For most people getting wisdom teeth removed, Exparel is probably not worth the extra cost. The largest clinical trial testing it specifically for wisdom tooth surgery found no meaningful difference in pain relief or opioid use compared to a placebo injection. That’s a surprising result for a product that works well in other types of surgery, and it comes down to how wisdom tooth pain behaves differently from, say, knee replacement pain.
What Exparel Is and How It Works
Exparel is a long-acting version of a common numbing agent. The medication itself is the same drug your dentist already uses for local anesthesia, but it’s packaged inside microscopic fat bubbles (liposomes) that break down slowly in your tissue. The idea is that instead of wearing off in a few hours like a standard dental injection, the numbing effect trickles out over two to three days.
This slow-release design has made Exparel popular in orthopedic and abdominal surgeries, where it can meaningfully reduce pain during the first 72 hours of recovery. But the mouth is a different environment, and the clinical data for wisdom teeth tells a different story.
What the Clinical Evidence Shows
The INNOVATE study, the key clinical trial examining Exparel for impacted wisdom tooth removal, compared it head-to-head against a placebo injection. The results were underwhelming across every measure.
Patients who received Exparel actually needed rescue pain medication at a slightly higher rate than the placebo group: 82% versus 77%. The difference wasn’t statistically significant, but it certainly didn’t favor Exparel. The time before patients first reached for pain relief was only modestly longer with Exparel (3.9 hours versus 3.1 hours), and total opioid use over the first 48 hours was nearly identical between the two groups (6.1 mg versus 6.9 mg). None of these differences reached statistical significance, meaning they could easily be explained by chance.
In a secondary analysis of patients with less severely impacted teeth, the pattern held. Rescue medication rates were similar (73% for Exparel, 67% for placebo), and 48-hour opioid consumption was virtually the same (2.9 mg versus 3.2 mg). In short, Exparel did not reduce pain or opioid use in any measurable way for this procedure.
Why It Doesn’t Work as Well for Wisdom Teeth
Wisdom tooth pain is driven heavily by inflammation rather than nerve damage at the surgical site. The swelling peaks around 48 to 72 hours after surgery, which means the worst pain often comes from tissue inflammation that a local anesthetic can’t fully address. A numbing agent, no matter how long-acting, blocks nerve signals at the injection site but doesn’t reduce the inflammatory cascade that causes most of the discomfort.
This is why anti-inflammatory medications like ibuprofen tend to be so effective for wisdom tooth recovery. They target the actual source of pain. In many cases, alternating ibuprofen and acetaminophen on a schedule provides better relief than any local anesthetic alone.
The Cost Factor
Exparel is expensive. A single vial typically costs several hundred dollars, and oral surgeons who offer it generally pass that cost along to the patient as an add-on fee. Some offices charge $200 to $500 or more per procedure. Most dental insurance plans do not cover it, though there are exceptions. Connecticut’s Medicaid program, for example, began reimbursing for Exparel with wisdom tooth extractions in 2019 under a specific billing code, but this is unusual.
If your surgeon’s office is offering Exparel as an upgrade, you’re likely paying out of pocket. Given that the clinical evidence shows no significant benefit over placebo for this specific surgery, that’s a difficult price to justify.
Side Effects to Consider
Exparel carries the same risk profile as standard local anesthetics, with some additional considerations. The most common side effects in clinical studies were nausea, constipation, and vomiting, each occurring in more than 10% of patients. Dizziness affected about 6% of patients, and headache occurred in roughly 4%.
One important restriction: after receiving Exparel, you should avoid any additional local anesthetic injections for 96 hours. The slow-release design means the drug stays in your system longer than a standard injection, and stacking additional numbing agents on top could push levels too high. For wisdom tooth patients, this is rarely an issue since the procedure is a one-time event, but it’s worth knowing if you have other dental work planned in the same week.
What Works Better for Wisdom Tooth Pain
The most effective approach for wisdom tooth recovery is simpler and cheaper than Exparel. A standard local anesthetic during the procedure keeps you comfortable in the chair. After that, a scheduled rotation of over-the-counter anti-inflammatories and acetaminophen handles the majority of post-operative pain for most patients. Ice packs during the first 24 hours help limit swelling, and soft foods reduce irritation at the extraction sites.
For patients with significant anxiety about pain, the conversation with your surgeon should focus on whether a short course of prescription pain medication makes sense as a backup, rather than whether to add a costly injection that clinical data doesn’t support. Most people recovering from wisdom tooth removal find that over-the-counter options, taken consistently rather than waiting until pain becomes severe, manage their discomfort well enough to get through the first few days.
Exparel has genuine value in larger surgeries like joint replacements and abdominal procedures, where the pain profile is different and the evidence is stronger. For wisdom teeth specifically, the data suggests you’d be paying a premium for no added benefit.
