Linzess is not a traditional laxative, though it does help you have bowel movements. It belongs to a different drug class called GC-C agonists (sometimes referred to as “secretagogues”), and it works through a fundamentally different mechanism than the laxatives you’d find over the counter. The distinction matters because Linzess does something most laxatives cannot: it also reduces abdominal pain.
How Linzess Differs From Traditional Laxatives
Over-the-counter laxatives generally fall into a few categories. Bulk-forming laxatives (like psyllium) add fiber to draw water into stool. Osmotic laxatives (like MiraLAX) pull water into the intestine through osmosis. Stimulant laxatives (like bisacodyl) trigger muscle contractions in the gut wall. All of these can relieve constipation, but none of them address abdominal pain or discomfort.
Linzess takes a different route entirely. Its active ingredient, linaclotide, is a small peptide that activates specific receptors on the inner surface of your intestinal lining. When those receptors are activated, your intestinal cells release chloride and bicarbonate into the gut, which pulls fluid along with it. The result is more liquid in the intestine, softer stool, and faster transit. So while the end result looks similar to a laxative, the biological pathway is distinct.
The bigger difference is pain relief. In animal studies, linaclotide reduced intestinal pain by desensitizing the nerve fibers that send pain signals from the gut. This dual action, easing both constipation and pain, is why Linzess is prescribed for conditions where abdominal discomfort is a major part of the picture, not just difficulty passing stool.
What Linzess Is Approved to Treat
The FDA has approved Linzess for three conditions:
- Irritable bowel syndrome with constipation (IBS-C) in adults and children 7 and older
- Chronic idiopathic constipation (CIC) in adults
- Functional constipation (FC) in children 6 and older
IBS-C is the condition where Linzess’s dual mechanism matters most. People with IBS-C don’t just struggle with infrequent bowel movements. They also deal with bloating, cramping, and abdominal pain that a standard laxative won’t touch. CIC is long-term constipation without a known underlying cause, and functional constipation in children is a similar pattern of persistent, difficult bowel movements.
Dosing Depends on the Condition
Linzess comes in three strengths: 72 mcg, 145 mcg, and 290 mcg. Adults with IBS-C typically take the highest dose (290 mcg once daily), while adults with chronic constipation use the 145 mcg or 72 mcg dose depending on how they respond. Children 7 and older with IBS-C take 145 mcg, and children 6 and older with functional constipation take 72 mcg.
You take it on an empty stomach, ideally 1 to 2 hours before your first meal of the day. This timing matters for how well the drug works, since food in the stomach can affect absorption.
Safety Considerations
The most common side effect is diarrhea, which makes sense given the drug’s mechanism of increasing intestinal fluid. For most people, this is manageable and may improve over time or with a dose adjustment.
Linzess carries a boxed warning, the FDA’s most serious label, regarding children under 2 years old. In studies with neonatal mice (roughly equivalent to newborn humans), a single dose caused fatal dehydration because very young intestines are far more sensitive to the drug’s fluid-secreting effect. Linzess is completely contraindicated in children under 2. It’s also contraindicated in anyone with a known or suspected mechanical bowel obstruction.
Why Your Doctor Prescribed It Instead of a Laxative
If you’ve been prescribed Linzess, it’s likely because over-the-counter options either didn’t work well enough or didn’t address your full set of symptoms. Standard laxatives are a reasonable first step for occasional constipation, but they aren’t designed for the chronic pain-plus-constipation pattern of IBS-C. Linzess targets both problems through a single mechanism, acting locally in the gut with minimal absorption into the bloodstream.
That local action is another key difference from some prescription medications. Linzess is a peptide that works almost entirely on the intestinal surface. It isn’t circulating through your body the way many systemic drugs do, which is part of why its side effect profile is relatively narrow.
