Is It Dangerous for Adults to Get Chickenpox?

Yes, chickenpox is significantly more dangerous for adults than for children. Adults face higher rates of hospitalization, more frequent complications like pneumonia and brain inflammation, and a greater risk of death. Before the vaccine became available in the mid-1990s, 100 to 150 people died from chickenpox each year in the United States, and adults accounted for a disproportionate share of those deaths despite making up a small fraction of total cases.

Why Adults Get Sicker Than Children

Children typically experience chickenpox as an uncomfortable but manageable illness: a week or so of itchy blisters, mild fever, and fatigue. Adults with the same virus tend to develop more blisters, higher fevers, and symptoms that last longer. The illness often hits hard enough to keep an otherwise healthy adult in bed for a week or more.

The exact biological reason for this difference remains unclear. Researchers have identified that immune compromise, pregnancy, and simply being an adult are all independent risk factors for severe chickenpox, but there is no confirmed mechanism explaining why the adult immune system handles the virus so much worse. One theory is that a more mature immune system mounts a stronger inflammatory response to the infection, and that heightened response itself causes more tissue damage. Whatever the cause, the pattern is consistent: the older you are when you catch it, the rougher it tends to be.

Pneumonia: The Most Common Serious Complication

Lung inflammation is the leading cause of complicated chickenpox in adults, occurring in roughly 1 in 400 adult cases. The true number may be higher, since mild cases often go undiagnosed without a chest X-ray. Varicella pneumonia typically develops a few days after the rash appears, starting with a dry cough, shortness of breath, fever, and deep fatigue. In severe cases, it can progress to respiratory failure requiring hospitalization.

Most otherwise healthy adults recover fully, but the window between “I feel lousy” and “I need emergency care” can close quickly. If you develop chickenpox as an adult and notice increasing difficulty breathing, that warrants urgent medical attention.

Other Complications to Know About

Pneumonia gets the most attention, but it’s not the only risk. Adults with chickenpox can develop:

  • Bacterial skin infections. The open blisters create entry points for bacteria, most commonly group A streptococcus. These infections can range from localized cellulitis to serious bloodstream infections and, in rare cases, toxic shock syndrome.
  • Encephalitis. Inflammation of the brain occurs in a small percentage of adult cases and can cause confusion, seizures, and coordination problems. A related condition called cerebellar ataxia affects balance and movement.
  • Scarring. Adults tend to develop more lesions than children and are more likely to scratch them during the longer illness course, increasing the chance of permanent scarring.

Special Risks During Pregnancy

Pregnant women who catch chickenpox face a double threat. The most common maternal complication is pneumonia, which tends to be more severe during pregnancy than at other times. Beyond the mother’s health, the virus can affect the developing baby.

If infection occurs during the first 20 weeks of pregnancy, there is a small risk of congenital varicella syndrome, a rare but serious condition that can cause skin scarring, eye problems, brain abnormalities, and limb or gastrointestinal defects. The risk is highest between weeks 8 and 20. Chickenpox that develops around the time of delivery carries its own dangers, since the newborn can be exposed to the virus before developing any immune protection.

The Shingles Connection

Getting chickenpox at any age, whether as a child or an adult, creates a lifelong passenger in your nervous system. After the initial infection clears, the varicella-zoster virus retreats into nerve cells near the spine and brain, where it stays dormant. In roughly one-third of people who’ve had chickenpox, this dormant virus reactivates later in life as shingles: a painful, blistering rash that follows a single nerve path, often accompanied by burning pain that can persist for months or even years.

Catching chickenpox as an adult doesn’t necessarily make shingles more likely than catching it as a child. But it does mean you’re adding that latent virus to your system at an older age, closer to the period of life when shingles most commonly strikes (typically after 50).

Antiviral Treatment and Timing

If you do catch chickenpox as an adult, antiviral medication can reduce the severity and duration of symptoms, but timing matters. Treatment needs to start within 24 hours of the rash first appearing to be effective. Started in that window, it shortens the period of new blister formation, brings fevers down faster, and reduces overall symptom duration.

This narrow treatment window means you shouldn’t wait to see if things get worse. If you’re an adult who hasn’t had chickenpox or the vaccine and you develop a telltale rash of small, fluid-filled blisters, contact a healthcare provider the same day. Beyond 24 hours, antivirals offer much less benefit in otherwise healthy adults.

Vaccine Protection for Adults

The most effective way to avoid all of this is vaccination. Adults who have never had chickenpox and were never vaccinated can receive a two-dose vaccine series. In clinical trials, two doses were 98% effective at preventing any form of chickenpox and 100% effective against severe disease. Real-world studies after the vaccine’s rollout show slightly lower but still strong effectiveness: about 92% protection against all chickenpox, assessed roughly five years after vaccination.

The U.S. vaccination program, launched in 1995, has prevented an estimated 2,000 deaths over its first 25 years. Hospitalizations dropped 94% and deaths fell 97% among people 50 and younger. If you were born before the vaccine era (roughly before 1995 in the U.S.) and don’t remember having chickenpox, a blood test can check whether you carry antibodies. If you don’t, getting vaccinated as an adult is straightforward and dramatically reduces your risk of a potentially serious illness.
