Is Fake Sugar Bad for You? Here’s What Science Says

Artificial sweeteners are not clearly harmful at the amounts most people consume, but they’re not as inert as once believed either. Regulatory agencies like the FDA consider all approved sweeteners safe within daily limits, and the best available evidence shows they don’t raise blood sugar, don’t increase appetite, and don’t cause cancer at normal intake levels. The more nuanced picture involves potential effects on gut bacteria, cardiovascular health, and whether they actually help people eat less over time.

They Don’t Spike Blood Sugar or Insulin

The most common reason people reach for artificial sweeteners is to avoid the blood sugar spikes that come with regular sugar, and on this front, the evidence is reassuring. Studies in people with type 2 diabetes have tested aspartame, sucralose, saccharin, and stevia over periods ranging from two weeks to 18 weeks. None of them affected fasting blood sugar, HbA1c (a marker of long-term blood sugar control), or insulin levels.

There’s been speculation that tasting something sweet might trick your body into releasing insulin even when no sugar arrives, a phenomenon called the cephalic phase insulin response. While this response does exist when you eat real food, the evidence that artificial sweeteners reliably trigger a meaningful insulin spike on their own is weak. For people managing diabetes or trying to reduce sugar intake, swapping to a zero-calorie sweetener does what you’d expect: it removes the sugar without introducing a new blood sugar problem.

They Don’t Seem to Increase Appetite

A persistent worry is that fake sugar tricks your brain into craving more food, ultimately causing you to eat more calories than you saved. The research doesn’t support this. A systematic review of clinical trials found that appetite hormones, including ghrelin (which drives hunger) and peptide YY (which signals fullness), were unchanged after consuming artificial sweeteners. Subjective hunger ratings and the amount of food people ate afterward were also no different compared to water or unsweetened controls.

Multiple individual trials confirm this pattern. Aspartame didn’t stimulate food intake beyond what people consumed after plain water. Sucralose showed no differences in appetite ratings or food intake. This doesn’t mean artificial sweeteners actively help you lose weight, but the idea that they backfire by making you hungrier doesn’t hold up in controlled settings.

Gut Bacteria Are a Real Concern

The most compelling area of concern involves your gut microbiome, the trillions of bacteria in your digestive tract that influence everything from immune function to mood. Not all sweeteners affect gut bacteria equally, but sucralose and saccharin both reduce microbial diversity, which is generally considered a marker of gut health.

Sucralose appears to be the most disruptive. In laboratory models using human gut bacteria, sucralose significantly shifted the bacterial community away from its normal composition, enriching certain families while suppressing others to less than 10% of the total community. Saccharin also reduced diversity, though to a lesser degree. These shifts included changes in bacteria involved in digestion and immune signaling. The practical health consequences of these changes in everyday life aren’t fully mapped out yet, but reduced microbial diversity is consistently linked to metabolic and inflammatory problems in other research.

Erythritol and Heart Health

Erythritol, a sugar alcohol found in many “natural” zero-calorie sweeteners and keto products, has drawn attention for a potential cardiovascular risk. A study published in Arteriosclerosis, Thrombosis, and Vascular Biology found that a single 30-gram dose of erythritol (roughly the amount in a couple of servings of an erythritol-sweetened product) enhanced platelet reactivity in healthy volunteers. Platelets are the blood cells responsible for clotting, and increased reactivity means a greater tendency to form clots, which is the mechanism behind heart attacks and strokes.

The same research group found that higher fasting blood levels of erythritol were clinically associated with increased risk of major cardiovascular events. This is a relatively new finding, and it doesn’t mean erythritol will cause a heart attack. But for people already at elevated cardiovascular risk, it’s worth knowing that this particular sweetener may not be as benign as its “natural” label suggests.

The Cancer Question Is Mostly Settled

In 2023, the International Agency for Research on Cancer classified aspartame as “possibly carcinogenic to humans,” a designation called Group 2B. That sounds alarming, but this is the same category that includes aloe vera and pickled vegetables. The classification was based on “limited evidence,” meaning some studies showed a statistical association but couldn’t rule out other explanations. The WHO simultaneously reaffirmed that aspartame is safe at current intake levels.

The FDA sets acceptable daily intake limits for each sweetener: 50 mg per kilogram of body weight for aspartame, 15 mg/kg for saccharin, and 5 mg/kg for sucralose. For a 150-pound person, the aspartame limit translates to roughly 18 to 19 cans of diet soda per day. Most people consume nowhere near these thresholds.

“Natural” Sweeteners Aren’t Safer

Stevia and monk fruit are often marketed as healthier alternatives because they come from plants rather than a lab. In terms of safety data, they’re equivalent. Michigan State University’s Center for Research on Ingredient Safety notes that stevia and monk fruit are classified as Generally Recognized as Safe, meaning manufacturers aren’t even required to perform the same rigorous safety studies as synthetic sweeteners. Neither offers additional health benefits over artificial options. The “natural” label is a marketing distinction, not a medical one.

One Sweetener Is Genuinely Dangerous for Dogs

Xylitol, a sugar alcohol used in sugar-free gum, candy, and peanut butter, is extremely toxic to dogs. Doses as low as 100 mg per kilogram of a dog’s body weight can cause dangerous drops in blood sugar, and doses above 500 mg/kg can trigger liver failure. A single stick of xylitol-sweetened gum can contain enough to poison a small dog. If you keep xylitol products in your home and have a dog, store them where your pet can’t reach them.

The Bottom Line on Daily Use

Artificial sweeteners are not poison, and they’re a reasonable tool for cutting sugar intake, particularly for people managing blood sugar. They don’t appear to increase hunger or cause you to overeat. The areas worth watching are gut health, where sucralose in particular may reduce beneficial bacterial diversity, and cardiovascular risk with erythritol. If you use artificial sweeteners occasionally or moderately, the evidence suggests you’re fine. If you’re consuming large amounts daily, rotating between different types or simply reducing your overall intake of sweet-tasting foods and drinks is a practical way to hedge against risks that science is still sorting out.
