Sleeping on your left side does offer several measurable health advantages over other positions, particularly for digestion, breathing during sleep, and pregnancy. The benefits come down to anatomy: your stomach, spleen, and major lymphatic structures sit on the left side of your body, and gravity works in your favor when you sleep on that side. That said, left-side sleeping isn’t ideal for everyone, especially people with certain heart conditions.
Why Left-Side Sleeping Helps Digestion
The most well-supported benefit of left-side sleeping involves acid reflux. The American Gastroenterological Association recommends sleeping on your left side specifically to reduce nighttime acid exposure in the esophagus. The reason is straightforward: your stomach curves to the left, and its junction with the esophagus sits higher than the rest of the stomach when you’re on your left side. Gravity keeps acid pooled away from that opening rather than letting it creep upward.
When you sleep on your right side, this geometry flips. Stomach acid can more easily reach the esophagus because the junction sits lower relative to the pool of acid. If you deal with heartburn or gastroesophageal reflux disease (GERD), switching from your right side to your left can make a noticeable difference in how often you wake up with that burning sensation.
Side Sleeping and Your Breathing
If you snore or have obstructive sleep apnea, sleeping on either side is significantly better than sleeping on your back. When you lie face-up, gravity pulls the tongue and soft tissues in the throat backward, partially blocking the airway. Rolling onto your side keeps the airway more open. A meta-analysis of positional therapy research found that people with sleep apnea who switched from back sleeping to side sleeping reduced their breathing disruptions by about 54%. That’s a substantial improvement from simply changing position, with no device or treatment involved.
There’s no strong evidence that the left side is better than the right for airway function specifically. The key distinction here is side versus back, not left versus right.
Brain Waste Clearance During Sleep
Your brain has its own waste-removal system that becomes most active during sleep, flushing out metabolic byproducts including proteins linked to Alzheimer’s disease. A study published in The Journal of Neuroscience found that this cleaning process was most efficient when subjects were in a lateral (side) position compared to lying on the back or stomach. The researchers proposed that side sleeping may have evolved specifically because it optimizes this waste clearance.
This research was conducted in rodents and used imaging to track how quickly waste compounds cleared from brain tissue. Both side and back positions outperformed stomach sleeping, but side sleeping showed the best overall transport efficiency. Human studies are still catching up to these findings, but the anatomical logic holds: side positioning appears to help cerebrospinal fluid circulate more effectively through the brain’s drainage pathways.
Pregnancy and Blood Flow
For pregnant women, left-side sleeping becomes increasingly important as the pregnancy progresses. Stanford Medicine recommends side sleeping during pregnancy generally, with the left side as the preferred choice because it allows the most blood flow to the baby. The reason involves a large vein called the inferior vena cava, which runs along the right side of the spine and carries blood back to the heart from the lower body. When a pregnant woman lies on her back, the weight of the uterus compresses this vein, reducing blood return to the heart and potentially limiting blood flow to the placenta. Lying on the left side takes pressure off the vein entirely.
If you’re pregnant and find yourself waking up on your back, don’t panic. Simply roll to your side when you notice. Many women use a body pillow behind them to discourage rolling onto their back during the night.
Lymphatic Drainage and the Spleen
Your body’s lymphatic system, which filters waste and helps fight infection, is predominantly structured on the left side. The thoracic duct, the largest lymphatic vessel in the body, drains into the left side of the venous system. Your spleen, which filters blood and functions like a large lymph node, also sits on the left. Sleeping on your left side lets gravity assist this drainage rather than working against it.
This doesn’t mean sleeping on your right side shuts down your lymphatic system. It functions in any position. But for people who experience swelling, fluid retention, or sluggish lymphatic flow, left-side sleeping provides a small mechanical advantage.
When Left-Side Sleeping Isn’t Ideal
People with heart failure often find that sleeping on their left side makes breathing more difficult. The heart sits slightly left of center in the chest, and in people whose hearts aren’t pumping effectively, the added pressure from lying on that side can worsen shortness of breath. According to the American Heart Association, many heart failure patients naturally prefer sleeping on their right side for this reason. If you find that lying on your left side consistently makes breathing uncomfortable, that’s worth mentioning to your doctor.
Shoulder pain is another common barrier. If you have a left shoulder injury or chronic pain, forcing yourself onto that side all night will likely make things worse. The health benefits of left-side sleeping don’t outweigh the cost of disrupted, painful sleep.
How to Sleep Comfortably on Your Side
Proper alignment matters more than which side you choose. Poor side-sleeping posture can lead to neck pain, back pain, and hip stiffness that make you dread going to bed. UCLA’s ergonomics guidelines offer a few practical rules that make a real difference:
- Pillow height: Your pillow needs to fill the gap between your ear and the mattress completely. Side sleepers need a thicker pillow than back sleepers to keep the neck in a neutral line with the spine. If your head tilts downward toward the mattress, the pillow is too thin.
- Knee pillow: Place a pillow between your knees to keep your hips level. Without it, the top leg pulls the pelvis forward and twists the lower spine.
- Leg position: Keep your top leg even with or slightly behind your bottom leg, with both knees gently bent. Drawing the knees too far up toward your chest rounds the lower back and can create stiffness by morning.
If you’re trying to train yourself to stay on your side, a body pillow running the length of your torso gives you something to anchor against and makes rolling onto your back less likely. Some people also place a tennis ball in a pocket sewn onto the back of a sleep shirt, which creates just enough discomfort to discourage back sleeping without fully waking you up.
