Is Jardiance Good for Your Heart? Benefits and Risks

Jardiance (empagliflozin) is one of the most effective medications available for protecting the heart, and the evidence behind it is strong. In a landmark trial of people with type 2 diabetes and existing heart disease, Jardiance reduced the risk of cardiovascular death by 38% and cut hospitalizations for heart failure by 35% compared to placebo. It’s now FDA-approved not only for diabetes but specifically to reduce the risk of cardiovascular death and hospitalization in adults with heart failure, regardless of whether they have diabetes.

Major cardiology organizations treat it as essential therapy. The American College of Cardiology lists SGLT2 inhibitors like Jardiance as one of the four core pillars of heart failure treatment, giving it the highest recommendation level (Class I, Level of Evidence: A).

What Jardiance Does for the Heart

Jardiance was originally designed to lower blood sugar by causing the kidneys to flush excess glucose into the urine. But researchers discovered its heart benefits go far beyond blood sugar control. The drug works on multiple systems at once.

First, it acts as a mild diuretic. By prompting the kidneys to excrete more sodium and water, it reduces the volume of fluid your heart has to pump. This lowers the pressure your heart works against on both sides: the blood returning to the heart (preload) and the resistance it pumps against (afterload). For a struggling heart, that’s a meaningful reduction in workload. It also reduces arterial stiffness and vascular resistance.

Second, Jardiance shifts the heart’s fuel source. When your body loses glucose through urine, it compensates by burning more fat and producing more ketone bodies. Ketones are a particularly efficient fuel for heart muscle, especially in hearts that are already weakened. This metabolic shift may help a failing heart generate energy more effectively.

Third, and perhaps most surprisingly, the drug appears to protect heart cells directly. It reduces inflammation and oxidative stress in cardiac tissue, slows the death of heart muscle cells, and improves how heart cells handle calcium and sodium. These effects happen even though the drug’s primary target doesn’t exist in heart tissue, meaning they’re independent of its blood sugar or diuretic effects. Researchers are still working out the exact pathways, but the clinical results are clear.

The Key Clinical Trials

The evidence for Jardiance’s heart benefits comes from several large trials. The EMPA-REG OUTCOME trial enrolled patients with type 2 diabetes and established cardiovascular disease. Compared to placebo, Jardiance reduced the composite risk of cardiovascular death, nonfatal heart attack, or nonfatal stroke by 14%. The reductions in cardiovascular death (38%) and heart failure hospitalization (35%) were especially striking, along with a 32% reduction in death from any cause.

Two additional trials focused specifically on heart failure. The EMPEROR-Reduced trial studied patients whose hearts pumped poorly (reduced ejection fraction), while EMPEROR-Preserved studied patients whose hearts pumped normally but filled stiffly (preserved ejection fraction). In both trials, Jardiance significantly reduced the combined risk of cardiovascular death or hospitalization for heart failure. In EMPEROR-Preserved, 13.8% of patients on Jardiance hit that endpoint compared to 17.1% on placebo. The benefit in both trials was driven primarily by fewer hospitalizations rather than a clear reduction in death.

One important nuance from EMPEROR-Preserved: patients whose hearts pumped very normally (ejection fraction above 60%) did not see a statistically significant benefit. The strongest results were in patients closer to the borderline range.

Benefits Without Diabetes

One of the most important findings about Jardiance is that you don’t need diabetes to benefit. A randomized trial of 84 non-diabetic patients with heart failure and reduced ejection fraction found that six months of Jardiance produced dramatic improvements. The heart physically remodeled: the left ventricle shrank in size, lost excess mass, and pumped more effectively, with ejection fraction improving by about 6 percentage points versus no change in the placebo group.

Patients also felt meaningfully better. They walked an average of 81 meters farther in a six-minute walk test (the placebo group walked 35 meters less), consumed more oxygen during peak exercise, and reported substantially higher quality of life scores. These aren’t subtle lab findings. They represent real improvements in how much people can do in daily life.

This is why the FDA approval for heart failure doesn’t require a diabetes diagnosis, and why cardiology guidelines recommend it for all patients with symptomatic heart failure regardless of blood sugar status.

How It’s Taken for Heart Health

For heart failure, the standard dose is 10 mg once daily. This is the same starting dose used for diabetes, but unlike with diabetes, there’s no reason to increase it to 25 mg for heart failure purposes. The higher dose exists only for patients who need additional blood sugar control.

Side Effects to Be Aware Of

Because Jardiance causes the kidneys to excrete more fluid, there’s a theoretical concern about blood pressure dropping too low, especially in heart failure patients who already take diuretics. In practice, this risk appears small. An analysis of patients hospitalized with acute heart failure found that blood pressure changes with Jardiance were tiny (roughly 2 mmHg in either direction compared to placebo) and not statistically significant. The drug was effective and safe across different blood pressure levels.

The most common side effects relate to the drug’s mechanism of flushing sugar through the urinary tract. The excess glucose creates an environment where yeast can thrive, leading to genital yeast infections, particularly in women. Urinary tract infections can also occur more frequently. These are manageable but worth knowing about, especially since heart failure patients may take the medication indefinitely.

Dehydration is possible, particularly in older adults or those on other diuretics. Staying well hydrated and monitoring for dizziness when standing up can help. Your prescriber will typically check kidney function periodically, since the drug’s effects depend on how well the kidneys are working.

How Kidney Protection Helps the Heart

Heart failure and kidney disease feed each other in a destructive cycle: a weak heart reduces blood flow to the kidneys, and failing kidneys cause fluid buildup that strains the heart further. Jardiance interrupts this loop from both sides. It reduces the volume of fluid the heart must handle while simultaneously slowing the progression of kidney disease. This dual protection is one reason the cardiovascular benefits are so consistent across trials. The drug essentially eases the burden on both organs at once, which is something few other medications accomplish with a single pill.
