Is Fish Oil the Same as Omega-3? Not Exactly

Fish oil and omega-3 are not the same thing. Fish oil is a supplement derived from fatty fish, while omega-3s are a family of essential fatty acids found in many different sources. Fish oil happens to be one of the most popular ways to get omega-3s, but it’s not the only one, and not all of the oil in a fish oil capsule is actually omega-3.

How Fish Oil and Omega-3 Are Related

Think of it this way: fish oil is the delivery vehicle, and omega-3 fatty acids are part of the cargo. A typical 1,000 mg fish oil capsule contains about 180 mg of EPA and 120 mg of DHA, the two omega-3s your body uses most readily. That means only about 30% of what’s in the capsule is actually omega-3. The rest is other fats naturally present in fish.

This distinction matters when you’re shopping for supplements. A label that says “1,000 mg fish oil” is not the same as “1,000 mg omega-3.” You need to flip the bottle over and check how much EPA and DHA are listed per serving. Some concentrated formulas pack 600 to 900 mg of combined EPA and DHA into the same size capsule, while budget options may stick closer to that 300 mg baseline.

The Three Main Types of Omega-3

Omega-3 fatty acids come in three forms that matter for nutrition. EPA and DHA are the two your body can put to work directly. They’re found in fatty fish (salmon, mackerel, sardines, anchovies), and by extension, in fish oil supplements. The third type, ALA, comes from plant sources like flaxseed, chia seeds, walnuts, and canola oil.

ALA is technically an essential nutrient, meaning your body can’t make it, but it’s not interchangeable with EPA and DHA. Your body has to convert ALA into EPA and DHA before it can use it in the same ways, and that conversion is remarkably inefficient. In healthy young men, roughly 8% of dietary ALA converts to EPA and somewhere between 0% and 4% converts to DHA. Women do somewhat better, converting about 21% to EPA and 9% to DHA, likely due to the influence of estrogen on the conversion pathway. So while flaxseed oil is a genuine source of omega-3, it’s a poor substitute if your goal is to raise EPA and DHA levels specifically.

Sources Beyond Fish Oil

Fish oil is the most common omega-3 supplement, but several alternatives exist. Krill oil, made from tiny crustaceans, delivers EPA and DHA in a slightly different molecular form and in smaller doses per capsule. Cod liver oil provides EPA and DHA along with vitamins A and D, though the omega-3 content per serving is generally lower than standard fish oil.

For people who avoid animal products, algae oil is the only plant-based source that provides DHA directly, and some products also contain EPA. This makes sense biologically: fish are rich in omega-3s because they eat algae (or eat smaller fish that ate algae). Algae oil cuts out the middleman. Most algal supplements emphasize DHA over EPA, so if you need both, check the label carefully.

Not All Fish Oil Absorbs the Same Way

Fish oil supplements come in two main molecular forms, and the difference affects how well your body absorbs them. In whole fish, omega-3s exist naturally as triglycerides. Many supplements, however, use an ethyl ester form, which is cheaper to manufacture. Ethyl esters are essentially omega-3 molecules bonded to an ethanol molecule instead of a fat backbone. Your body has to strip off the ethanol and rebuild the fat before it can absorb and use the omega-3.

That extra step slows things down considerably. The digestive enzyme responsible for breaking down fats processes ethyl ester fish oil 10 to 50 times more slowly than the triglyceride form. In one human study comparing the two, EPA and DHA from the triglyceride form absorbed at 57% to 68%, while the ethyl ester form absorbed at only about 20% to 21%. Another study found that ethyl ester EPA showed up in the blood at roughly one-third the level of other forms and peaked three hours later.

The majority of fish oil products on the market use the ethyl ester form. If absorption matters to you, look for labels that specify “triglyceride form” or “rTG” (re-esterified triglyceride). These products typically cost more, but you may need fewer capsules to reach the same effective dose.

Freshness and Quality Markers

Fish oil is a fat, and fats go rancid. Oxidized fish oil not only tastes and smells worse, it may not deliver the same benefits. The supplement industry uses two key measurements to gauge freshness: peroxide value, which captures early-stage oxidation, and anisidine value, which captures later-stage breakdown. The Global Organization for EPA and DHA Omega-3s (GOED) sets voluntary limits: a peroxide value under 5 and an anisidine value under 20 for finished products.

A few practical signs can help you at home. Fishy-tasting burps are normal with low-quality oil but less common with fresh, well-made products. If you bite into a capsule and it tastes strongly rancid or bitter, that’s a red flag. Storing fish oil in the refrigerator or freezer slows oxidation. Checking for a third-party testing seal (such as IFOS or USP) is one way to verify that a product has been independently assessed for purity and freshness. These programs test for contaminants like mercury (held to limits of 0.1 parts per million or less) and PCBs (45 parts per billion or less).

Reading the Label Correctly

The single most useful thing you can do when choosing an omega-3 supplement is ignore the front of the bottle and go straight to the Supplement Facts panel. Look for milligrams of EPA and milligrams of DHA per serving. Add those two numbers together. That combined total is what actually matters for comparison shopping. A “triple strength” fish oil with 900 mg of combined EPA and DHA per capsule delivers three times more active omega-3 than a standard capsule with 300 mg, even though both might come in similarly sized softgels.

Also check the serving size. Some brands list impressive omega-3 totals but define a serving as two or three capsules. Per-capsule math gives you the real comparison. If you’re taking fish oil for general health, most major health organizations suggest somewhere around 250 to 500 mg of combined EPA and DHA per day from food and supplements together. Higher amounts are sometimes used for specific health goals, but the baseline is achievable with one or two standard capsules daily, or two servings of fatty fish per week.
