Is It Good to Take a Multivitamin Every Day?

For most healthy adults who eat a reasonably balanced diet, a daily multivitamin is unlikely to prevent major diseases like cancer or heart disease. The U.S. Preventive Services Task Force reviewed the available evidence and concluded there isn’t enough proof to say multivitamins prevent cardiovascular disease or cancer in the general population. That said, a daily multivitamin isn’t useless. It can fill specific nutritional gaps that are surprisingly common, and for certain groups of people, it offers clear, measurable benefits.

Most People Don’t Get Enough Key Nutrients

The strongest argument for a daily multivitamin has nothing to do with disease prevention. It’s about covering basic nutritional shortfalls. National survey data from NHANES found that when looking at food intake alone, 94% of Americans fall short of the daily requirement for vitamin D, 89% for vitamin E, 52% for magnesium, and 44% for calcium. These aren’t exotic nutrients. They’re essential for bone health, immune function, muscle contraction, and cell protection.

A multivitamin won’t fix a poor diet, and it can’t replicate the fiber, healthy fats, and thousands of plant compounds you get from whole foods. But if your diet has blind spots, a basic multivitamin acts as a nutritional safety net, closing the gap between what you eat and what your body needs.

What the Evidence Says About Disease Prevention

If you’re taking a multivitamin hoping to lower your risk of a heart attack or stroke, the data is discouraging. A large meta-analysis published in an American Heart Association journal found no association between multivitamin use and death from cardiovascular disease, coronary heart disease, or stroke. An initial hint of reduced coronary heart disease risk disappeared when researchers looked only at the most rigorous randomized controlled trials.

The picture is similar for cancer. The USPSTF gave multivitamins an “I” grade for both heart disease and cancer prevention, meaning the evidence is simply insufficient to recommend them for those purposes. Notably, beta carotene and vitamin E supplements received a “D” grade, meaning they should be avoided entirely for disease prevention due to potential harms.

Where Daily Multivitamins Show Real Benefits

Memory and Cognition in Older Adults

One area where multivitamins have shown a measurable effect is brain health in people over 60. In the COSMOS study, more than 3,500 older adults were randomly assigned to take either a daily multivitamin or a placebo for three years. By the end of the first year, the multivitamin group showed improved memory compared to the placebo group. A separate COSMOS analysis of over 2,200 older adults found similar improvements in overall cognition, memory recall, and attention. In both studies, the benefits were more pronounced in participants with underlying cardiovascular disease.

Pregnancy and Folic Acid

For women who are pregnant or could become pregnant, a daily multivitamin containing folic acid is one of the clearest evidence-based recommendations in nutrition. Experts recommend at least 400 micrograms of folic acid daily, even for women who aren’t actively planning a pregnancy, because neural tube defects develop in the earliest weeks, often before a woman knows she’s pregnant. Most prenatal vitamins contain 800 to 1,000 micrograms. Women who have previously had a baby with a neural tube defect are advised to take a much higher dose of 5 milligrams daily.

Adults Over 50

As you age, your body becomes less efficient at absorbing certain nutrients from food. Vitamin B12 is a prime example. Between 5% and 15% of adults are deficient, and the rate climbs with age because the digestive system gradually loses its ability to extract B12 from food. Since B12 plays a role in red blood cell formation, nerve function, and bone health, a daily supplement (whether standalone or as part of a multivitamin) can help prevent a deficiency that develops slowly and is easy to miss until symptoms become serious.

Taking It at the Right Time Matters

Multivitamins contain both water-soluble vitamins (like the B vitamins and vitamin C) and fat-soluble vitamins (A, D, E, and K). The fat-soluble ones need dietary fat to be absorbed properly. This means you’ll get the most out of your multivitamin by taking it with a meal that contains some fat, even something as simple as eggs, avocado, nuts, or olive oil on a salad. Taking a multivitamin on an empty stomach can also cause nausea in some people, so pairing it with food solves both problems at once.

Synthetic vs. Food-Based Formulas

You’ll find multivitamins marketed as “whole food” or “organic” at a premium price, with the implication that they’re absorbed better than standard synthetic versions. A single-blinded randomized study compared the two directly and found that organic and synthetic multivitamin formulations produced comparable changes in circulating nutrient levels after both a two-hour and a 30-day ingestion period. The only notable difference was that the organic formula raised folate and transferrin (a protein involved in iron transport) slightly more. For most people, this distinction doesn’t justify a significant price difference.

Risks of Overdoing It

A standard once-daily multivitamin is generally safe, but problems arise when people stack a multivitamin on top of individual supplements or fortified foods without realizing how quickly certain nutrients add up. Preformed vitamin A, for example, has a tolerable upper limit of 3,000 micrograms per day for adults. Exceeding that over time can cause liver damage, bone loss, and birth defects during pregnancy. If you’re already eating fortified cereals, drinking fortified milk, and taking a separate vitamin A or fish oil supplement alongside your multivitamin, you could be closer to that ceiling than you think.

Iron is another nutrient to watch. Most multivitamins formulated for men or postmenopausal women contain little or no iron, but formulas marketed to younger women often include it. If you don’t have an iron deficiency, extra iron can cause digestive issues and, in rare cases, contribute to iron overload. The simplest safeguard is to avoid taking multiple supplements that overlap in their ingredients.

Who Benefits Most

A daily multivitamin makes the most practical sense for people who fall into one or more of these categories:

  • Adults over 50, particularly for B12 and vitamin D, which become harder to absorb or produce with age
  • Women who are or could become pregnant, for folic acid and iron
  • People on restricted diets, including vegans, those with food allergies, or anyone cutting out entire food groups
  • People with absorption issues, such as those with celiac disease or a history of gastric surgery

For a generally healthy adult with a varied diet, a multivitamin is unlikely to produce a noticeable change in how you feel or dramatically shift your disease risk. It won’t compensate for smoking, inactivity, or a diet built around processed food. But as an inexpensive way to cover nutritional blind spots, especially if your diet isn’t perfect every day, the risk is low and the potential upside is modest but real.
