Cyclobenzaprine doesn’t act directly on your muscles. It works in the brain and spinal cord, interrupting the nerve signals that keep muscles locked in spasm. This central action is why it causes drowsiness and mental fog in many people, and it’s also why mixing it with other substances that slow your nervous system can be dangerous.
The standard starting dose for the immediate-release tablet is 5 mg three times a day, which can be increased to 10 mg three times a day based on how you respond. It’s designed as a short-term fix to get you through the worst of an acute muscle injury, not as a long-term medication. The FDA label explicitly states that use beyond two to three weeks is not recommended.
Common Side Effects
The most frequently reported side effects are what you’d expect from a drug that acts on your brain: drowsiness, dry mouth, and dizziness. Many people also notice fatigue and a foggy, slightly “off” feeling. These effects are more pronounced at higher doses and tend to be strongest in the first few days of use.
Because it can significantly impair your alertness, you should avoid driving or operating heavy machinery until you know how it affects you. Some people tolerate it well at the 5 mg dose but feel heavily sedated at 10 mg, so starting low makes sense.
Who Should Not Take It
Cyclobenzaprine is off-limits for people with certain heart conditions. The FDA label specifically lists recent heart attack, irregular heart rhythms, heart block or other conduction problems, and congestive heart failure as contraindications. The drug’s chemical structure is closely related to older antidepressants that can affect heart rhythm, which is why these restrictions exist.
People with an overactive thyroid should also avoid it. If you have liver problems, your body may process the drug more slowly, leading to stronger and longer-lasting effects.
Risks for Older Adults
If you’re over 65, this medication carries extra risk. Cyclobenzaprine is listed on the American Geriatrics Society’s Beers Criteria, a well-known list of medications considered potentially harmful for older adults. The sedation and dizziness it causes increase fall risk, and older adults are more sensitive to its effects on heart rhythm and mental clarity. If you’re in this age group and your provider prescribes it, the dose will likely be lower and the duration shorter.
Dangerous Drug Interactions
The most serious interaction risk involves medications that raise serotonin levels in the brain. Combining cyclobenzaprine with certain antidepressants (particularly SSRIs, SNRIs, or MAO inhibitors) can trigger serotonin syndrome, a potentially life-threatening condition that causes rapid heart rate, high body temperature, agitation, and muscle rigidity. If you take any antidepressant, make sure your prescriber knows before you start cyclobenzaprine.
Other sedating medications amplify the drowsiness. Benzodiazepines, sleep aids, opioid painkillers, and antihistamines that cause drowsiness all compound the sedation and can dangerously slow your breathing and reflexes when combined with cyclobenzaprine.
Mixing With Alcohol
Alcohol and cyclobenzaprine together intensify each other’s effects on the nervous system. Even a moderate amount of alcohol can produce significant dizziness, heavy drowsiness, and impaired judgment beyond what either substance would cause alone. This combination increases your risk of falls, car accidents, and poor decision-making. The safest approach is to avoid alcohol entirely while you’re taking it.
What Happens in Overdose
Cyclobenzaprine overdose is rarely fatal. A five-year review of overdose cases found no deaths, even with ingested doses ranging from 5 mg to 1,000 mg. The most common overdose symptoms were excessive drowsiness, rapid heart rate, and agitation. Both high and low blood pressure were sometimes seen.
Serious symptoms requiring medical treatment beyond basic care didn’t occur with ingestions under 100 mg, and all symptomatic cases showed signs within four hours of ingestion. Life-threatening heart rhythm disturbances were extremely rare. That said, any suspected overdose still warrants emergency medical attention, especially if other substances were involved.
Dependence and Withdrawal
Cyclobenzaprine is not classified as a controlled substance, and it doesn’t produce the same kind of “high” as opioids or benzodiazepines. However, using it continuously for longer than recommended can lead to physical dependence. Your body adjusts to its presence, and stopping abruptly after prolonged use can cause withdrawal symptoms: returning muscle stiffness and spasms, fatigue, anxiety, insomnia, nausea, and sweating.
The risk of dependence is one more reason the two-to-three-week limit matters. If you’ve been taking it for an extended period, tapering off gradually rather than stopping cold is a safer approach. Tolerance can also develop, meaning the same dose becomes less effective over time, which sometimes leads people to take more than prescribed.

For most healthy adults with acute muscle spasms and no heart conditions, cyclobenzaprine at the lowest effective dose for a short course is a reasonable option. The practical rules are straightforward: stick to the prescribed dose, don’t combine it with alcohol or other sedating substances, and don’t extend use beyond a few weeks. Take it at bedtime if drowsiness is a problem during the day. If you have heart disease, take antidepressants, or are over 65, the risk-benefit balance shifts, and alternatives may be worth discussing with your provider.
