Fish oil is excellent at lowering triglycerides but does not lower LDL cholesterol, the number most people mean when they say “high cholesterol.” In fact, fish oil can slightly raise LDL. That doesn’t make it useless for heart health, but it does mean the answer depends on which part of your lipid panel needs work.
The Triglyceride Effect Is Real
Where fish oil genuinely shines is triglyceride reduction. At doses of 2 to 4 grams of EPA and DHA per day, omega-3 fatty acids lower triglycerides by roughly 25 to 30 percent. That’s a meaningful drop, especially for people whose levels are above 500 mg/dL, where the risk of pancreatic inflammation starts climbing.
The mechanism is straightforward: your liver normally packages fats into particles called VLDL and sends them into your bloodstream. Fish oil slows that production by nearly half, according to metabolic studies measuring the liver’s output rate directly. The result is fewer fat-rich particles circulating in your blood. At the same time, the particles that do get released tend to be smaller and cleared from the bloodstream faster.
What Happens to LDL (“Bad” Cholesterol)
This is where fish oil gets complicated. A large meta-analysis of randomized controlled trials published in the Journal of the American Heart Association found that omega-3 supplements consistently raised LDL cholesterol by a small but measurable amount. At a dose of about 1 gram per day, LDL went up an average of roughly 3 mg/dL. At 2 grams per day, the increase was closer to 3.5 mg/dL. These are modest bumps, but they move in the wrong direction if LDL is your main concern.
The type of omega-3 matters here. DHA, one of the two main fatty acids in fish oil, is more likely to raise LDL than EPA alone. Most over-the-counter fish oil capsules contain a mix of both. People with high cholesterol who aren’t already on a statin or other lipid-lowering medication may be especially vulnerable to this LDL increase.
HDL Gets a Small Boost
Fish oil does modestly raise HDL, sometimes called “good” cholesterol. The same meta-analysis found an HDL increase of about 3.5 mg/dL at a dose of roughly 1.75 grams per day. That’s a real change, though not large enough on its own to dramatically shift your cardiovascular risk profile. For context, regular aerobic exercise typically raises HDL by 5 to 10 percent, so fish oil offers a smaller contribution in the same direction.
Prescription Fish Oil vs. Store-Bought Supplements
There’s a significant gap between the fish oil capsules you find at a pharmacy and the prescription versions your doctor might recommend. Prescription omega-3 products are highly purified and contain much higher concentrations of EPA (or EPA plus DHA) than retail supplements. The most studied prescription form, icosapent ethyl, contains only purified EPA and is dosed at 4 grams per day.
In the landmark REDUCE-IT trial, this prescription EPA reduced major cardiovascular events (heart attack, stroke, cardiovascular death, coronary procedures, and unstable angina) by 25 percent compared to placebo. The absolute difference was striking: 17.2 percent of patients on the medication had an event versus 22 percent on placebo. These patients already had elevated triglycerides and were taking statins, so the benefit came on top of standard cholesterol treatment.
Over-the-counter fish oil supplements have not shown this kind of cardiovascular benefit. A separate large trial called STRENGTH tested a high-dose combination of EPA and DHA and found no reduction in cardiovascular events compared to placebo. The American Heart Association’s most recent guidelines specifically recommend against nonprescription fish oil for heart disease prevention, noting these products “have not demonstrated clinical benefit in patients with hypertriglyceridemia or ASCVD.” If your doctor thinks you need omega-3 therapy, they’ll likely write a prescription rather than point you to the supplement aisle.
A Heart Rhythm Risk Worth Knowing
High-dose omega-3 supplementation carries a notable safety signal: increased risk of atrial fibrillation, an irregular heart rhythm that can raise stroke risk. The STRENGTH trial found that 4 grams per day of marine omega-3s was associated with a 69 percent higher risk of new-onset atrial fibrillation in people already at high cardiovascular risk. A meta-analysis pooling seven randomized trials confirmed the link, finding a 25 percent higher rate of atrial fibrillation events among omega-3 users overall.
This doesn’t mean a standard 1-gram fish oil capsule will cause heart rhythm problems. The risk appears most relevant at the higher therapeutic doses used for triglyceride management. But it’s one reason self-prescribing large quantities of fish oil without medical oversight isn’t a great idea.
Who Actually Benefits
If your cholesterol problem is primarily high LDL, fish oil is not the right tool. It won’t bring that number down and may nudge it higher. Statins and other cholesterol-lowering medications target LDL directly and remain the first-line approach.
If your triglycerides are the issue, particularly if they’re above 150 mg/dL and you’re already on a statin, prescription EPA therapy has strong evidence behind it. For very high triglycerides (500 mg/dL or above), prescription omega-3s can meaningfully reduce the risk of pancreatitis.
For people with a mixed picture, where both LDL and triglycerides are elevated, the strategy usually involves a statin to handle LDL combined with prescription omega-3s if triglycerides remain stubbornly high. The statin can offset the small LDL bump that omega-3s may cause, and the combination has been shown to reduce VLDL cholesterol by 30 to 40 percent beyond what a statin achieves alone. Taking prescription omega-3s with a meal containing some fat improves absorption significantly, since your digestive enzymes need dietary fat present to break the capsule’s contents into a usable form.
