calcium carbonate tablets (Tums, Rolaids) are another straightforward choice. They’re slower to kick in but widely available, low in sodium, and carry fewer safety concerns for most people. For heartburn that keeps coming back several times a week, acid reducers like famotidine work on a different mechanism entirely, blocking acid production rather than neutralizing it after the fact. They take longer to start but control symptoms for hours.

Occasional heartburn after a big meal is normal. Heartburn that shows up more than twice a week, disrupts your sleep, or doesn’t respond well to over-the-counter antacids could signal gastroesophageal reflux disease (GERD), which benefits from a different treatment approach.
