Most people feel little or no pain during cataract surgery. The procedure uses local anesthesia, either numbing drops applied to the eye’s surface or an injection near the eye, so you stay awake but shouldn’t feel sharp pain. The entire operation typically takes 15 to 30 minutes, and full recovery takes about four weeks, with minimal discomfort throughout that period.
What You Actually Feel During Surgery
Because your eye is numbed, you won’t feel the surgeon’s instruments cutting or working inside the eye. What you will notice is light. Between 80 and 100 percent of patients retain some light perception in the operated eye during the procedure, and many see flashes, colors, shifting brightness, or even the movement of instruments and the surgeon’s hands. These visual sensations are normal, but roughly 3 to 16 percent of patients find them frightening, which can spike anxiety and make the experience feel more uncomfortable than it physically is.
You may also feel a mild pressure sensation or awareness that something is happening in or around the eye. This is distinct from pain. If your surgeon uses an injection-based nerve block rather than topical drops, the injection itself can cause a brief sting or pressure, but it fades quickly. Knowing what to expect visually and physically before you go in makes a real difference in how comfortable the experience feels.
Pain Levels for First Eye vs. Second Eye
If you need both eyes done, the second surgery tends to feel worse. In one study, only 35 percent of patients reported any pain during their first cataract surgery, while 85 percent reported pain during the second. Among patients who had both eyes treated, 58 percent said the second surgery was more painful than the first.
The reasons aren’t entirely clear, but the second procedure also tends to take slightly longer. Timing matters too: patients who had their second surgery within two weeks of the first reported significantly more pain both during and the day after the operation. Waiting longer between procedures appears to reduce that gap. Women, younger patients, and those with higher education levels also tend to report higher pain scores, possibly reflecting differences in pain perception or willingness to report discomfort.
The First Few Days After Surgery
Some mild irritation is normal and usually settles within one to two days. The most common sensation is a gritty, “something in my eye” feeling, similar to having sand or an eyelash stuck under your lid. This foreign body sensation comes from a few overlapping causes: the small incision in the cornea disrupts the eye’s nerve fibers, the surface dries out more easily after surgery, and the preservatives in medicated eye drops can irritate the outer layer of the eye. Exposure to the bright microscope light during surgery also contributes to temporary surface irritation.
For most patients without pre-existing dry eye, this grittiness peaks in the first few days and fades within about ten days. If you already had dry eye before surgery, your symptoms may flare and take up to two months to settle back down. Your surgeon will prescribe anti-inflammatory eye drops to control swelling and discomfort during this window. Over-the-counter lubricating drops (artificial tears) can help with the dryness and gritty feeling as well.
Pressure Spikes in the First 24 Hours
Some patients experience a temporary rise in pressure inside the eye after surgery. This increase typically peaks three to seven hours after the procedure and resolves within the first 24 hours. When it happens, it can cause a deep ache or headache-like sensation around the eye. Your surgical team usually checks your eye pressure before sending you home specifically to catch this. If pressure climbs unusually high or persists, it can cause corneal swelling and more significant pain, but this is uncommon and treatable with pressure-lowering drops.
Laser-Assisted vs. Traditional Surgery
Some surgeons offer femtosecond laser-assisted cataract surgery as a premium alternative to the standard manual technique. A large randomized trial comparing the two found no meaningful difference in patient-reported quality of life or vision outcomes between them. The laser version automates certain steps of the procedure, but it doesn’t translate into a noticeably more comfortable experience or faster recovery for the patient.
When Pain Signals a Problem
Mild irritation that gradually improves is normal. Pain that gets worse instead of better is not. Severe pain developing two to five days after surgery is a hallmark of endophthalmitis, a serious infection inside the eye. It typically comes with increasing redness around the cornea, swelling, discharge, and worsening vision. A similar-looking reaction called toxic anterior segment syndrome can appear even sooner, caused by contaminated solutions used during the procedure rather than bacteria.
The general rule: worsening vision, increasing pain, growing redness, swelling, or discharge after the first day or two should prompt an urgent call to your surgeon. Slight discomfort is expected. Actual pain, the kind that makes you reach for medication or keeps you up at night, is not part of normal healing and needs evaluation quickly. Catching infections or inflammatory reactions early makes a significant difference in outcomes.
