Kratom does appear to have diuretic properties, meaning it can increase how much water your body releases through urination. Users commonly report frequent urination, dry mouth, and signs of dehydration, all consistent with a diuretic effect. However, the picture is more complicated than a simple “yes.” Kratom’s influence on fluid balance involves multiple mechanisms, some of which can push your body in opposite directions depending on dose and duration of use.
Why Kratom Increases Urination
Kratom comes from the same plant family as coffee, and users frequently compare the two. At lower doses, kratom acts as a stimulant, and like caffeine, it promotes water loss. Social media analyses of kratom users consistently mention frequent urination as a recognizable side effect, often alongside dry mouth and thirst. These symptoms point to increased fluid output without a matching increase in fluid intake.
The exact mechanism isn’t fully mapped out in clinical research. Kratom contains dozens of active alkaloids, and their combined effect on the kidneys hasn’t been isolated the way it has for well-studied diuretics. What researchers do know is that kratom’s primary alkaloid interacts with opioid receptors and other signaling systems in the brain and body, which can alter how the kidneys handle water and sodium. The stimulant effects at lower doses likely contribute to increased urine production in a way similar to other mild stimulants.
The Opposite Problem: Water Retention at Higher Doses
Here’s where it gets counterintuitive. While kratom can push water out of your body at some doses, chronic or heavy use may cause your body to hold onto too much water in a different way. Kratom appears to increase secretion of antidiuretic hormone (ADH), which tells your kidneys to reabsorb water instead of releasing it. This can dilute the sodium in your blood, a condition called hyponatremia.
In one published case, a 62-year-old man with chronic kratom use arrived at the emergency department with severely altered mental status. His blood sodium had dropped to 103, well below the normal range of 135 to 145. Sodium that low causes brain swelling and can be life-threatening. The mechanism researchers identified was that kratom’s alkaloids block certain inhibitory signals in the brain, leading to excess ADH release. So while you might urinate more frequently in the short term, long-term heavy use can paradoxically disrupt your body’s ability to regulate water balance in the opposite direction.
Dehydration and Kidney Stress
The more immediate concern for most users is straightforward dehydration. If kratom increases your urine output and causes dry mouth but you don’t drink extra water, you lose more fluid than you’re taking in. Over time, this can stress your kidneys.
Case reports have documented acute kidney injury linked to kratom’s primary alkaloid. In one case, dehydration from kratom use combined with muscle breakdown (triggered by seizures from toxicity) overwhelmed the kidneys, causing a sharp rise in waste products in the blood and a drop in urine output. That’s an extreme scenario involving toxicity, not typical use, but it illustrates how dehydration from kratom can compound other problems.
A Malaysian study that followed long-term kratom users found no significant changes in standard kidney function markers, even among people who had used kratom for years. However, the researchers did note that a more sensitive kidney marker called cystatin C was slightly elevated beyond the reference range in some users, regardless of how much they consumed. The study also excluded people with diabetes, high blood pressure, or other conditions that affect the kidneys, so it doesn’t tell us much about how kratom interacts with pre-existing kidney problems.
Signs You’re Losing Too Much Fluid
The early signs of kratom-related dehydration are the ones you’d expect: dry mouth, darker urine, headaches, and fatigue. These are common enough among kratom users that online communities treat them as routine side effects rather than warning signs. But if you’re also experiencing muscle cramps, dizziness when standing, or confusion, your fluid and electrolyte balance may be more seriously disrupted.
Electrolyte imbalances from chronic use are harder to detect on your own. Dangerously low sodium, for instance, primarily shows up as neurological symptoms: confusion, restlessness, sluggishness, or in severe cases, unresponsiveness. These symptoms develop gradually with chronic use and can be mistaken for effects of the kratom itself rather than a separate medical problem.
Staying Hydrated if You Use Kratom
The practical takeaway is that kratom does increase your body’s water needs. Drinking a glass of water before taking kratom and another within the following hour helps offset the initial fluid loss. Beyond that, aiming for at least eight glasses of water throughout the day provides a reasonable baseline, with more needed if you’re exercising, in hot weather, or using kratom multiple times daily.
Pay attention to your urine color as a simple hydration check. You want a pale yellow. Clear urine actually suggests you may be flushing out electrolytes, which creates its own problems. If your urine is consistently dark or you notice a significant increase in how often you’re urinating, your body is telling you to adjust your water intake. Pairing water with foods or drinks that contain electrolytes (sodium, potassium, magnesium) can help maintain balance, especially with regular use.
