Fish oil’s reputation as a heart supplement is more complicated than the label on the bottle suggests. Standard over-the-counter fish oil supplements have not been shown to prevent heart attacks or strokes in most people. However, a specific high-dose, prescription form of omega-3 has demonstrated a 25% reduction in major cardiovascular events in high-risk patients. The answer depends heavily on what kind of fish oil you’re taking, how much, and what your baseline risk looks like.
What Fish Oil Actually Does in Your Body
The two omega-3 fatty acids in fish oil, EPA and DHA, affect your cardiovascular system through several pathways. The most well-established effect is lowering triglycerides, a type of fat in your blood that contributes to artery disease when levels stay elevated. EPA in particular reduces triglyceride levels without raising LDL (“bad”) cholesterol. It does this by slowing the liver’s production of triglyceride-rich particles while also helping your body clear them from the bloodstream faster.
Beyond triglycerides, omega-3s help dial down inflammation inside your arteries. When your body metabolizes these fats, it produces compounds called resolvins and protectins that actively work to calm inflamed tissue. Omega-3s also compete with other fats for the same enzymes, which limits the production of molecules that cause blood vessels to constrict and blood to clot. There’s also evidence that EPA gets absorbed directly into arterial plaques, potentially making them more stable and less likely to rupture and trigger a heart attack.
The Trial That Changed the Conversation
The landmark study for fish oil and heart health is the REDUCE-IT trial, published in 2018. It tested 4 grams daily of icosapent ethyl, a purified, prescription-grade form of EPA, in about 8,000 people who were already on statin medications and had elevated triglycerides plus either existing heart disease or diabetes with additional risk factors. Over five years, the group taking the EPA supplement experienced a 25% reduction in major cardiovascular events compared to placebo. That included fewer heart attacks, strokes, and cardiovascular deaths. The primary outcome occurred in 17.2% of the treatment group versus 22% of the placebo group.
This trial led the FDA to approve icosapent ethyl specifically for reducing cardiovascular risk in a select group of high-risk patients: those with triglycerides at or above 150 mg/dL who have established heart disease, or who have diabetes along with two or more additional cardiovascular risk factors.
Why Most Fish Oil Supplements Fall Short
Here’s where it gets important for the average person buying fish oil at the pharmacy. The REDUCE-IT results came from a prescription product containing nearly 4 grams of pure EPA per day. A typical over-the-counter fish oil capsule contains a fraction of that, often around 300 mg of combined EPA and DHA, meaning you’d need more than a dozen standard capsules daily to approach the dose used in the trial. And it’s not just about quantity.
Several large trials have tested standard-dose fish oil supplements (around 1 gram per day) and consistently found no meaningful benefit. The ORIGIN trial, the Risk & Prevention trial, and the ASCEND trial each enrolled over 12,000 high-risk individuals and followed them for five to seven years. None found a reduction in heart attacks, strokes, or cardiovascular deaths. The OMEGA trial and the more recent OMEMI trial in elderly heart attack survivors also came up empty. Even the STRENGTH trial, which used a higher dose of 4 grams daily but combined both EPA and DHA (rather than EPA alone), found no difference in outcomes.
The VITAL trial, one of the largest, randomized nearly 26,000 people to receive 1 gram of fish oil or placebo for over five years. It found no significant reduction in overall major cardiovascular events. There was one notable finding buried in the data: when heart attacks were analyzed separately, the fish oil group had a 28% lower risk of heart attack and a 50% lower risk of fatal heart attack. But these were secondary analyses, not the study’s primary endpoint, so they’re considered suggestive rather than conclusive.
Triglyceride Lowering by Dose
If you have high triglycerides, the dose of omega-3 matters enormously. According to an American Heart Association science advisory, prescription omega-3s at 4 grams per day (delivering about 3.4 grams of EPA and DHA) lower triglycerides by roughly 20% to 30% in people with elevated levels. Cut that dose in half to 2 grams per day, and the reduction drops to a modest 11% to 15%, with some studies showing no significant difference from placebo at the lower dose. Over-the-counter supplements at typical doses of 1 gram or less have minimal impact on triglyceride numbers.
The Atrial Fibrillation Risk
Fish oil isn’t without cardiovascular downsides. A large prospective cohort study published in BMJ Medicine found that regular fish oil supplement use was associated with a 13% increased risk of developing atrial fibrillation (an irregular heart rhythm) in people who were otherwise healthy at baseline. The same study found a small, borderline increase in stroke risk among healthy users. This doesn’t mean fish oil causes atrial fibrillation in every person who takes it, but the signal has appeared consistently enough across multiple studies that it’s a real consideration, particularly for people taking higher doses.
Eating Fish vs. Taking Supplements
Whole fish may offer heart benefits that capsules don’t replicate. The Physician’s Health Study followed over 20,000 male doctors for 11 years and found that eating fish one to two times per week was associated with a lower risk of sudden cardiac death compared to eating fish less than once a month. A 2006 meta-analysis estimated that consuming roughly 250 to 500 mg per day of EPA and DHA through diet resulted in a 36% relative risk reduction in cardiovascular death, with no additional benefit from higher intakes.
Why might whole fish work better than pills? Fish replaces less healthy proteins in your diet, delivers omega-3s in their natural form alongside other nutrients like selenium and vitamin D, and the people who eat fish regularly tend to have other health-promoting habits. None of those factors come in a capsule.
Who Actually Benefits
The clearest evidence for fish oil’s heart benefits is narrow and specific. If you have elevated triglycerides and are already at high cardiovascular risk due to existing heart disease or diabetes with additional risk factors, prescription-strength EPA (4 grams daily) has strong trial evidence behind it. This is a medication prescribed and monitored by a doctor, not a supplement you pick up off the shelf.
For everyone else, the evidence is far less encouraging. Standard over-the-counter fish oil supplements at typical doses have failed to reduce heart attacks, strokes, or cardiovascular deaths in trial after trial. Eating fatty fish twice a week is a better-supported strategy for general heart health, and it comes with nutritional benefits that extend well beyond omega-3 content. If you’re currently taking fish oil capsules hoping to protect your heart, the honest takeaway from the research is that they’re unlikely to make a measurable difference for most people.
