Chewing on clothes can be related to autism, but it is not, on its own, a sign that someone is autistic. Clothing chewing is a form of oral sensory seeking, and while sensory differences are extremely common in autistic people (affecting an estimated 69% to 93% of children and adults on the spectrum), this behavior also shows up in children with anxiety, ADHD, sensory processing difficulties unrelated to autism, or sometimes no developmental condition at all.
Understanding why someone chews on clothing matters more than the behavior itself. The cause shapes whether it’s something to monitor, something to address, or simply a phase.
Why Chewing Feels Regulating
The jaw is packed with sensory receptors in the muscles, joints, and ligaments. When you chew, those receptors send a steady stream of feedback to the brain, a type of input called proprioception. This input helps the nervous system organize itself. For someone whose brain has trouble processing sensory information, chewing on a shirt collar or sleeve can act like a volume knob, either dialing stimulation up when the brain is understimulated or calming things down when it’s overwhelmed.
Children who are orally hyposensitive (meaning their mouth registers less sensation than typical) often seem to always have something in their mouth: toys, pen caps, shirt collars, candy, or other objects. They aren’t being defiant or careless. Their nervous system is actively seeking the input it needs to feel grounded. One autistic author described the sensation of a “scattered” body caused by unreliable proprioceptive processing, which could be temporarily restored by engaging in repetitive physical behaviors.
When It’s Developmentally Typical
Babies and toddlers explore the world through their mouths. Nearly all infants go through a mouthing phase, and most outgrow it between 15 months and 2 years of age. Some children take longer, and that delay alone is usually not a sign of a developmental issue.
Where it starts to stand out is when a child well past toddlerhood, say age four or older, consistently chews on clothing, pencils, or other non-food items. At that point the behavior is less likely simple exploration and more likely serving a sensory or emotional function. Even then, plenty of neurotypical children chew their shirt collars when they’re anxious, bored, or concentrating. Context matters: a child who chews during stressful situations like tests may be managing anxiety, while a child who chews throughout the day across many settings may have broader sensory processing needs.
How Sensory Behaviors Fit Into an Autism Diagnosis
The current diagnostic criteria for autism (DSM-5) explicitly include sensory differences as one of the features under the “restricted and repetitive behaviors” category. The specific language describes “hyper or hypo reactivity to sensory input or unusual interest in sensory aspects of the environment.” Chewing on clothes could fall under this umbrella as a form of sensory seeking.
But a diagnosis requires a pattern of traits across two broad areas: social communication differences and restricted or repetitive behaviors. No single behavior qualifies on its own. If your child chews on clothing and also shows differences in eye contact, social reciprocity, flexibility with routines, or imaginative play, those pieces together might point toward an evaluation. If clothing chewing is the only thing you’ve noticed, autism is much less likely to be the explanation.
Chewing Versus Pica
It’s worth distinguishing between chewing on objects and actually eating non-food items, a condition called pica. Pica involves an appetite for things like dirt, paper, chalk, or fabric, where the person swallows the material rather than just chewing on it. In autistic individuals, pica sometimes stems from the same sensory-seeking drive. In other cases, it reflects difficulty distinguishing food from non-food items, or it can be triggered by genuine nutritional deficiencies like low iron or zinc.
Chewing on a shirt sleeve without swallowing fabric is different from pica and generally carries fewer health risks, though it’s not entirely without consequences.
Dental and Health Risks of Chronic Chewing
Persistent chewing on non-food objects can cause real damage over time. The most common problems are dental: cracked, chipped, or worn-down front teeth, loss of enamel, and increased tooth sensitivity. Children with braces face a higher risk of root shortening or tooth loss. Chronic chewing also raises the likelihood of developing bruxism, the habitual grinding or clenching of teeth, even during sleep.
Beyond the teeth, repetitive chewing can cause sore or torn gum tissue, canker sores, bleeding, and scarring inside the mouth. Bacteria from clothing or objects can spread to other parts of the body through damaged oral tissue. These risks are why finding safer alternatives is worth pursuing, regardless of the underlying cause.
Practical Alternatives That Work
Simply telling a child to stop chewing rarely helps, because the behavior is serving a regulatory purpose. Punishing it can actually increase stress, which increases the need for self-regulation, which increases the chewing. The more effective approach is twofold: figure out what’s driving the behavior, then redirect it toward something safer.
Start by noticing when the chewing happens. Is your child bored and understimulated? Overwhelmed and anxious? Trying to focus? The pattern tells you what the nervous system needs. From there, occupational therapists typically recommend strategies like:
- Chewable jewelry or tools: Silicone chew necklaces and pencil toppers are designed to withstand heavy chewing and give the same proprioceptive input as clothing without the dental damage.
- Crunchy or chewy snacks: Carrots, pretzels, dried fruit, or gum can satisfy oral sensory needs through food. Offering these during high-demand times like homework can reduce clothing chewing significantly.
- Other sensory input: Deep pressure (like a weighted blanket), physical movement (jumping, swinging), or tactile tools (putty, textured fidgets) can meet the same regulatory need through a different sensory channel.
An occupational therapist can help identify specific sensory patterns and build a plan tailored to your child. This is especially valuable if the chewing is intense, happens across many settings, or comes alongside other sensory differences.
What Persists Into Adulthood
Oral sensory seeking is not just a childhood behavior. Many autistic adults chew on pens, ice, gum, or the insides of their cheeks as a way to regulate throughout the day. The need for proprioceptive input through the jaw doesn’t necessarily disappear with age. Adults are often better at choosing less visible outlets or recognizing when they need a break, but the underlying sensory wiring stays the same.
