In the United States, mebendazole is not available over the counter. It requires a prescription and is sold under the brand name Emverm, which costs roughly $430 per dose. In several other countries, including the UK and Australia, mebendazole is available without a prescription for treating pinworm infections.
Why It’s Prescription-Only in the US
Mebendazole was once more accessible and affordable in the United States. But after a series of corporate acquisitions, the drug was relaunched in 2016 as Emverm, a chewable tablet priced at around $400 per pill. No generic version is currently available in the US market, leaving the prescription brand-name product as the only option.
For pinworms specifically, there is an over-the-counter alternative in the US: a product called Reese’s Pinworm Medicine, which contains a different active ingredient (pyrantel pamoate) and costs less than $15. Many parents searching for mebendazole are dealing with a pinworm infection in their household, and this OTC product treats the same problem at a fraction of the cost.
Where Mebendazole Is Available OTC
Australia’s Therapeutic Goods Administration lists mebendazole as an approved over-the-counter medicine for treating pinworm (threadworm) infections, available as 100 mg chewable tablets or a 20 mg/mL oral suspension. In the UK, it’s sold without a prescription under the brand name Ovex for the same indication. Several other countries also allow OTC sales for pinworm treatment, though the specific regulations and brand names vary.
The key distinction is that OTC availability in these countries is limited to pinworm infections only. If you need mebendazole for hookworm, roundworm, whipworm, or other parasitic infections, a prescription is typically required regardless of where you live.
What Mebendazole Treats
Mebendazole works by blocking parasites from absorbing glucose and disrupting the internal structural proteins they need to survive. Without energy or intact cell structures, the worms die within a few days and are passed naturally from the body.
In the US, the prescription version (Emverm) is approved to treat pinworm, whipworm, roundworm, and hookworm infections. It’s occasionally used off-label for tapeworm infections as well. Pinworm is by far the most common of these in developed countries, particularly among school-age children.
How It’s Taken
For pinworm infections, the standard dose is a single 100 mg tablet, with a second dose taken two weeks later. The medication kills adult worms but not their eggs. That second dose catches any worms that have hatched since the first treatment, which is why skipping it often leads to reinfection.
Because pinworms spread easily within households, all family members may need to be treated at the same time, even if only one person has symptoms. The tiny eggs can survive on surfaces like bedding, towels, and toilet seats, making reinfection common if only the symptomatic person is treated. Washing bedding and towels in hot water on the day of treatment helps reduce this risk.
Mebendazole is approved for adults and children aged 2 and older. For other parasitic infections like whipworm or hookworm, the dosing schedule is different and typically involves taking the medication for three consecutive days.
Your Options If You’re in the US
If you suspect a pinworm infection, the most practical route in the US is to pick up pyrantel pamoate (Reese’s Pinworm Medicine) at a pharmacy or online without a prescription. It works differently from mebendazole but is effective against pinworms and widely recommended as a first-line option.
If your doctor determines that mebendazole specifically is needed, or if the infection involves a parasite other than pinworms, you’ll need a prescription for Emverm. The high price tag can be a barrier, so it’s worth asking about discount programs or whether your insurance covers it. Some compounding pharmacies may also be able to prepare mebendazole at a lower cost, though availability varies.
If you’re traveling to a country where mebendazole is sold over the counter, you can legally purchase it there for personal use. However, importing prescription medications into the US comes with its own regulatory considerations.
