Is Fasting a Good Way to Lose Weight? Pros & Cons

Fasting can help you lose weight, but it works about as well as simply eating fewer calories each day. In clinical trials, people lost 5.5 to 6.5 kg (roughly 12 to 14 pounds) over six months whether they used a fasting approach or traditional calorie restriction. Fasting does offer some short-term advantages in fat loss and blood sugar control, but it also carries real risks, particularly muscle loss, that are worth understanding before you start.

How Much Weight You Can Expect to Lose

A meta-analysis of nine randomized trials involving 565 people found that fasting-based strategies produced about 1 kg (2.2 pounds) more weight loss than daily calorie restriction over the first six months. The fasting group also lost more body fat specifically, averaging an extra kilogram of fat loss compared to the calorie-cutting group. That’s a modest but real difference in the short term.

The catch is that this advantage disappears with time. When studies ran longer than six months, the difference between fasting and regular dieting was essentially zero. Both groups ended up at similar weights. This suggests fasting isn’t a magic metabolic trick. It’s primarily another way to eat less overall, and whichever method you can stick with longest is the one that will work best for you.

Three Common Fasting Methods Compared

Not all fasting schedules produce the same results. The three most studied approaches differ in how much weight people typically lose from their starting weight:

  • Time-restricted feeding (16:8): You eat within an 8-hour window and fast for 16 hours each day. This is the easiest to maintain but produces the smallest losses, around 2.6% to 3.6% of body weight.
  • The 5:2 method: You eat normally five days a week and restrict to about 500 calories on two non-consecutive days. This tends to produce 5.4% to 6% body weight loss.
  • Alternate-day fasting: You alternate between regular eating days and very low calorie or no-calorie days. Results range widely, from 3.2% to 6.4% of body weight lost.

For someone weighing 200 pounds, these ranges translate to roughly 5 to 7 pounds lost with 16:8, and 10 to 13 pounds with 5:2 or alternate-day fasting. The more aggressive the fasting schedule, the greater the calorie deficit, which explains the larger losses.

The Muscle Loss Problem

This is the most important downside that many fasting advocates don’t mention. A study from the University of Hawaiʻi Cancer Center tracked body composition in people using intermittent fasting and found that 65% of the weight they lost came from lean mass, primarily muscle. That’s more than double the normal rate. During typical weight loss from any method, lean mass accounts for 20% to 30% of the total weight lost.

Losing muscle matters because muscle tissue burns more calories at rest than fat does. When you lose a large proportion of muscle, your metabolism slows, making it harder to keep weight off long-term. It also affects your strength, bone health, and ability to stay active as you age. If you do choose fasting, combining it with resistance training and adequate protein intake on eating days can help protect against excessive muscle loss.

Benefits Beyond the Scale

Fasting does appear to improve how your body handles blood sugar, at least in the short term. In clinical trials, people who fasted showed a significant drop in fasting insulin levels and a measurable improvement in insulin resistance compared to those who simply cut daily calories. One study tracking lifestyle interventions in people with obesity found that insulin resistance (measured by a standard clinical index called HOMA-IR) dropped by 45% over one year, even though body mass only decreased by 10%. That disproportionate improvement suggests fasting may have effects on blood sugar regulation that go beyond just losing weight.

Fasting glucose levels also fell more in fasting groups than in calorie-restriction groups, by about 3.6 mg/dL on average. That’s a small but statistically meaningful difference, and it could be relevant for people at risk of type 2 diabetes.

You may have also heard about autophagy, the cellular cleanup process where your body breaks down and recycles damaged components. Animal studies suggest this process kicks in after 24 to 48 hours of fasting. However, there isn’t enough human research to confirm the exact timing or whether shorter fasts like the 16:8 method trigger meaningful autophagy at all.

Effects on Women’s Hormones

Fasting affects reproductive hormones in women in ways that standard calorie restriction may not. A systematic review found that intermittent fasting reduced testosterone, luteinizing hormone, and anti-Müllerian hormone levels in women. For women with polycystic ovary syndrome (PCOS), some of these changes could be beneficial, since PCOS involves elevated testosterone and irregular cycles.

For women without PCOS, however, shifts in reproductive hormones can disrupt menstrual cycles or affect fertility. The hormonal response to fasting tends to be more pronounced in women than in men, which is why many clinicians recommend women start with milder protocols like 14:10 (a 14-hour fast) rather than jumping into longer fasts.

Who Should Avoid Fasting

Fasting is not safe for everyone. Clinicians at Mass General Brigham identify several groups who should either avoid fasting entirely or consult a doctor before trying it:

  • Anyone under 18 or still growing, including young adults who haven’t finished developing
  • Adults over 65, who are already at higher risk for muscle loss
  • People with diabetes, heart disease, kidney disease, or liver disease
  • Anyone with a history of eating disorders or disordered eating patterns
  • People who are pregnant or breastfeeding
  • Those with low blood pressure or who take blood thinners, diuretics, blood pressure medications, or blood sugar medications

The concern with medications is that fasting can amplify their effects. Blood pressure drugs combined with the natural blood pressure drop from not eating can cause dizziness or fainting. Blood sugar medications taken without food can trigger dangerous hypoglycemia.

Is Fasting Worth It?

Fasting produces real weight loss, but over time it’s no more effective than eating fewer calories each day. Where fasting may have an edge is in simplicity: some people find it easier to skip meals entirely than to carefully portion every meal. If restricting your eating window helps you naturally eat less without feeling deprived, fasting can be a practical tool.

The trade-off is a higher risk of muscle loss, potential hormonal changes (especially for women), and the possibility that the rigid on/off structure can trigger unhealthy eating patterns in people prone to binge-restrict cycles. The most effective approach for lasting weight loss is whichever one lets you maintain a moderate calorie deficit while keeping enough protein and physical activity to preserve muscle. For some people that’s fasting. For many others, it’s not.
