Is Doodling a Sign of ADHD or Just Fidgeting?

Doodling by itself is not a sign of ADHD. Nearly everyone doodles from time to time, and it doesn’t appear in any official diagnostic criteria for the condition. That said, doodling is more common among people with ADHD, and the reason has to do with how the ADHD brain regulates attention and arousal. If you doodle constantly and also struggle with focus, impulsivity, or restlessness, those broader patterns are worth paying attention to.

What the Diagnostic Criteria Actually Say

The DSM-5, the manual clinicians use to diagnose ADHD, does not mention doodling. The closest criterion is under the hyperactivity category: “Often fidgets with or taps hands or feet or squirms in seat.” Doodling could fall under the umbrella of fidgeting, but so could clicking a pen, bouncing a leg, or picking at a fingernail. None of these behaviors alone point to ADHD.

An ADHD diagnosis requires a persistent pattern of inattention, hyperactivity, or both that shows up across multiple settings (school, work, home) and meaningfully interferes with daily functioning. A single habit like doodling doesn’t meet that threshold. It’s one small piece of a much larger picture.

Why People With ADHD Doodle More

The ADHD brain tends to be underaroused in regions that control attention and alertness. Researchers at CHADD (Children and Adults with Attention Deficit/Hyperactivity Disorder) describe the increased movement seen in people with ADHD as an unconscious way to compensate for this underactivity. Physical movement, even something as small as drawing shapes on a notepad, appears to increase dopamine and norepinephrine activity in the brain. These are the same chemical messengers targeted by ADHD medications.

This is called optimal stimulation theory. Everyone has a level of arousal where they perform best. When the environment isn’t stimulating enough, boredom sets in, and the brain looks for ways to close that gap. People without ADHD experience this too, but their baseline arousal is typically closer to the optimal zone. People with ADHD start further from it, so they need more supplemental stimulation to stay engaged. Doodling, tapping, rocking in a chair: these are all ways the body tries to bridge that gap.

How Doodling Helps Focus

Doodling isn’t just a distraction. For people with ADHD, it can function as a focus tool. Sydney Zentall, an ADHD researcher, found that when children with ADHD perform a mindless secondary task like doodling while working on a primary task, they actually focus better on the primary task. The key is that the doodling occupies just enough mental bandwidth to prevent the brain from wandering off entirely, without competing for the attention needed for the main activity.

One proposed explanation involves two brain networks that operate like a seesaw. The default mode network is active when your mind wanders, daydreams, or drifts. The task positive network kicks in when you’re focused on something specific. In ADHD, the default mode network tends to intrude at unhelpful times. Doodling may help switch the brain from that wandering state into the focused state, acting as a kind of manual override. Edward Hallowell, co-author of “ADHD 2.0,” lists doodling alongside stretching and breathing exercises as activities that can trigger this switch.

Importantly, this benefit is more pronounced in people with ADHD than in those without it. Research suggests that children without ADHD don’t gain the same focusing advantage from fidgeting because the relevant brain regions are already adequately activated. For them, doodling during a task is more likely neutral or mildly distracting.

Doodling vs. Other Fidgeting Behaviors

Doodling is just one form of fidgeting, and fidgeting itself exists on a spectrum. Some common ADHD-related fidgeting behaviors include:

  • Gross motor fidgeting: bouncing a leg, rocking in a chair, pacing during phone calls
  • Fine motor fidgeting: clicking pens, picking at skin or nails, twisting hair, manipulating small objects
  • Sensory seeking: chewing on pen caps, rubbing textures, tapping surfaces in rhythmic patterns
  • Creative fidgeting: doodling, folding paper, building small structures from nearby materials

All of these serve the same basic purpose: raising arousal to a functional level. Doodling stands out because it’s quiet and socially acceptable in most settings, which is why many adults with ADHD gravitate toward it in meetings and lectures without even realizing they’re self-regulating.

When Doodling Might Signal Something More

If you’re wondering whether your doodling habit is connected to ADHD, the doodling itself isn’t the thing to focus on. Instead, consider the broader context. Do you also lose track of conversations easily? Do you struggle to finish tasks that don’t interest you? Do you feel restless when forced to sit still for long periods? Do you frequently misplace things, miss deadlines, or feel like your mind has too many tabs open at once?

Doodling in the presence of these other patterns could suggest that your brain is working harder than average to maintain baseline attention. The compulsive quality matters too. Someone who occasionally sketches in the margins is different from someone who physically cannot sit through a 30-minute meeting without their hands doing something. The latter pattern, especially when it’s been present since childhood and shows up across different areas of life, is more consistent with ADHD.

Context also matters for children. A child who doodles during class but pays attention, follows instructions, and performs well is probably just a doodler. A child who doodles constantly, can’t stay seated, loses assignments, and struggles to wait their turn is showing a pattern that goes well beyond the doodling.

Using Doodling as a Strategy

Whether or not you have an ADHD diagnosis, doodling during low-stimulation tasks like lectures, meetings, or phone calls can help maintain engagement. For people with ADHD, it’s especially effective because it targets the specific neurological gap between available stimulation and what the brain needs to stay alert. The doodling should be automatic and unstructured. If you’re concentrating on creating a detailed drawing, that becomes the primary task and defeats the purpose.

Some people pair doodling with other strategies: sitting near the front of a room to reduce visual distractions, using textured pens for additional sensory input, or alternating between doodling and brief note-taking to stay anchored in the material. These small adjustments can make a meaningful difference in how much information you retain from a long meeting or class.
