Is Massage Good for You? Benefits and Risks Explained

Massage therapy offers measurable benefits for pain, anxiety, muscle recovery, and immune function. It’s not just a luxury or a feel-good indulgence. A growing body of clinical evidence shows that regular massage produces real, lasting changes in how your body handles pain, stress, and inflammation.

Pain Relief, Especially for Back Pain

Chronic low back pain is one of the most studied conditions in massage research, and the results are striking. In a study of 401 adults with nonspecific chronic low back pain, participants received one-hour massages weekly for 10 weeks. About two-thirds of those in the massage groups improved substantially, compared to only one-third of those receiving usual medical care. People getting massage were twice as likely to report significant improvements in both pain and daily function.

Those improvements held up at follow-up assessments conducted six months and one year later, suggesting that the benefits aren’t purely temporary. For neck pain specifically, research indicates that 60-minute sessions two or three times per week produce better results than once-weekly sessions or shorter 30-minute appointments. If you’re dealing with persistent pain, frequency matters more than most people expect.

Anxiety and Depression

Massage doesn’t just work on muscles. It reliably lowers anxiety and improves mood. In clinical trials involving people with generalized anxiety disorder, twice-weekly Swedish massage led to significant improvements on both clinician-rated and self-reported anxiety scales. For depression, a meta-analysis of multiple trials found a moderately large treatment effect, placing massage in the same ballpark as some established therapies for mild to moderate depressive symptoms.

The mechanism is straightforward: massage lowers cortisol (your body’s primary stress hormone) while promoting relaxation through sustained, rhythmic physical contact. This combination of reduced stress signaling and nervous system calming explains why many people report sleeping better after regular sessions.

Muscle Recovery After Exercise

If you exercise hard, post-workout soreness is familiar. That deep ache that peaks a day or two after intense effort, known as delayed onset muscle soreness, responds well to massage. A systematic review and meta-analysis found that massage significantly reduced soreness ratings at every time point measured: 24 hours, 48 hours, and 72 hours after strenuous exercise. The largest reductions appeared at the 48- and 72-hour marks, exactly when soreness tends to peak.

This makes massage particularly useful for athletes or anyone training on a tight schedule who needs to recover between sessions. The effect appears to come from improved blood flow to damaged tissue and reduced local inflammation, helping your muscles clear metabolic waste faster.

Immune System Effects

Research on massage and immune function is more preliminary, but the early signals are interesting. In controlled studies on healthy adults, massage sessions increased the activity of both T and B lymphocytes, two key types of white blood cells involved in fighting infections. Levels of certain antibodies also rose during massage periods compared to control periods. At the same time, cortisol levels dropped, which matters because chronically elevated cortisol suppresses immune function.

This doesn’t mean massage will prevent you from catching a cold. But for people whose immune systems are suppressed by chronic stress, regular massage may help restore some of that lost protective capacity.

Different Types Serve Different Purposes

Not all massage is interchangeable. The type you choose should match what you’re trying to accomplish.

  • Swedish massage uses long, flowing strokes at moderate pressure. It targets surface muscles and nerve endings, primarily promoting relaxation, improved circulation, and stress reduction. This is the best starting point if you’re new to massage or looking for general wellness benefits.
  • Deep tissue massage applies sustained, firm pressure to reach the inner layers of muscles, tendons, and connective tissue. It’s designed to break up contracted areas and reduce inflammation. This is the better choice for chronic pain, injury recovery, or persistent muscle tightness that lighter techniques can’t reach.
  • Lymphatic drainage uses very light, rhythmic strokes to encourage the movement of lymph fluid through your body. It’s commonly recommended after surgery to reduce swelling and speed recovery by clearing fluid buildup from tissues.

How Often and How Long

Session length and frequency depend on your goals. For chronic pain conditions, the research points toward 60-minute sessions at least twice a week as the threshold for meaningful improvement. A study on deep tissue massage found that even daily 30-minute sessions over 10 days significantly reduced pain. For general stress management and wellness, once or twice a month is a reasonable maintenance schedule, though you’ll get more from weekly sessions if your budget allows.

Even brief sessions have value. Healthy adults experienced positive effects from just 20 minutes in a massage chair, making shorter, more frequent sessions a practical option if hour-long appointments aren’t realistic for you.

When Massage Can Be Harmful

Massage is safe for most people, but there are situations where it can cause real harm. The most dangerous is deep vein thrombosis, a blood clot in a deep leg vein. Massage near a clot can dislodge it and send it traveling to the lungs, which is a life-threatening emergency. If you have known clots or are at high risk for them, massage to the affected area is off limits.

Other conditions that require caution or avoidance:

  • Fresh injuries with active swelling or inflammation. Massage increases blood flow, which can worsen swelling in the acute phase of a sprain, strain, or fracture.
  • Active infections or fever. Massage can spread infection through the body and raise body temperature further.
  • Bleeding disorders or blood thinner use. The pressure involved in massage increases the risk of bruising or internal bleeding.
  • Uncontrolled high blood pressure or recent heart surgery. The circulatory stimulation from massage adds workload to the heart.
  • Certain pregnancy complications like preeclampsia or placenta previa.
  • Open wounds, burns, or contagious skin infections. Massage over these areas can worsen damage or spread infection.
  • Varicose veins. Direct pressure on them can cause pain or vein damage.

For people with active or untreated cancer, particularly cancers involving the lymphatic system, massage requires careful coordination with an oncology team. Some forms of gentle massage are safe and beneficial during cancer treatment, but the specifics depend on the type and stage of disease.

If none of these conditions apply to you, massage carries very little risk and a solid track record of benefits. The biggest barrier for most people isn’t safety. It’s simply making it a consistent habit rather than an occasional treat.
