Is Filler Bad for You? Side Effects and Real Risks

Dermal fillers are not inherently bad for you, but they carry real risks that range from mild and temporary to rare but serious. When injected by a qualified provider using FDA-approved products, fillers have a strong safety record. The vast majority of side effects are short-lived: redness, tenderness, and swelling that resolve within days. Serious complications like tissue damage or lasting inflammatory reactions affect fewer than 1% of patients, but they do happen, and certain health conditions can raise your odds.

What Fillers Are Made Of

Most fillers used today fall into two categories: temporary and permanent. The temporary options include hyaluronic acid (the most common), calcium hydroxylapatite, and poly-L-lactic acid. Hyaluronic acid is a substance your body already produces naturally, which is one reason it’s considered relatively safe. Its effects last roughly 6 to 12 months. Calcium hydroxylapatite lasts about 18 months, while poly-L-lactic acid builds results gradually over several weeks and can last up to two years.

The only FDA-approved permanent filler uses tiny plastic microspheres (PMMA) that the body never absorbs. Permanent fillers carry higher long-term risks because they stay in your tissue indefinitely, giving your immune system more time to react. Liquid silicone injections, which sometimes appear on the black market or in unlicensed settings, are not FDA-approved for filling wrinkles or augmenting tissue anywhere in the body.

Common Side Effects

Nearly everyone who gets filler experiences some immediate reaction. In a study of 500 patients, redness occurred in 91%, tenderness in 88%, bruising in 85%, numbness in 79%, and swelling in 78%. These are expected parts of the process, not warning signs. They’re generally mild to moderate and clear up within a week or two. Icing the area and avoiding blood-thinning medications like ibuprofen beforehand can reduce bruising, but some degree of tenderness and puffiness is essentially unavoidable.

Rare but Serious Complications

The complications worth understanding are the uncommon ones, because recognizing them early makes a significant difference in outcomes.

Vascular Occlusion

The most dangerous acute risk is vascular occlusion, which happens when filler is accidentally injected into or compresses a blood vessel, blocking blood flow to surrounding tissue. Without blood supply, skin can die. The warning signs are intense pain, blanching or unusual color changes in the skin, and a mottled or dusky appearance near the injection site. If you notice these symptoms during or shortly after a procedure, it’s a medical urgency. An experienced injector can recognize the signs immediately and take steps to restore blood flow.

In the 500-patient study mentioned earlier, skin necrosis (tissue death from lost blood supply) occurred in just 2 cases, a rate of 0.4%. It’s rare, but the consequences can be permanent scarring, which is why injector skill and anatomical knowledge matter enormously.

Delayed Nodules and Granulomas

Some complications don’t show up for months or even years. Foreign body granulomas, which are hard lumps that form when your immune system walls off the filler material, have an estimated incidence between 0.01% and 1%. They can occur with any type of injectable filler, but they appear more frequently with particulate fillers like poly-L-lactic acid, where the gradual breakdown of microparticles can continuously provoke an immune response for 14 months or longer.

Bacterial biofilms may also play a role. Some bacteria can form a protective coating around themselves near the filler material, entering a dormant state that evades both the immune system and antibiotics. These bacteria can reactivate during illness or after repeated injections in the same area, triggering inflammation, abscesses, or nodules long after the original treatment.

How Your Body Breaks Down Filler

Your body contains natural enzymes called hyaluronidases that break down hyaluronic acid as part of normal tissue maintenance. These enzymes are present throughout your tissues and gradually degrade hyaluronic acid fillers over time, which is why the results are temporary. The fragments produced during this breakdown process interact with receptors on nearby cells and are eventually cleared through your lymphatic system.

This natural degradation is also why hyaluronic acid fillers can be reversed in an emergency. Injecting a concentrated form of hyaluronidase dissolves the filler much faster than your body would on its own. However, the results aren’t always perfect. In one analysis, 59% of patients had a satisfactory result after dissolution, 24% needed additional rounds of the enzyme, and 18% experienced unwanted facial changes like hollowing. Reversal is a valuable safety net, but it’s not a guaranteed reset button.

Fillers made from other materials, like calcium hydroxylapatite, poly-L-lactic acid, or permanent PMMA microspheres, cannot be dissolved with hyaluronidase. If complications arise with those products, treatment options are more limited.

Filler Migration

Migration, where filler drifts away from its original placement, is a growing concern among patients. The properties of the filler itself appear to influence how likely this is. Thicker, more cohesive fillers are thought to be less prone to movement compared to thinner, more fluid products. Placement technique also matters: filler injected into highly mobile areas of the face or placed too superficially may be more likely to shift over time. Research using MRI and ultrasound imaging is ongoing to better characterize how different filler types behave in tissue over weeks and months.

Who Faces Higher Risks

Certain health conditions can tip the balance from “generally safe” to “proceed with caution.” People with active autoimmune diseases face a meaningfully higher chance of delayed inflammatory reactions. The immune system in these individuals is already in a heightened state, making it more likely to mount an aggressive response against the foreign material. One study found that 8.6% of HIV-positive patients developed granulomatous nodules after poly-L-lactic acid injections, well above the baseline rate.

Specific conditions flagged in the literature include active psoriasis, psoriatic arthritis, and systemic lupus erythematosus. Medications matter too. Immunosuppressants raise infection risk, and certain biologic drugs used for autoimmune conditions have been linked to granulomatous reactions as a known side effect. If you’re managing an autoimmune condition, getting filler during a flare-up, when your immune system is most reactive, is particularly risky.

There’s also some evidence that mixing different brands of hyaluronic acid filler over time may increase the severity of adverse reactions if they occur. This hasn’t been definitively proven to raise the overall risk, but when reactions do happen in patients with a history of multiple filler brands, they tend to be more severe and longer-lasting.

What Actually Makes Fillers Dangerous

The biggest variable in filler safety isn’t the product itself. It’s who is injecting it and where they’re doing it. Vascular occlusion, the most serious acute complication, is almost entirely a function of injection technique and anatomical knowledge. An injector who understands the vascular anatomy of the face can avoid the high-risk zones around the nose, temples, and glabella (the area between your eyebrows) where blood vessels are most vulnerable.

Unregulated settings pose the greatest danger. The FDA has specifically warned against certain filler products not approved for use in the United States, and liquid silicone injections performed outside medical settings have caused disfigurement, chronic inflammation, and in rare cases, death. The product being injected and the credentials of the person holding the needle are the two factors most within your control.

For most healthy people getting FDA-approved fillers from a trained, licensed provider, the experience is straightforward: some temporary swelling and bruising, results that last several months to a couple of years, and a product that your body will eventually break down on its own. The risks are real but low, and most of the serious ones are preventable with proper technique and patient screening.
