Is It Safe to Take Gabapentin? Risks and Side Effects

Gabapentin is generally safe when taken as prescribed for its approved uses, but it carries real risks that depend on your health, your age, and what other medications you take. It’s FDA-approved for two conditions: nerve pain after shingles (postherpetic neuralgia) in adults and as an add-on treatment for partial seizures in people aged 3 and older. Millions of people take it without serious problems, yet there are specific situations where gabapentin becomes genuinely dangerous.

Common Side Effects

The most frequent side effects are dizziness, drowsiness, and swelling in the hands or feet. These tend to be worst during the first few weeks as your body adjusts, especially during the initial dose increases. Most people start at 300 mg once daily, gradually working up to 900 to 1,800 mg per day split into three doses. That slow ramp-up exists specifically to minimize side effects.

Fatigue and coordination problems are common enough that you should avoid driving or operating heavy equipment until you know how gabapentin affects you. Some people also experience blurred vision, weight gain, or mild nausea. These side effects are usually tolerable and often improve over time, but they don’t always go away completely.

The Opioid Combination Risk

The most serious safety concern with gabapentin involves taking it alongside opioid painkillers or other drugs that slow your central nervous system. A large population-based study found that people prescribed both gabapentin and opioids had roughly 49% higher odds of opioid-related death compared to those on opioids alone. At moderate to high doses of gabapentin, that figure climbed to about 60% higher odds. The mechanism is straightforward: both drugs suppress breathing, and together they can slow respiration to a dangerous degree.

In 2019, the FDA issued a formal safety warning about this. The agency specifically flagged several groups at higher risk for serious breathing problems while on gabapentin: people using opioids, people taking other sedating medications, people with lung conditions like COPD, and older adults. Warning signs to watch for include extreme sleepiness, shallow or slowed breathing, confusion, and bluish tinting of the lips or fingertips.

Alcohol and Gabapentin

Despite what you might expect, clinical laboratory studies have found no significant pharmacological interaction between alcohol and gabapentin. Gabapentin didn’t affect blood alcohol levels, the degree of intoxication, or sedation in controlled settings. That said, both substances cause drowsiness on their own, so combining them can still amplify fatigue and impair coordination in practice. The real-world risk is less about a dangerous chemical interaction and more about compounded sedation affecting your balance and judgment.

Kidney Function Matters

Gabapentin is eliminated almost entirely through your kidneys. If your kidneys aren’t working well, the drug builds up in your body and side effects intensify, sometimes dangerously. Dose adjustments are essential and dramatic. Someone with normal kidney function might safely take up to 3,600 mg per day, while a person with moderately reduced kidney function (creatinine clearance of 30 to 49 mL/min) should take no more than 900 mg per day. For people with severe kidney impairment, the maximum drops to 300 mg daily or even 300 mg three times per week after dialysis sessions.

If you have any degree of kidney disease, your prescriber needs to know. Gabapentin toxicity from impaired kidney clearance can cause severe sedation, confusion, and respiratory depression.

Risks for Older Adults

Gabapentin deserves extra caution in people over 65. A study tracking older adults with normal cognition at baseline found that starting gabapentin was associated with measurable cognitive decline. Within one year, gabapentin users had 55% higher odds of worsening on a standard dementia rating scale and 94% higher odds of decline on a more detailed cognitive assessment compared to non-users. By two years, the risk of falls more than doubled, with 2.5 times higher odds compared to those not taking the drug.

These findings are particularly concerning because gabapentin is frequently prescribed off-label for general pain in older adults, many of whom already face elevated fall and cognitive decline risks. The combination of dizziness, sedation, and balance impairment can turn a routine trip to the bathroom into a broken hip.

Suicidal Thoughts

Gabapentin belongs to a class of drugs called anticonvulsants, and all anticonvulsants carry an FDA boxed warning about an increased risk of suicidal thoughts and behavior. The risk is relatively small in absolute terms but consistent enough across clinical trials that the FDA requires it on every label. This applies to all conditions gabapentin treats, not just epilepsy. If you notice new or worsening depression, unusual mood changes, or thoughts of self-harm after starting gabapentin, that warrants immediate contact with your prescriber.

Stopping Gabapentin Safely

One of the less well-known risks of gabapentin is what happens when you stop it abruptly. Withdrawal symptoms can begin as early as 12 hours after the last dose, though they sometimes take up to a week to appear. Common withdrawal effects include anxiety, restlessness, insomnia, sweating, nausea, body pain, and dizziness. In more severe cases, people experience heart palpitations, confusion, and seizures, even in people who don’t have epilepsy.

Most withdrawal episodes last 5 to 10 days, but some people need as long as 18 weeks to fully taper off while managing symptoms. The standard approach is a gradual dose reduction over at least a week, though longer tapers are common for people who’ve been on high doses or used gabapentin for an extended period. Never stop gabapentin cold turkey, especially if you take it for seizures, where abrupt discontinuation can trigger dangerous rebound seizure activity.

Pregnancy Concerns

For pregnant women or those planning pregnancy, the picture is mixed. A large US study using Medicaid data found no overall increase in major birth defects from first-trimester gabapentin exposure. However, there was a signal for a higher risk of heart malformations in the baby, particularly among women who filled two or more prescriptions (about 40% higher risk). Gabapentin use later in pregnancy was associated with a 28% higher risk of preterm birth, a 39% higher risk of the baby being small for gestational age, and a 35% higher risk of the baby needing intensive care admission. These are relative increases over baseline rates, so the absolute risk to any individual pregnancy remains modest, but they’re worth weighing against the benefits of continued treatment.

Off-Label Use and Growing Scrutiny

A large portion of gabapentin prescriptions are for conditions it was never formally approved to treat: general chronic pain, anxiety, migraines, restless legs, and insomnia. This off-label prescribing exploded partly because gabapentin was seen as a safer alternative to opioids. While it does carry fewer addiction risks than opioids, the assumption that it’s harmless has eroded as evidence of respiratory risks, cognitive effects in the elderly, and withdrawal potential has accumulated. Several states now classify gabapentin as a controlled substance, requiring the same prescription monitoring used for opioids.

Gabapentin is safe for many people when the dose matches their kidney function, when it’s not combined with opioids or other sedating drugs, and when they’re monitored for the side effects that matter most for their age and health status. The risks are real but manageable with the right precautions.
