Is Codeine the Same as Oxycodone? Not Quite

Codeine and oxycodone are not the same drug. They are both opioid painkillers, but oxycodone is roughly 10 times stronger than codeine on a milligram-for-milligram basis. They differ in chemical structure, how your body processes them, their legal classifications, and the conditions they’re typically prescribed for.

How Their Strength Compares

The simplest way to understand the difference is through equianalgesic dosing, which measures how much of each drug you’d need for the same level of pain relief. To match the effect of 30 mg of oral morphine, you’d need about 200 mg of codeine but only 20 mg of oxycodone. That makes oxycodone approximately 10 times more potent than codeine per milligram.

This potency gap is reflected in how the drugs bind to opioid receptors in the brain. Oxycodone binds to the mu-opioid receptor (the main target for pain relief) with moderate strength, in the same general range as morphine. Codeine, on the other hand, binds very weakly to that receptor on its own. Most of codeine’s painkilling effect actually comes from what your body converts it into.

How Your Body Processes Each Drug

Codeine is what pharmacologists call a prodrug. It has little pain-relieving activity by itself. After you swallow it, a liver enzyme called CYP2D6 converts a portion of the codeine into morphine, and that morphine is what actually reduces your pain. This matters because roughly 5 to 10 percent of people of European descent (and varying percentages in other populations) have a genetic variation that makes this enzyme sluggish or nonfunctional. If you’re one of those “poor metabolizers,” codeine may barely work for you at all.

Oxycodone is also processed by CYP2D6, but it is not as completely dependent on that enzyme for its effects. Oxycodone is active on its own before any liver conversion takes place. So while genetic differences in CYP2D6 can still influence how you respond to oxycodone, the impact is less dramatic than with codeine.

In terms of timing, immediate-release oxycodone reaches peak blood levels in about 1.6 hours and has an elimination half-life of roughly 3.2 hours, meaning half the drug is cleared from your system in that time. Codeine works on a similar timeline, with effects generally peaking within one to two hours and lasting around four to six hours per dose.

What Each Drug Is Prescribed For

Codeine is considered a milder opioid and is most often prescribed for mild to moderate pain or as a cough suppressant. You’ll usually find it combined with acetaminophen in products like Tylenol with Codeine (Tylenol #3, for example, contains 30 mg of codeine and 300 mg of acetaminophen). Codeine tablets typically range from 15 to 60 mg per dose.

Oxycodone is reserved for moderate to severe pain. It’s available in immediate-release combination products like Percocet (oxycodone plus acetaminophen) and in extended-release formulations like OxyContin for around-the-clock pain management. Combination tablets typically contain 2.5 to 10 mg of oxycodone paired with 325 to 750 mg of acetaminophen. The extended-release version is dosed every 12 hours rather than every 4 to 6.

Different Legal Classifications

The DEA classifies these drugs differently based on their potential for misuse. Oxycodone in any form is a Schedule II controlled substance, the most restrictive category for drugs that have accepted medical uses. Schedule II prescriptions cannot be refilled and often require a new written prescription each time.

Codeine combination products (those containing no more than 90 mg of codeine per dosage unit, like Tylenol with Codeine) are classified as Schedule III. This reflects codeine’s lower potency and somewhat lower risk profile. Schedule III prescriptions can be refilled up to five times within six months, making them easier to manage for ongoing mild pain.

Pure codeine, however, is Schedule II, the same as oxycodone. The lower scheduling only applies to combination products with limited codeine per dose.

Side Effects and Risk Profile

Both drugs carry the core side effects of all opioids: constipation, drowsiness, nausea, and the risk of slowed breathing at high doses. Both can cause physical dependence with regular use and carry a risk of addiction.

The key difference is scale. Because oxycodone is far more potent, its risks are proportionally higher. A smaller dosing error or accidental extra tablet carries more serious consequences. Oxycodone also produces stronger euphoric effects, which contributes to its higher misuse potential and its Schedule II classification across all formulations.

Codeine’s unique risk relates to that CYP2D6 enzyme. On the opposite end of the spectrum from poor metabolizers, some people are “ultra-rapid metabolizers” who convert codeine to morphine much faster and more completely than normal. For these individuals, a standard codeine dose can produce dangerously high morphine levels. This risk is especially concerning in children, and codeine is no longer recommended for patients under 12.

Can One Be Substituted for the Other?

These drugs are not interchangeable. Swapping one for the other without adjusting the dose could leave you with inadequate pain relief (if switching from oxycodone to the same number of milligrams of codeine) or a dangerous overdose (if going the other direction). Even when doses are properly converted, cross-tolerance between opioids is incomplete, so clinicians typically reduce the calculated equivalent dose by about 50 percent when switching between them.

If codeine isn’t controlling your pain, that doesn’t automatically mean oxycodone is the next step. It could mean you’re a poor CYP2D6 metabolizer and a different mild-to-moderate opioid would work, or that a non-opioid approach would be more effective. If you’ve been prescribed one and are wondering about the other, the potency difference alone makes this a conversation worth having with your prescriber rather than assuming they’re similar enough to compare casually.
