Is Augmentin Stronger Than Amoxicillin? Key Differences

Augmentin is not stronger than amoxicillin in the traditional sense, but it does work against a wider range of bacteria. Both drugs contain the exact same antibiotic, amoxicillin, at the same doses. The difference is that Augmentin adds a second ingredient, clavulanic acid, which disables a defense mechanism that certain bacteria use to survive amoxicillin. That makes Augmentin effective in situations where plain amoxicillin would fail, but it also comes with more side effects.

Why Augmentin Covers More Bacteria

Many bacteria produce enzymes called beta-lactamases that break apart amoxicillin’s core structure before it can do its job. This is one of the most common ways bacteria resist penicillin-type antibiotics. In hospital studies, nearly half of certain bacterial species test as intermediate or resistant to amoxicillin, with the vast majority of that resistance driven by these protective enzymes.

Clavulanic acid solves this problem by acting as a decoy. It binds to the bacterial enzyme’s active site the same way amoxicillin would, then undergoes a chemical reaction that permanently locks the enzyme in place. Once the enzyme is disabled, it can no longer destroy amoxicillin molecules, and the antibiotic works as intended. Think of it as a bodyguard: clavulanic acid doesn’t kill bacteria on its own, but it protects amoxicillin from being neutralized.

This extends Augmentin’s reach to bacteria that routinely shrug off plain amoxicillin, including Staph aureus (though not MRSA), Haemophilus species, E. coli, Klebsiella pneumoniae, Proteus species, and certain anaerobic bacteria like Bacteroides. For infections likely caused by these organisms, plain amoxicillin simply won’t be reliable.

When Doctors Choose One Over the Other

Plain amoxicillin remains the first-line choice for straightforward infections where beta-lactamase resistance isn’t a major concern. Strep throat is the classic example: group A Streptococcus doesn’t produce beta-lactamase, so adding clavulanic acid provides no benefit. The same logic applies to many simple ear infections in children and uncomplicated urinary tract infections caused by susceptible bacteria.

Augmentin gets the nod when the likely bacteria are known to carry resistance enzymes. The Infectious Diseases Society of America recommends amoxicillin-clavulanate over plain amoxicillin as the preferred first-line treatment for acute bacterial sinus infections in both children and adults. The American Academy of Otolaryngology endorses the same recommendation. That’s because the bacteria most commonly responsible for sinus infections, particularly Haemophilus influenzae, frequently produce beta-lactamases.

Other situations where Augmentin is typically preferred include bite wounds (animal or human), certain skin infections, lower respiratory tract infections, and more complicated ear infections that haven’t responded to initial treatment. It’s also recommended in specific outbreak scenarios involving group A Streptococcus carriers, such as community outbreaks of rheumatic fever or invasive strep infections.

Augmentin Causes More Side Effects

The added coverage comes at a cost to your gut. In pediatric studies, the rate of antibiotic-associated diarrhea was 19.8% for Augmentin compared to 8.1% for amoxicillin alone. That means roughly one in five people taking Augmentin can expect loose stools or diarrhea, compared to fewer than one in twelve on plain amoxicillin. Clavulanic acid is the culprit. It disrupts the balance of bacteria in your intestines more aggressively than amoxicillin by itself.

Nausea and stomach discomfort are also more common with Augmentin. Taking it at the start of a meal helps reduce these effects. If you’ve been prescribed Augmentin and diarrhea becomes severe or persistent, that’s worth a call to your prescriber, as it occasionally signals a more serious gut infection.

The Amoxicillin Dose Is the Same

A point that often gets lost: the amoxicillin component in Augmentin is dosed identically to standalone amoxicillin. An Augmentin 875 mg tablet contains 875 mg of amoxicillin plus 125 mg of clavulanic acid. A 500 mg tablet contains 500 mg of amoxicillin plus 125 mg of clavulanic acid. The clavulanic acid stays fixed at 125 mg per tablet regardless of the amoxicillin strength. For adults, a typical course for severe or respiratory infections is one 875 mg tablet every 12 hours, while less severe infections call for one 500 mg tablet every 12 hours or one 250 mg tablet every 8 hours.

This matters because it means Augmentin doesn’t “hit harder” against bacteria that are already susceptible to amoxicillin. If a bacterium doesn’t produce beta-lactamase, the clavulanic acid is doing nothing useful, and you’re just taking on extra side effects for no clinical benefit.

Stronger Isn’t Always Better

The instinct to want the “stronger” antibiotic is understandable, but antibiotics aren’t like pain relievers where a higher-powered option always gives a better result. The best antibiotic is the narrowest one that still kills the bacteria causing your infection. Using a broader-spectrum drug when a narrower one would work contributes to antibiotic resistance in your body’s own bacterial population, increases side effects, and provides no faster recovery.

When your doctor prescribes plain amoxicillin, it’s not a weaker choice. It means the infection you have is likely caused by bacteria that don’t need the extra protection-busting power of clavulanic acid. When they prescribe Augmentin, it’s because the suspected bacteria have a track record of resisting amoxicillin on its own. Both prescriptions are the right tool for different jobs.
