Is Manuka Honey Low FODMAP? The Fructose Truth

Manuka honey is high FODMAP and best avoided during the elimination phase of a low FODMAP diet. Like all honey varieties, it contains significantly more fructose than glucose, and that excess fructose is the specific problem for people with IBS or fructose malabsorption.

Why Honey Is High FODMAP

Honey is roughly 80% simple sugars, with fructose making up 35 to 40% and glucose 30 to 35%. That gap matters. When a food contains more fructose than glucose, the extra fructose has to be absorbed on its own, and the human intestine is not well equipped for this. Fructose absorption relies on a transport protein that can be overwhelmed by even small loads of excess fructose.

When that transport system gets overloaded, unabsorbed fructose travels to the colon, where gut bacteria ferment it. This produces hydrogen, carbon dioxide, and other gases while also drawing water into the intestine through osmosis. The result is bloating, gas, cramping, and diarrhea, symptoms that closely mirror IBS.

Monash University, the research group that developed the FODMAP system, lists honey as a high FODMAP sweetener alongside high fructose corn syrup.

Manuka Honey Is No Exception

Different honey varieties (clover, orange blossom, yellow box, manuka) have slightly different sugar profiles, but most land around the same 30% glucose to 40% fructose ratio. That roughly 10% excess fructose is enough to trigger symptoms in sensitive individuals. Manuka honey follows this pattern and is not considered safer than regular honey from a FODMAP perspective.

Manuka honey is also particularly dense in oligosaccharides, another category of FODMAPs, which adds to its potential to cause digestive trouble. FODMAP-focused dietitians typically recommend avoiding it entirely during the elimination phase of the diet.

The Methylglyoxal Factor

Manuka honey may actually be worse for IBS sufferers than regular honey because of methylglyoxal (MGO), the compound responsible for its antibacterial properties and the reason people seek it out. Research published in PLOS One found that methylglyoxal can independently trigger IBS-like symptoms, including diarrhea, increased gut sensitivity, and changes in intestinal bacteria. In animal studies, even moderate concentrations of MGO significantly increased fecal water content and lowered the threshold for visceral pain.

Higher-rated manuka honey (higher UMF or MGO numbers) contains more methylglyoxal. So the premium manuka products that people pay the most for are, paradoxically, the ones most likely to aggravate a sensitive gut.

Can You Have a Tiny Amount?

Some FODMAP resources suggest that regular honey may be tolerable at around one teaspoon, though this sits right at the boundary and varies from person to person. For manuka honey specifically, even this small amount is risky given the added MGO content and higher oligosaccharide density. During the elimination phase, skipping it entirely is the safest approach. If you want to test your personal tolerance later during the reintroduction phase, start with half a teaspoon and monitor your symptoms over 24 hours.

Low FODMAP Sweetener Alternatives

If you use honey in cooking, tea, or baking, several sweeteners work well as replacements without the excess fructose problem.

  • Maple syrup is the most versatile liquid substitute. It’s considered safe up to 2 tablespoons per serving and can replace honey at a 1:1 ratio. If you’re baking, reduce other liquids by about 2 tablespoons per quarter cup of maple syrup to keep the consistency right.
  • Rice malt syrup is another solid option, safe up to 1 tablespoon per serving. It has a mild, slightly butterscotch flavor that works especially well in Asian-inspired dishes and granola bars. It substitutes for honey at a 1:1 ratio.
  • Table sugar (sucrose) is low FODMAP because it contains equal parts fructose and glucose. The glucose helps carry fructose across the intestinal wall, so there’s no excess fructose left to cause trouble.

For anyone specifically drawn to manuka honey for its antibacterial or wound-healing properties, those benefits are well documented for topical use. Applying it to skin doesn’t involve the digestive system at all, so FODMAP content is irrelevant in that context. The issue is only with eating it.
