Gaviscon is not a PPI. It is classified as an antacid and works through an entirely different mechanism than proton pump inhibitors like omeprazole or lansoprazole. While both are used to treat heartburn and acid reflux, they belong to separate drug categories and affect your body in fundamentally different ways.
How Gaviscon Actually Works
Gaviscon’s active ingredients are aluminum hydroxide and magnesium carbonate, both of which neutralize stomach acid on contact. But what sets Gaviscon apart from a basic antacid is its other key ingredient: sodium alginate, listed among its inactive ingredients in U.S. formulations but central to how the product performs.
When sodium alginate meets stomach acid, it forms a gel. Carbon dioxide gets trapped inside that gel, creating a buoyant “raft” that floats on top of your stomach contents. This physical barrier sits right where acid would normally splash up into your esophagus, blocking reflux at its source. It also displaces what’s known as the postprandial acid pocket, a concentrated pool of acid that forms near the top of the stomach after meals. On top of that, the raft can bind to digestive enzymes and bile, pulling them out of any material that does reflux upward.
The relief is essentially immediate. Because Gaviscon creates a physical barrier rather than waiting for a chemical process to unfold in your cells, it starts working as soon as the raft forms in your stomach.
How PPIs Work Differently
Proton pump inhibitors take the opposite approach. Instead of blocking acid after it’s produced, they shut down the tiny pumps in your stomach lining that manufacture acid in the first place. This is a systemic, chemical process. After you swallow a PPI, it gets absorbed into your bloodstream, travels to the acid-producing cells in your stomach wall, and permanently disables individual proton pumps. Your body then has to grow new pumps to replace them.
This means PPIs don’t provide instant relief. Omeprazole, one of the most common PPIs, inhibits about 70% of proton pumps by the first day but doesn’t reach full effectiveness until around day three. Your stomach is still producing some acid during that ramp-up period. Once a PPI is fully active, though, it suppresses acid production around the clock, making it more suitable for healing damage to the esophagus over weeks of treatment.
Gaviscon vs. PPIs for Symptom Relief
Despite their different mechanisms, the two can perform surprisingly similarly for moderate reflux symptoms. In a direct comparative trial, the average time to a first full heartburn-free day was 2.0 days for both Gaviscon and omeprazole. The difference is in the pattern of relief: Gaviscon works within minutes of each dose but wears off, while a PPI builds up sustained acid suppression over several days.
For occasional heartburn after a heavy meal, Gaviscon’s rapid action is the main advantage. For persistent reflux that’s causing inflammation or erosion in the esophagus, PPIs are generally the preferred choice because they maintain consistent acid reduction throughout the day and night.
Using Gaviscon Alongside a PPI
Some people already on a daily PPI still get breakthrough symptoms, and Gaviscon can serve as a rescue option in those situations. A randomized clinical trial tested this approach in over 300 patients who were taking a standard PPI dose but still experiencing reflux. In an initial exploratory study, 75% of patients adding Gaviscon after meals and at bedtime achieved meaningful symptom reduction, compared to 36% on placebo. However, a larger confirmatory study found the gap narrowed considerably, with 51% responding to Gaviscon versus 48% to placebo. The researchers noted that breakthrough symptoms sometimes have a functional component (meaning the esophagus is sensitive even without excess acid), which may explain why a physical barrier doesn’t always help.
If you’re considering adding Gaviscon to an existing PPI regimen, spacing them apart is a reasonable precaution. Antacids can interfere with how other medications are absorbed, so taking Gaviscon at least an hour or two before or after your PPI helps ensure both work as intended.
Safety Differences Worth Knowing
One reason people search for alternatives to PPIs is concern about long-term use. PPIs have been linked in observational studies to issues like reduced mineral absorption, changes in gut bacteria, and increased fracture risk when taken for years. These associations remain debated, but they make many people uncomfortable staying on a PPI indefinitely.
Gaviscon’s safety profile is generally simpler because it acts locally in the stomach rather than systemically. It doesn’t get absorbed into your bloodstream in a meaningful way. The main consideration is sodium. A standard 10 mL dose of Gaviscon oral suspension contains about 143 mg of sodium, which is roughly 7% of the World Health Organization’s recommended daily maximum. If you’re taking multiple doses per day, that adds up. This is particularly relevant if you’re on a salt-restricted diet due to high blood pressure, heart failure, or kidney problems.
Aluminum-containing antacids can also cause constipation, while magnesium-based ones tend toward the opposite effect. Gaviscon contains both, which partially balances things out, but digestive changes are still possible with regular use.
