Baclofen is not considered a classically addictive drug. It does not produce the strong dopamine surges in the brain’s reward center that make substances like opioids or stimulants so easy to abuse. It is not classified as a controlled substance by the DEA. However, your body can become physically dependent on baclofen with regular use, and stopping it abruptly can trigger serious, even life-threatening withdrawal symptoms. That distinction between “habit-forming” and “physically dependent” matters a lot if you’re taking this medication.
How Baclofen Affects the Brain
Baclofen works on a specific type of receptor in the nervous system that calms overactive nerve signals. This is why it’s primarily prescribed for muscle spasticity. Unlike drugs that flood the brain with feel-good chemicals, baclofen actually inhibits dopamine transmission at standard therapeutic doses. Preclinical studies show that at normal doses, it has no detectable effect on the key reward regions of the brain, including the nucleus accumbens and the ventral tegmental area, the areas most associated with addiction.
There is one wrinkle. Some research suggests baclofen’s effects on dopamine are dose-dependent: low doses may slightly increase dopamine activity, while higher doses suppress it. This pattern has been observed in animal studies, cell preparations, and humans. It may help explain why a small number of people do experience mild euphoria at certain doses, even though the drug isn’t designed to produce a high.
Physical Dependence Is Real
Even though baclofen has low addictive potential, your body adapts to its presence over time. If you’ve been taking it regularly and stop suddenly, the nervous system can rebound dramatically. Withdrawal symptoms can appear within 48 hours of abrupt discontinuation and may include agitation, insomnia, confusion, hallucinations, seizures, high fever, and a sharp increase in muscle spasticity. In severe cases, withdrawal resembles alcohol or sedative withdrawal, with delirium, dangerously elevated heart rate, and high blood pressure.
The risks are significantly higher for people who receive baclofen through an implanted pump that delivers the drug directly into spinal fluid. The FDA places a boxed warning (its strongest safety alert) on intrathecal baclofen, noting that abrupt interruption of pump delivery has caused rhabdomyolysis (severe muscle breakdown), multiple organ failure, and death. In the first nine years after this delivery method reached the market, 27 withdrawal cases were reported and six patients died. Most symptoms appeared within hours to a few days after the drug was interrupted. Even oral baclofen withdrawal, while generally less severe, can require intensive care in some cases.
Can People Misuse Baclofen?
Yes, though it’s uncommon compared to traditional drugs of abuse. Reports of recreational baclofen use have increased in recent years, particularly among people already struggling with alcohol or other substance use. At doses up to about 60 mg (within or just above the typical prescribed range), some users report euphoria. Acute ingestion of around 200 mg over a few hours can cause lethargy, confusion, and sedation. Doses above 200 mg carry risks of seizures, cardiac arrhythmias, respiratory failure, and death, with significantly higher rates of ICU admission.
One documented case involved a person who began combining baclofen with alcohol and found the combined effects pleasurable. Over six months, this escalated to taking up to 150 mg of baclofen two to three times per week alongside heavy daily drinking. Cases like this are relatively rare, but they illustrate that baclofen’s abuse potential isn’t zero, especially in people with a history of substance use disorders.
Baclofen as an Addiction Treatment
Ironically, baclofen is being studied and used off-label as a treatment for alcohol use disorder, precisely because of its ability to dampen the brain’s reward circuitry. A large review of clinical trials found that baclofen likely reduces the risk of relapsing to any drinking by about 13% compared to placebo and increases the percentage of days a person stays abstinent by roughly 9 additional days. These benefits were most consistent in people who had already gone through detox.
The medication did not significantly reduce the number of heavy drinking days or the number of drinks consumed on days when people did drink. Common side effects in these trials included fatigue, dizziness, drowsiness, dry mouth, tingling sensations, and muscle stiffness. Dropout rates were similar between baclofen and placebo groups, suggesting the side effects are manageable for most people.
Stopping Baclofen Safely
The single most important thing to know about baclofen is that you should never stop taking it abruptly after regular use. A gradual taper, reducing your dose in small steps over days to weeks, gives your nervous system time to readjust. The exact tapering schedule depends on how much you’ve been taking and for how long, so this is something to work out with whoever prescribed the medication.
Early warning signs of withdrawal to watch for include a return of your original spasticity symptoms, itching, low blood pressure, and tingling. If withdrawal progresses, it can escalate to confusion, hallucinations, high fever, and seizures. Anyone using an intrathecal pump should be especially vigilant about keeping refill appointments and monitoring the device for alarms or malfunctions, since pump failures are the most common cause of life-threatening baclofen withdrawal.
In short, baclofen is not habit-forming in the way most people mean when they ask the question. It doesn’t create cravings or compulsive drug-seeking behavior in the vast majority of users. But physical dependence develops with chronic use, and the withdrawal syndrome is genuinely dangerous. Treat it with the same respect you’d give any medication that your body learns to rely on.
