Daxxify lasts significantly longer than Botox, roughly six months versus three to four months, but “better” depends on what matters most to you: duration, cost, speed of results, or how many areas you want treated. Both are FDA-approved neurotoxins that temporarily relax muscles to smooth wrinkles, and both work well. The differences come down to practical tradeoffs.
How Long Each One Lasts
This is where Daxxify stands out most clearly. In the large SAKURA clinical trials, the median duration of wrinkle improvement was 24 weeks, or about six months. That held consistent across repeat treatments, with the second round lasting a median of 24.1 weeks. Botox, by comparison, typically lasts three to four months based on its own pivotal trials. For someone who currently gets Botox three or four times a year, Daxxify could cut that down to two visits.
The longer duration comes from Daxxify’s formulation. Instead of using a human blood protein (albumin) as a stabilizer like Botox does, Daxxify uses a synthetic peptide to keep the toxin stable and attached to the muscle. This design appears to help the product stay active at the treatment site longer.
Speed of Results
Daxxify also has a faster onset. You can expect to see visible smoothing within one to two days after injection. Botox takes longer to kick in, with full results sometimes not appearing for 10 to 14 days. If you’re getting treated before an event or on a tight timeline, that difference matters.
Side Effects Are Similar
Both products carry the same general risks you’d expect from any neurotoxin injection: headache, temporary eyelid drooping, and injection site reactions. In the SAKURA 3 safety study of nearly 2,700 patients, Daxxify caused headaches in about 6% of cases and eyelid drooping in 1.3%. A phase 2 study of Botox at the standard cosmetic dose reported headaches in 13% of patients, though direct head-to-head comparisons between the two products haven’t been conducted, and differences in study design make precise comparisons tricky.
Neither product showed any unusual or unexpected safety concerns. Eyelid drooping, when it occurred with Daxxify, was temporary and resolved on its own. One thing worth noting: because Daxxify lasts longer, any side effects could also persist longer than they would with Botox. If you experience unwanted muscle relaxation or asymmetry, you may need to wait it out for a longer stretch.
What Each One Can Treat
Botox has a major advantage here. It’s FDA-approved for a wide range of cosmetic and medical uses: frown lines, forehead lines, crow’s feet, chronic migraines, excessive sweating, overactive bladder, and more. Daxxify currently has only two FDA-approved uses: frown lines (the “11s” between your eyebrows) and cervical dystonia, a condition involving involuntary neck muscle contractions.
Providers do sometimes use neurotoxins off-label in other areas, but if you’re looking for a product with an established track record across multiple treatment zones, Botox has years of data and formal approvals that Daxxify hasn’t caught up to yet.
Cost Per Year May Be Comparable
Daxxify costs more per session. A unit of Daxxify runs roughly $15 to $25, while Botox averages $10 to $20 per unit. But because Daxxify lasts longer, you need fewer appointments per year. If you’re currently paying for three or four Botox sessions annually, switching to two Daxxify sessions could land you in a similar price range. The math varies depending on how many units you need and how your body metabolizes the product, so the annual cost difference is often smaller than the per-session sticker shock suggests.
Keep in mind that fewer office visits also means less time spent on appointments, less downtime, and fewer co-pays or consultation fees if your provider charges them separately.
Storage and Availability
Daxxify can be stored at room temperature before it’s mixed, which is unusual for neurotoxins. Botox requires refrigeration from the moment it arrives at a provider’s office. This isn’t something you’ll notice as a patient, but it does make Daxxify easier for clinics to stock and handle. Both products need to be used within a certain window once they’re prepared for injection.
Availability can be a practical consideration. Botox has been on the market since 2002 for cosmetic use and is offered at virtually every med spa and dermatology office. Daxxify, approved in 2022, is newer and not yet carried everywhere. You may need to call around to find a provider who offers it.
Which One Is Right for You
Daxxify is the stronger choice if your main priority is fewer appointments and longer-lasting results for frown lines. It kicks in faster, lasts about twice as long, and carries a similar safety profile. Botox is the more versatile option, with approvals across many treatment areas and decades of real-world use backing it up. It also gives you more flexibility: if you don’t like your results or experience a side effect, the shorter duration means it wears off sooner.
For someone who’s been getting Botox regularly and is happy with the results but tired of the frequency, Daxxify is worth trying. For someone new to neurotoxins or looking to treat areas beyond frown lines, Botox remains the safer starting point simply because there’s more data and more approved uses to work with.
