Is Guanfacine a Controlled Substance for ADHD?

Guanfacine is not a controlled substance. It has no DEA schedule classification, no known potential for abuse, and no recognized risk of dependence. This puts it in a fundamentally different legal category than the stimulant medications most commonly prescribed for ADHD, like Adderall and Ritalin, which are Schedule II controlled substances.

Because guanfacine is unscheduled, the prescribing process is simpler than what you may be used to with stimulant ADHD medications. Schedule II drugs like amphetamine (Adderall) and methylphenidate (Ritalin) require a new written prescription each time, cannot be called in to a pharmacy in most states, and typically limit you to a 30-day supply with no automatic refills. Guanfacine takes a narrower approach. It activates a specific type of receptor in the prefrontal cortex, the brain region responsible for attention, impulse control, and working memory. By strengthening the signaling between neurons in this area, it improves focus and reduces impulsivity without triggering the reward circuits that drive addiction. There is no “high” associated with taking guanfacine, even at higher doses.

Research has actually explored guanfacine as a tool for reducing drug cravings. In a study of cocaine-dependent individuals, those taking guanfacine experienced lower stress-related and cue-related cravings compared to a placebo group, along with increased activity in prefrontal brain regions associated with self-control. How Guanfacine Is Used for ADHD

Guanfacine comes in two forms. The extended-release version, sold as Intuniv, is FDA-approved for ADHD in children and adolescents. It can be used on its own or added to a stimulant medication that isn’t fully controlling symptoms. The immediate-release version, originally marketed as Tenex, was first developed to treat high blood pressure and is sometimes prescribed off-label for ADHD. Side Effects to Expect

The most common side effect of guanfacine is drowsiness. Because the medication lowers norepinephrine activity, it also lowers blood pressure and heart rate, which can cause fatigue, dizziness, or lightheadedness, particularly during the first few weeks or after a dose increase. These effects often improve as your body adjusts. Some people take it in the evening specifically because of the sedating effect.

Guanfacine’s blood pressure-lowering action is worth knowing about even if you’re taking it for ADHD rather than hypertension. It’s the same drug doing the same thing to your cardiovascular system regardless of the reason it was prescribed.

Why You Shouldn’t Stop It Abruptly

Even though guanfacine isn’t a controlled substance and doesn’t cause psychological dependence, stopping it suddenly is not safe. Your body adjusts to the blood pressure-lowering effect over time, and abrupt discontinuation can cause a rebound increase in blood pressure. In clinical observations of patients who stopped guanfacine after long-term use, blood pressure gradually climbed back up over two to four days, sometimes overshooting pre-treatment levels. The standard approach is to taper the dose down gradually rather than quitting all at once.

This is a physiological rebound effect, not withdrawal in the addiction sense. It’s comparable to what happens when you suddenly stop certain blood pressure medications or corticosteroids. Your body needs time to recalibrate.
