Insomnia is not listed as a common side effect of Wegovy, and clinical trial data suggest it occurs at roughly the same rate in people taking the medication as in those taking a placebo. That said, some users do report sleep difficulties, and there are plausible reasons why Wegovy could disrupt sleep indirectly, even if the drug itself isn’t directly causing insomnia.
What Clinical Trials Actually Show
Across the four major clinical trials used to evaluate Wegovy (known as the STEP 1 through STEP 4 trials), insomnia was reported by 1.7% to 4.7% of participants taking semaglutide. In the placebo groups, the rate was nearly identical: 1.0% to 4.9%. That overlap is important. It means the drug didn’t produce a statistically meaningful increase in insomnia compared to a sugar pill.
Because of this, the FDA does not list insomnia among Wegovy’s common adverse reactions. The official prescribing information only flags side effects occurring in 5% or more of users, and insomnia falls well below that threshold. It doesn’t appear in any of the adverse reaction tables for adults or adolescents.
Why Some People Still Lose Sleep
Even though the clinical data don’t point to a direct link, plenty of people on Wegovy report trouble sleeping. There are a few reasons this can happen that don’t require the drug to be acting on your brain’s sleep centers.
The most likely culprit is gastrointestinal discomfort. Nausea, bloating, and acid reflux are among Wegovy’s most frequent side effects, especially during the dose escalation phase in the first several months. Wegovy slows gastric emptying, meaning food sits in your stomach longer than usual. This can worsen or trigger gastroesophageal reflux, particularly in people who already deal with it. If you’ve ever tried to fall asleep with heartburn or a heavy feeling in your stomach, you know how disruptive that can be. Clinical guidelines for managing these symptoms specifically recommend avoiding lying down shortly after meals, which suggests the connection between GI side effects and poor sleep is well recognized.
Rapid weight loss itself can also affect sleep. As your body composition changes, so can factors like sleep apnea severity, hormone levels, and overall energy regulation. Some people find their sleep improves as they lose weight (especially if sleep apnea was an issue), while others experience a temporary adjustment period.
How GLP-1 Affects Your Body Clock
There is a biological reason to take sleep changes on Wegovy seriously, even if the clinical trial numbers are reassuring. Semaglutide, the active ingredient in Wegovy, mimics a hormone called GLP-1 that your body naturally produces. Researchers are increasingly finding that GLP-1 signaling is tightly connected to circadian timing systems, the internal clocks that govern your sleep-wake cycle.
GLP-1 receptors are found in the hypothalamus, the brain region that coordinates circadian rhythms, hunger, and metabolism. GLP-1 signaling appears to be regulated by your body clock, and it may influence those clock systems in return. This creates a feedback loop: when you introduce a powerful GLP-1 mimic like semaglutide, it could theoretically nudge your circadian rhythms in subtle ways. The science here is still early, but it offers a plausible explanation for why some individuals notice sleep changes that don’t show up as a clear signal in large trials.
Practical Ways to Protect Your Sleep
If you’re experiencing insomnia or restless nights after starting Wegovy, a few adjustments can help. The most impactful one is timing your last meal. Eating dinner at least three hours before bed reduces the chance that slowed digestion will cause reflux or discomfort while you’re lying down. Smaller, lower-fat meals in the evening are easier on a stomach that’s already emptying slowly.
Elevating the head of your bed slightly (not just adding pillows, but tilting the whole surface) can reduce nighttime reflux if that’s contributing to wakefulness. Staying hydrated throughout the day matters too, since nausea and reduced appetite can lead to lower fluid intake, and even mild dehydration affects sleep quality.
Pay attention to the timing of your injection as well. Some people find that injecting in the morning versus evening changes the pattern of side effects they experience. There’s no official guidance on this, but experimenting with injection timing is low-risk and worth trying if sleep disruption follows a predictable pattern after your weekly dose.
