Gabapentin is not an NSAID. It belongs to a completely different drug class called anticonvulsants (sometimes called anti-seizure medications), and it works through an entirely different mechanism in the body. The confusion is understandable, though, because gabapentin is frequently prescribed for pain, which is also the primary reason people take NSAIDs like ibuprofen or naproxen.
How Gabapentin and NSAIDs Differ
The difference between gabapentin and NSAIDs comes down to where and how each drug works in your body.
NSAIDs reduce pain by blocking enzymes called COX-1 and COX-2. These enzymes produce prostaglandins, chemicals your body releases during injury or illness that cause inflammation, swelling, and pain. By shutting down prostaglandin production, NSAIDs tackle pain at its source in the inflamed tissue. Common NSAIDs include ibuprofen (Advil, Motrin), naproxen (Aleve), and aspirin.
Gabapentin works in the nervous system instead. It binds to specific calcium channels on nerve cells in the brain and spinal cord, reducing the release of excitatory chemical signals that amplify pain. In simpler terms, gabapentin turns down the volume on overactive nerves rather than reducing inflammation in the affected area. Despite its name resembling GABA (a calming brain chemical), gabapentin does not actually interact with GABA receptors or affect GABA levels in your brain.
Why Gabapentin Gets Prescribed for Pain
Gabapentin is primarily used for two types of pain that NSAIDs handle poorly: nerve pain and seizure-related conditions. Its main approved uses are for epilepsy and postherpetic neuralgia, the burning nerve pain that can linger for months or years after a shingles outbreak. Doctors also prescribe it off-label for other types of nerve pain, restless legs syndrome, and certain anxiety conditions.
The typical dose for nerve pain in adults ranges from 900 to 3,600 mg per day, split into three doses. Doctors usually start low and increase gradually over several days to reduce side effects like drowsiness and dizziness. This slow ramp-up is very different from NSAIDs, which generally work at full strength from the first dose.
Nerve pain feels different from the aching, throbbing pain of a sprained ankle or sore muscle. It tends to burn, tingle, or shoot, and it happens because the nerves themselves are damaged or misfiring. NSAIDs do little for this kind of pain because the problem isn’t inflammation in the traditional sense. That’s where gabapentin fills a gap that NSAIDs can’t.
Does Gabapentin Reduce Inflammation at All?
This is where things get interesting. Animal studies have found that gabapentin may have some anti-inflammatory effects, but through completely different pathways than NSAIDs. In lab and animal models, gabapentin has been shown to suppress inflammatory signaling pathways and reduce levels of key inflammatory molecules like IL-6, IL-1β, and TNF-α. Researchers have described it as a “multi-targeted immunomodulator” because it appears to influence several inflammatory pathways at once rather than working through a single mechanism like COX inhibition.
However, no clinical trials have confirmed these anti-inflammatory effects in humans. The evidence so far comes entirely from preclinical research, so gabapentin should not be considered a substitute for anti-inflammatory drugs in practice.
Can You Take Gabapentin With an NSAID?
Because gabapentin and NSAIDs work through entirely different mechanisms, they can often be used together. In fact, research suggests the combination may work better than either drug alone for certain types of pain. A study on gabapentin combined with naproxen found that the two drugs interacted synergistically to reduce pain sensitivity associated with inflammation, meaning the combined effect was greater than you’d expect from simply adding the two together. Importantly, neither drug altered the blood levels of the other, suggesting they don’t interfere with each other’s absorption or metabolism.
This combination approach is sometimes used for patients dealing with pain that has both inflammatory and nerve-related components, such as pain after surgery or certain chronic pain conditions.

Gabapentin’s side effects reflect its action on the nervous system. Drowsiness, dizziness, and fatigue are the most common complaints, especially when starting the medication or increasing the dose. Some people experience coordination problems or blurred vision. Unlike NSAIDs, gabapentin does not irritate the stomach or increase bleeding risk, which makes it a useful option for people who can’t tolerate anti-inflammatory drugs.
