Is Hydroxyzine Stronger Than Benadryl for Sleep?

Hydroxyzine is generally considered a stronger sedative than Benadryl (diphenhydramine), though both medications cause significant drowsiness. Hydroxyzine produces longer-lasting sedation because it stays active in your body roughly four to five times longer than Benadryl, and its sedative effects have been consistently demonstrated in clinical studies at standard doses. That said, “stronger” doesn’t necessarily mean “better for sleep,” and neither drug is ideal as a long-term sleep solution.

How the Two Compare for Sedation

Both hydroxyzine and diphenhydramine are first-generation antihistamines, meaning they cross into the brain and block the same receptor that helps regulate wakefulness. The drowsiness you feel from either one isn’t a side effect being repurposed; it’s a direct result of that brain activity being suppressed.

In a crossover study of 20 healthy adults comparing single doses of hydroxyzine 50 mg, diphenhydramine 50 mg, and placebo, both drugs increased subjective sleepiness compared to placebo. Hydroxyzine specifically increased a brain-wave marker associated with slower cognitive processing, along with self-reported somnolence. A separate study found that a single 25 mg dose of hydroxyzine produced significant sedation and slower reaction times for at least four hours after taking it. Diphenhydramine at 50 mg three times daily also significantly reduced the time it took people to fall asleep compared to placebo.

The practical difference comes down to duration. Diphenhydramine has a half-life of about 4 hours, meaning its sedative punch fades relatively quickly. Hydroxyzine’s half-life is roughly 14 to 20 hours, so its effects linger well into the next day. This can be a benefit if you wake up frequently during the night, but it also means you’re more likely to feel groggy the next morning.

Why Hydroxyzine Feels Different

Hydroxyzine does something Benadryl doesn’t: it also has anti-anxiety properties. It’s prescribed for generalized anxiety, and the calming effect it provides can make falling asleep feel easier, especially if racing thoughts are part of what keeps you up. This dual action is why many prescribers reach for hydroxyzine over Benadryl when a patient reports both anxiety and insomnia.

Benadryl, on the other hand, is available over the counter and works faster for most people. Its shorter duration means less next-day impairment, which matters if you need to drive or concentrate in the morning. For occasional sleeplessness with no underlying anxiety, some people actually prefer Benadryl’s quicker in-and-out profile.

Anticholinergic Side Effects

Both drugs score a 3 out of 3 on the Anticholinergic Cognitive Burden Scale, the highest category. That means both carry the potential to cause dry mouth, blurred vision, constipation, urinary retention, and confusion. At this level, both are classified as medications that “may cause delirium,” particularly in older adults.

The side effect profiles are similar enough that choosing one over the other for tolerability reasons doesn’t make much practical difference. If you’re sensitive to anticholinergic effects with Benadryl, you’ll likely experience them with hydroxyzine too. Older adults are especially vulnerable. Research from Indiana University’s Center for Aging Research found that each definite anticholinergic medication (score of 2 or 3) may increase the risk of cognitive impairment by 46% over six years.

Tolerance Builds Quickly With Both

One of the biggest limitations of using either drug for sleep is that your body adapts fast. Experts at Baylor College of Medicine have warned that most people develop tolerance to the sedative effects of antihistamines very quickly, sometimes within days. Once tolerance sets in, the drug stops helping you sleep, but the anticholinergic side effects remain. This often leads people to increase their dose, which raises the risk of side effects without restoring the original sleep benefit.

This tolerance issue is why neither hydroxyzine nor diphenhydramine is recommended as a long-term sleep aid. They can be useful for a few rough nights, but if you’re relying on either one regularly, the sedation you felt the first week will fade.

Long-Term Risks Worth Knowing

A large retrospective study published in The Journal of Allergy and Clinical Immunology: In Practice tracked over 714,000 patients in Taiwan and found a dose-dependent increase in dementia risk among first-generation antihistamine users. Those with the highest cumulative use had a 51% greater risk of dementia compared to nonusers. Even second-generation antihistamines (the non-drowsy types) showed an elevated risk at high cumulative doses, though the increase was smaller at 26%.

This doesn’t mean taking hydroxyzine or Benadryl a few times will cause cognitive decline. The risk scales with how much you take over how many years. But it’s a meaningful consideration if you’ve been using either drug nightly for months or longer.

Which One Is Right for You

If you’re comparing the two purely on sedative strength, hydroxyzine is the more potent and longer-lasting option. It also addresses anxiety, which makes it a better fit when stress or worry is the root of your sleep trouble. The tradeoff is more next-day grogginess and the need for a prescription.

Benadryl is easier to access, wears off faster, and works well enough for the occasional sleepless night. It’s not a good choice for regular use, and neither is hydroxyzine. Both carry the same anticholinergic burden, both lose their sedative effect as tolerance develops, and both are associated with cognitive risks at high cumulative doses. For persistent insomnia, treatments that target the actual mechanics of sleep, like cognitive behavioral therapy for insomnia, tend to outperform both medications without the same long-term concerns.
