Is Extra Virgin Olive Oil Good for You?: What Science Says

Extra virgin olive oil is one of the most well-supported healthy fats in nutrition research. Regular consumption is linked to lower rates of heart disease, reduced inflammation, and a longer lifespan. Every additional 25 grams per day (just under 2 tablespoons) is associated with an 11% lower risk of dying from any cause and a 16% lower risk of cardiovascular disease.

Heart Disease and Mortality Benefits

The strongest evidence for extra virgin olive oil centers on heart health. In the landmark PREDIMED trial, participants who supplemented their diet with extra virgin olive oil reduced their risk of major cardiovascular events by 31% compared to a control group. A large study of U.S. adults published in the Journal of the American College of Cardiology found that higher olive oil intake was associated with a 19% lower risk of dying from cardiovascular disease specifically.

These benefits scale with intake. For every additional 25 grams per day, researchers found a 16% reduced risk of cardiovascular disease, a 22% lower risk of type 2 diabetes, and an 11% reduction in all-cause mortality. That 25-gram serving is roughly 1.5 to 2 tablespoons, a realistic amount to use in cooking and salad dressings throughout the day.

A Natural Anti-Inflammatory Compound

If you’ve ever noticed a peppery sting at the back of your throat when tasting good olive oil, that’s oleocanthal, a compound unique to extra virgin olive oil. Oleocanthal works through the same pathway as ibuprofen, blocking the enzymes that drive inflammation throughout the body. It also has antioxidant effects in the brain. The more throat-sting you feel, the more oleocanthal the oil contains.

This anti-inflammatory activity is one reason researchers believe olive oil protects against chronic diseases. Low-grade, persistent inflammation plays a role in heart disease, cancer, diabetes, and neurodegeneration. Getting a small dose of a natural anti-inflammatory compound at every meal, day after day, adds up over years.

What About Blood Sugar?

Despite olive oil’s association with lower diabetes risk in population studies, controlled trials haven’t shown a clear direct effect on blood sugar levels. A systematic review of 13 clinical trials found that fasting blood sugar, insulin levels, and insulin resistance all trended slightly lower in groups consuming extra virgin olive oil, but none of these reductions reached statistical significance. Adjusting for age, health status, dose, and duration didn’t change the results.

This doesn’t mean olive oil is useless for metabolic health. The diabetes risk reduction seen in larger studies likely comes from olive oil replacing less healthy fats (like butter or refined vegetable oils) and from its role within an overall dietary pattern rather than from a single blood-sugar-lowering mechanism.

Brain Health and Cognitive Decline

Animal research on olive oil and brain health is striking. In multiple mouse models of Alzheimer’s disease, diets enriched with extra virgin olive oil significantly reduced the accumulation of amyloid plaques and abnormal tau protein, two hallmarks of the disease. Mice fed olive oil consistently performed better on memory and learning tests, showed less brain inflammation, and maintained healthier connections between brain cells.

Human data is more limited but points in the same direction. The Italian Longitudinal Study on Aging found that higher intake of monounsaturated fats (olive oil’s primary fat) was associated with less cognitive decline. A pilot trial in patients with mild cognitive impairment found that those consuming high-polyphenol extra virgin olive oil performed better across nearly all cognitive domains compared to those on a standard Mediterranean diet without the oil. These are early findings, but the consistency across cell, animal, and human studies is notable.

Cooking With Extra Virgin Olive Oil

A persistent myth holds that extra virgin olive oil can’t handle heat, but this isn’t accurate. Its smoke point ranges from 347°F to 464°F depending on quality and freshness. That comfortably covers sautéing, roasting, and most home cooking. According to research from UC Davis, good quality extra virgin olive oil contains natural antioxidants that resist heat degradation, preventing the formation of harmful compounds during cooking. You don’t need to save your extra virgin olive oil only for drizzling.

How to Choose and Store It

To earn the “extra virgin” label under international standards, olive oil must have a free acidity below 0.8%. Lower acidity generally indicates fresher, better-handled olives. This matters because the health benefits come largely from polyphenols and other bioactive compounds that degrade over time, and higher-quality oils start with more of them.

Storage conditions have a dramatic effect on how much of that good stuff remains. Keeping olive oil sealed in a cool, dark place preserves its potency far longer than leaving it open near your stove. In one study, key antioxidant compounds retained about 63% of their original levels after six months in sealed bottles at room temperature, but dropped to just 52% in bottles that were regularly opened. Heat accelerates the loss: at elevated temperatures, some protective compounds fell to 28% of their starting levels within three months.

To get the most from your oil, buy in quantities you’ll use within a few months, store the bottle away from light and heat, and look for a harvest date on the label rather than just a “best by” date. Dark glass bottles or tins protect against light degradation better than clear glass.

How Much to Use Daily

Most of the observed health benefits in research correspond to about 25 grams per day, which works out to roughly 1.5 to 2 tablespoons. That’s enough to cook a meal and dress a salad. At around 120 calories per tablespoon, olive oil is calorie-dense, so the practical approach is to use it as a replacement for other fats you’re already eating rather than adding it on top of everything else. Swap it in for butter when cooking vegetables, use it as the base of salad dressings, or drizzle it over grains and soups. The benefits come from consistency over time, not from dramatic single doses.
