Is Being Vegan Healthier Than Eating Meat? The Evidence

A vegan diet lowers your risk of heart disease, type 2 diabetes, and early death compared to a meat-based diet, but it also introduces real nutritional gaps that require attention. The honest answer is that a well-planned vegan diet outperforms a typical omnivorous diet on most major health markers, while a poorly planned one can leave you worse off in specific ways. Quality matters more than the label.

Heart Disease and Cardiovascular Risk

This is where plant-based eating shows its clearest advantage. People who eat the most plant-based foods have about a 16% lower risk of cardiovascular disease (including heart attacks, strokes, and heart failure) compared to those who eat the least. The Adventist Health Study-2, which tracked over 88,000 people, found that vegetarians had a 27% lower risk of dying from ischemic heart disease at age 65 compared to non-vegetarians. For general cardiac deaths, the risk was 25% lower at the same age.

The mechanism is straightforward: plant-heavy diets tend to lower blood pressure, reduce LDL cholesterol, and decrease chronic inflammation. These are the three biggest controllable drivers of heart disease. Meat-heavy diets, particularly those rich in processed and red meat, push all three in the wrong direction.

Type 2 Diabetes Risk

A healthy plant-based diet is associated with a 24% lower risk of developing type 2 diabetes compared to a diet low in plant foods. The protection comes largely through lower body weight and smaller waist circumference, which together account for more than half the risk reduction. Lower blood sugar levels, lower triglycerides, and reduced inflammation explain most of the rest.

There’s an important caveat here. A plant-based diet built around refined grains, sugary drinks, and processed snacks actually increases diabetes risk by 37%. Swapping a steak for white bread and chips is not a health upgrade. The benefits come from whole grains, legumes, fruits, vegetables, and nuts, not simply from removing meat.

Longevity and Overall Mortality

Vegetarians in the Adventist Health Study-2 had an 11% lower risk of dying from any cause at age 65 compared to meat eaters. The advantage was especially striking for certain causes of death: a 49% lower risk of dying from diabetes, a 48% lower risk of dying from kidney failure, and a 43% lower risk of dying from infectious disease.

By age 85, though, the gap narrowed considerably. Overall mortality risk was nearly identical between the two groups. And older vegetarians actually showed higher rates of death from neurological conditions. Stroke mortality was 17% higher among vegetarians at age 85, and dementia mortality was 13% higher. The reasons aren’t fully understood, but possible nutrient shortfalls (particularly B12 and omega-3 fatty acids, both critical for brain health) may play a role over decades.

Where Vegan Diets Fall Short

Vitamin B12

B12 deficiency is the most well-documented risk of a vegan diet. Among Czech vegans studied, about 17% were clinically deficient. But the numbers split dramatically by supplement use: only 5.6% of those who took B12 supplements regularly were deficient, compared to 53% of those who didn’t supplement at all. For comparison, just 3.5% of non-vegans in the same study were deficient. B12 is essential for nerve function and red blood cell production, and deficiency over years can cause irreversible neurological damage. If you’re vegan, a B12 supplement isn’t optional.

Iron

About 46% of vegans had low iron stores compared to 33% of non-vegans, based on ferritin levels. When combined with low hemoglobin (a sign of actual anemia rather than just depleted reserves), roughly 12% of vegans showed possible deficiency versus 7% of non-vegans. Plant-based iron is harder for your body to absorb than the type found in meat, but pairing iron-rich foods with vitamin C significantly improves uptake.

Bone Health

Vegans have roughly 6% lower bone mineral density at the lumbar spine than omnivores, and about 6% lower density at the femoral neck (the part of the hip most vulnerable to fracture). Lacto-ovo vegetarians fare better, with only about a 2% reduction. The effect is more pronounced in white women than in other groups. Calcium, vitamin D, and protein intake all tend to be lower on a vegan diet, and each one matters for bone strength. This doesn’t guarantee fractures, but it does mean vegans need to be intentional about calcium-rich foods, vitamin D supplementation, and weight-bearing exercise.

Gut Health

Plant-based diets consistently promote a more diverse gut microbiome. The fiber and complex carbohydrates in plants feed beneficial bacteria that produce short-chain fatty acids, compounds that strengthen your gut lining, reduce inflammation, and may improve immune function. Meat-heavy, fiber-poor diets tend to favor bacteria associated with inflammation. This is one area where the vegan advantage is relatively clear-cut, though any diet high in whole plant foods (including one that also includes some meat) produces similar benefits.

Is a Vegan Diet Safe at Every Age?

The American Dietetic Association’s position is that well-planned vegan diets are nutritionally adequate for all stages of life, including pregnancy, breastfeeding, infancy, childhood, and adolescence. The key phrase is “well-planned.” That means reliable sources of B12, iron, calcium, vitamin D, omega-3 fats, zinc, and iodine, either through fortified foods or supplements. For children especially, calorie and protein density needs attention since plant foods tend to be less calorie-dense, and growing bodies have high energy demands relative to their size.

Extremely restrictive versions of plant-based eating, like fruitarian or raw food diets, have not been studied enough in children to be considered safe, and major nutrition organizations do not recommend them for kids.

The Bottom Line on Diet Quality

The research consistently points to the same pattern: the healthiest diets are rich in whole plant foods, regardless of whether they include meat. A vegan who lives on processed snacks and refined carbohydrates has a higher diabetes risk than an omnivore who eats plenty of vegetables, legumes, and whole grains. And a vegan who neglects B12 and calcium may face neurological and bone problems that a moderate meat eater never encounters.

Where a well-planned vegan diet genuinely excels is in cardiovascular protection and metabolic health. If your primary concern is heart disease, diabetes, or maintaining a healthy weight, shifting toward plant-based eating offers measurable benefits. But those benefits depend on what replaces the meat. Whole foods, consistent supplementation of the nutrients plants can’t provide, and attention to protein and calorie needs are what separate a health-promoting vegan diet from one that trades old problems for new ones.
