A full-day fast is not appropriate for everyone. People with diabetes face the most obvious risk, since fasting can cause dangerous drops in blood sugar, especially when combined with insulin or other glucose-lowering medications. If you take medications for blood pressure or heart disease, you may be more prone to mineral imbalances during extended periods without food. People who need to take medications with food to avoid nausea or stomach irritation will also have trouble with this approach.

If you’re already at a low body weight, a 24-hour fast can push you into a deficit that affects your bones, immune function, and energy levels. Pregnant or breastfeeding women, children, and anyone with a history of disordered eating should avoid prolonged fasting entirely. For these groups, the risks clearly outweigh any potential benefits.
