Head lice are genuinely difficult to get rid of, and it’s not because you’re doing something wrong. The biology of lice is stacked in their favor: their eggs are glued to hair with a protein cement that resists water and most shampoos, many lice populations have developed resistance to common over-the-counter treatments, and a single missed egg can restart the entire cycle. Most families need two to three weeks of consistent effort to fully clear an infestation.
Why Lice Eggs Are So Hard to Remove
The biggest obstacle isn’t killing adult lice. It’s the eggs, called nits, which female lice cement directly onto individual hair strands near the scalp. That cement is made of proteins arranged in layered sheets and cross-linked with other compounds, creating a sheath that hardens through oxidation once it hits air. The result is a bond so strong that normal washing, conditioner, and even vigorous combing often can’t dislodge it.
Making matters worse, the chemical composition of this glue is remarkably similar to human hair itself. That means any product strong enough to dissolve the nit sheath could also damage your hair. This is why so many parents find themselves picking nits out one by one with a fine-toothed comb, even after using a treatment product. The eggs that survive are the ones that keep the infestation going.
The Life Cycle Works Against You
Lice reproduce on a tight schedule that punishes any gaps in treatment. Eggs hatch in about 8 to 9 days. The newly hatched nymphs mature into egg-laying adults within another 9 to 12 days. An adult louse lives for about 30 days on a human head, laying several eggs per day. If even a few nits survive the first round of treatment, you can have a full new generation within two weeks.
This is exactly why most treatments require a second application 7 to 10 days after the first. The goal is to kill any nymphs that hatched from eggs that survived round one, before those nymphs are old enough to lay eggs themselves. Skip that second treatment or do it too late, and the cycle resets.
Many Over-the-Counter Products No Longer Work Well
The most common drugstore lice treatments contain permethrin or pyrethrins, chemicals that kill lice by attacking their nervous system. These worked well for decades, but lice have been evolving resistance. The CDC acknowledges that lice have developed some resistance to permethrin, though the exact prevalence hasn’t been well studied. What is clear from clinical data is that these products now fail frequently.
In one comparative study, students treated with permethrin for a full 14 days had a recovery rate of just 15.4%, with the remaining 84.6% still infested. That’s a striking failure rate for a product many parents reach for first. The researchers pointed to drug resistance as the likely explanation.
Silicone-based treatments containing dimethicone work through a completely different mechanism. Instead of poisoning lice, they physically coat the insects and block their breathing holes, suffocating them. Because this is a mechanical kill rather than a chemical one, lice can’t develop resistance to it. In the same study, dimethicone cleared lice in 82.6% of cases after two weeks, compared to 54.7% for permethrin. That’s a meaningful difference, and it’s why many dermatologists now recommend silicone-based products over traditional insecticides.
Prescription Options for Stubborn Cases
When over-the-counter products fail, prescription treatments can be significantly more effective. One option is a topical lotion that works with a single 10-minute application and requires no nit combing afterward. In two large clinical trials submitted to the FDA, about 76% of treated people were completely lice-free at day 15 after just one application, compared to roughly 16 to 20% in the placebo groups. That’s not a perfect cure rate, but for a single application with no repeat treatment required, it represents a major improvement over what most families experience with drugstore products.
Wet Combing Alone Has Limits
Many parents try to avoid chemicals entirely and rely on wet combing, the method of saturating hair with conditioner and methodically working through it with a fine-toothed nit comb every few days for two weeks. It can work, but the success rate is lower than you might expect. A randomized trial published in The Lancet found that wet combing cured only 38% of cases, while a medicated lotion cured 78%. Children in the combing group were 2.8 times more likely to still have lice at the end of treatment.
Wet combing is most effective as a complement to treatment rather than a replacement for it. Using a nit comb after applying a product helps remove dead lice and loosened eggs, improving your odds. But relying on combing alone means you need near-perfect technique across multiple sessions, and missing even a small section of hair can leave viable eggs behind.
Home Remedies Fall Short
Mayonnaise, olive oil, coconut oil, and vinegar are among the most commonly tried home remedies. The theory behind occlusive agents like mayonnaise is that they suffocate adult lice, and there’s some truth to that. Thick oils can slow down and kill some adult lice. But they do not kill eggs. Since eggs are the primary reason lice keep coming back, a remedy that leaves nits intact is only solving half the problem.
Vinegar is often suggested as a way to dissolve nit glue, but given the complex protein structure of the cement, household vinegar doesn’t reliably break that bond. There’s no published clinical evidence showing vinegar improves nit removal rates enough to matter.
Why Treatment Keeps Failing
If you’ve treated lice and they keep coming back, the cause is usually one of a few common issues rather than some uniquely resistant strain. The most frequent reasons for treatment failure, according to DermNet, are:
- Reinfestation from another person. Your child may be getting lice again from a classmate or family member who hasn’t been treated. Everyone in close contact needs to be checked.
- Misdiagnosis. Old, empty nit casings (white shells) can stay glued to hair for months after lice are gone. If you’re seeing white specks but no live lice, the infestation may already be over.
- Improperly applied treatment. Not using enough product to saturate all the hair, rinsing too soon, or skipping the second application are all common mistakes that leave lice alive.

What Actually Gets Rid of Them
The most reliable approach combines a proven treatment product with thorough manual removal. Start with a silicone-based or prescription product rather than permethrin if you want the best odds on your first attempt. Apply it exactly as directed, making sure every strand of hair from scalp to tip is saturated. Follow up with a fine-toothed nit comb on wet, conditioned hair to remove as many eggs as possible.
Repeat the treatment at the interval specified on the product (typically 7 to 10 days) to catch any nymphs that hatched after the first round. Continue combing every few days during this window. Check all household members and treat anyone with live lice at the same time.
You don’t need to panic about your house. Lice die within two days once they fall off a human head, and eggs can’t hatch at room temperature. Washing bedding and recently worn clothes in hot water is reasonable, but deep-cleaning your entire home isn’t necessary. Lice are a hair problem, not a house problem. Focusing your energy on thorough, correctly timed treatment on the head is what actually ends the cycle.
