Feline leukemia virus (FeLV) has no cure, but it is treatable in the sense that many cats can be managed with supportive care and live well beyond their diagnosis. The median survival time after an FeLV diagnosis is about 2.5 years, and cats whose immune systems partially suppress the virus can live as long as uninfected cats. How your cat does depends heavily on which type of infection they develop.
Progressive vs. Regressive Infection
Not all FeLV diagnoses are the same. After a cat is exposed to the virus, their immune system either controls it or doesn’t, and the outcome falls into one of two broad categories that determine everything about prognosis and treatment.
In a progressive infection, the virus replicates freely and circulates throughout the body. These cats remain contagious to other cats and face the highest risk of serious complications: lymphoma, severe anemia, and immune suppression that leaves them vulnerable to secondary infections. A Brazilian study tracking naturally infected cats found that progressively infected cats had a mean survival time of roughly 140 days, with a median of just 30 days from diagnosis. That short median reflects how many of these cats are already critically ill when they’re identified.
In a regressive infection, the cat’s immune system fights the virus into a dormant state. The virus persists in the cat’s DNA but stops actively replicating. These cats test negative on standard blood antigen tests, can’t spread the virus to other cats, and are very unlikely to show any clinical signs of FeLV. The same study found that regressively infected cats had a life expectancy statistically indistinguishable from uninfected cats, with a mean survival exceeding 490 days and still climbing at the study’s end.
This distinction matters because a positive FeLV test at the vet doesn’t automatically mean a death sentence. Retesting over the following weeks and months helps determine whether the infection is progressive or regressive, and that classification changes the entire outlook.
What Treatment Looks Like
Because there’s no drug that eliminates FeLV from a cat’s body, treatment focuses on two goals: supporting the immune system and aggressively managing any secondary diseases that arise. For cats with regressive infections, this often means very little active treatment at all, just regular veterinary checkups and standard preventive care.
For progressively infected cats, treatment gets more involved. Secondary infections from bacteria, fungi, or other viruses are common because FeLV suppresses the immune system. These infections often require longer and more intensive courses of antibiotics or antifungals than they would in a healthy cat. Anemia is another frequent complication. Depending on severity, your vet may recommend blood transfusions or medications that stimulate red blood cell production.
One of the more painful complications is severe inflammation of the gums and mouth, a condition called stomatitis. In FeLV-positive cats, this is typically best managed by extracting all the teeth rather than relying on medications alone. That sounds drastic, but cats adapt well to eating without teeth, and it often provides more lasting relief than repeated rounds of anti-inflammatory drugs.
Antiviral and Immune-Boosting Options
A few treatments aim to fight the virus more directly, though none are considered curative. Raltegravir, a drug originally developed for HIV in humans, works by blocking the enzyme retroviruses use to insert their genetic material into host cells. It has shown some activity against FeLV in cats and is well tolerated at the doses studied, but there’s no strong evidence yet that it meaningfully changes long-term outcomes for most cats.
An immune-boosting therapy called Lymphocyte T-Cell Immunomodulator (LTCI) has a conditional license from the USDA as a treatment aid for FeLV-positive cats. It works by activating certain immune cells to produce signaling molecules that help the body’s own killer cells target virus-infected and tumor cells more effectively. Studies in both lab settings and naturally infected cats have shown decreased levels of circulating virus and improved blood work. The conditional license means it met a reasonable expectation of efficacy and passed safety testing, but it hasn’t yet achieved full licensure, so the evidence is promising rather than definitive.
Interferon therapy is another option some veterinarians use. These treatments aim to broadly boost antiviral immune responses. Results vary, and availability depends on your region.
Daily Life With an FeLV-Positive Cat
Keeping an FeLV-positive cat indoors is essential, both to prevent spreading the virus to neighborhood cats and to reduce your cat’s exposure to infections their weakened immune system may struggle to fight. If you have other cats in the home, they should be vaccinated against FeLV and ideally kept separate from a progressively infected cat. Regressively infected cats pose virtually no transmission risk.
Routine veterinary visits become more important after an FeLV diagnosis. Many vets recommend checkups every six months rather than annually, with blood work to catch problems like anemia or changes in white blood cell counts before they become emergencies. Early detection of secondary illnesses is one of the biggest factors in extending a positive cat’s life, since infections that would be minor in a healthy cat can escalate quickly in an immunosuppressed one.
A high-quality diet matters more than usual. Raw food is generally discouraged because it can harbor bacteria and parasites that a compromised immune system can’t handle as easily. Beyond that, keeping stress low, maintaining a clean environment, and staying current on parasite prevention all help reduce the burden on your cat’s immune system.
What Testing Involves
Most initial FeLV screening uses a simple blood test called a SNAP test, which detects viral proteins circulating in the bloodstream. This test costs around $40 to $45 at most labs, though your vet’s pricing may vary. A positive result should be confirmed with a second type of test, called an IFA, which costs around $30 and checks whether the virus has infected the bone marrow. A positive IFA generally indicates progressive infection, while a negative IFA alongside a positive SNAP test suggests the cat may have a regressive or early infection that could resolve.
Because a cat’s infection status can change over time, especially in the weeks and months after initial exposure, the current veterinary guidelines emphasize retesting. A kitten that tests positive at adoption may later test negative as its immune system matures and suppresses the virus. Quantitative testing, which measures how much virus is actually circulating rather than just whether it’s present, is becoming more useful for tracking how a cat’s infection is evolving and making better treatment decisions.
Realistic Expectations
The gap between progressive and regressive outcomes is enormous. A cat with regressive FeLV can live a normal, full lifespan with minimal medical intervention. A cat with progressive FeLV faces a significantly shortened life, though quality supportive care can extend that timeline and keep the cat comfortable. The 2.5-year median survival figure from Cornell encompasses both groups, so your cat’s individual prognosis depends heavily on which category they fall into.
Costs can add up for progressively infected cats. Beyond the initial testing, you may be looking at more frequent vet visits, blood work, medications for secondary infections, and potentially emergency care for complications like severe anemia or lymphoma. There’s no single monthly cost estimate because it depends entirely on what complications arise, but budgeting for more frequent veterinary care than a healthy cat requires is realistic planning.
FeLV is a serious diagnosis, but “positive” and “terminal” are not the same thing. Many FeLV-positive cats, particularly those with regressive infections, live comfortable lives for years. For progressively infected cats, the focus shifts to maximizing quality of life and catching complications early, which is a genuinely achievable goal with attentive care.
