Is Gabapentin a Sedative for Cats? Uses and Side Effects

Gabapentin is not technically a sedative, but it’s one of the most commonly prescribed medications for calming cats before vet visits, travel, and other stressful events. It works primarily by reducing anxiety and fear, and drowsiness is a frequent side effect rather than its main purpose. Veterinarians have widely adopted it as a go-to option for making cats easier to handle, even though it was originally developed as a pain and seizure medication for humans.

How Gabapentin Calms Cats

Gabapentin binds to a specific part of calcium channels on nerve cells, reducing the flow of calcium into those cells. This slows down the transmission of excitatory signals in the nervous system. The result is a dampening of pain signals and a reduction in the heightened nerve activity that drives fear and anxiety responses.

The drug also has effects higher up in the brain, where it influences the release of several chemical messengers involved in mood and alertness. It activates a descending pain-inhibition pathway, which is why it pulls double duty as both an anxiety reducer and a pain reliever. For cats dealing with both chronic pain and stress (common in older cats), this combination effect is particularly useful.

What It’s Used For

The most popular use of gabapentin in cats is reducing fear before veterinary visits. A single dose given at home before the appointment can make the difference between a cat that’s hissing, biting, and impossible to examine and one that tolerates handling. Studies have consistently shown that gabapentin reduces perceived distress and increases compliance during veterinary exams compared to placebo.

Beyond vet visits, gabapentin is used for travel anxiety, carrier training, and managing chronic nerve pain from conditions like arthritis or injuries. In trap-neuter-return programs for community cats, it’s given to reduce fear in cats confined to cage traps. A field trial of community cats found that both 50 mg and 100 mg doses reduced fear responses in confined cats without producing measurable sedation over three hours, reinforcing the idea that the drug works more on anxiety than on consciousness itself.

Typical Dosing and Timing

Gabapentin reaches its peak concentration in a cat’s blood about one hour after being swallowed, and its effects wear off with a half-life of roughly 3.5 to 4 hours. That means the calming effects are strongest in the first couple of hours and gradually fade over the following several hours. Veterinarians typically recommend giving it at least 90 minutes before the stressful event.

Doses for anxiety reduction generally fall in the range of 50 to 100 mg per cat, with some veterinary recommendations going up to 150 mg per cat. In terms of body weight, a range of roughly 3 to 10 mg/kg is considered reasonable for pre-visit anxiety, though doses up to 20 mg/kg or higher have been studied for pain management. Your vet will choose a dose based on your cat’s size, age, and health status. It comes in capsules or can be compounded into a liquid.

A Warning About Liquid Formulations

Many commercially available liquid gabapentin products made for humans are sweetened with xylitol, which is toxic to dogs. While xylitol toxicity is better documented in dogs than cats, the safest approach is to use veterinary-compounded liquid formulations rather than grabbing a human product off the pharmacy shelf.

How Sedated Will Your Cat Actually Be?

This varies a lot from cat to cat. In one study of healthy cats given 10 mg/kg, gabapentin alone did not produce statistically significant sedation compared to placebo. Other studies at higher doses do show noticeable drowsiness. About half of cats in one trial showed changes in coordination and gait after a dose, with roughly a third of younger cats developing mild to moderate wobbliness (ataxia). A small number of cats became noticeably sluggish or obtunded: one out of 32 younger cats and two out of 12 geriatric cats in that study.

In other words, some cats will seem barely affected while others will look quite drowsy and unsteady on their feet. Older cats tend to be more sensitive to the sedative effects. Trying the medication at home before the actual stressful event is a common recommendation so you know how your individual cat responds.

Gabapentin vs. Trazodone

Trazodone is the other medication frequently prescribed for pre-visit calming in cats. A study comparing the two head-to-head found that trazodone at 5 mg/kg produced more consistent and reliable sedation than gabapentin at 10 mg/kg. Gabapentin alone at that dose failed to produce significant sedation in the study population, while trazodone did. Combining both drugs together produced the most profound sedation of any group, with no observed side effects in healthy cats.

This suggests that for cats who don’t respond well to gabapentin alone, adding trazodone (or switching to it) may be more effective. Both drugs have good safety profiles in healthy cats, but the choice depends on whether you’re aiming more for anxiety reduction, true sedation, or both.

Side Effects to Watch For

The most common side effects are sedation itself (when it’s more intense than intended) and ataxia, that wobbly, uncoordinated walk. Vomiting and drooling (hypersalivation) have been reported but are uncommon. In the study that tracked neurological effects closely, about 32% of younger cats and 42% of geriatric cats developed new gait abnormalities after a dose. A small number temporarily lost their blink reflex in response to visual threats, which resolved on its own.

These effects are generally short-lived given the drug’s relatively short half-life, but they can look alarming if you’re not expecting them. A cat stumbling around the house for a few hours after a dose is not unusual and doesn’t typically indicate a dangerous reaction.

Cats With Kidney Disease Need Lower Doses

Gabapentin is cleared from the body through the kidneys, which matters because chronic kidney disease is extremely common in older cats. Research has shown that cats with kidney disease accumulate significantly higher blood levels of gabapentin than healthy cats given the same dose per kilogram. Their drug-normalized blood concentrations were higher even when given half the dose that healthy cats received.

The correlation is straightforward: the worse the kidney function, the higher the gabapentin levels climb. In clinical practice, a dose reduction of at least 50% is commonly used for cats with kidney disease. Without that adjustment, these cats are at higher risk for excessive sedation and drops in blood pressure. If your cat has been diagnosed with any stage of kidney disease, the gabapentin dose will need to be tailored accordingly.
