Naproxen is an effective pain reliever for a wide range of conditions, from headaches and menstrual cramps to arthritis and muscle injuries. What sets it apart from other over-the-counter options like ibuprofen is how long it lasts: a single dose works for up to 12 hours, meaning you only need to take it twice a day instead of every four to six hours.
That longer duration makes naproxen particularly useful when you’re dealing with pain that lingers throughout the day, like joint stiffness from arthritis or back pain that won’t quit. But like all pain relievers in the NSAID family, it comes with tradeoffs worth understanding before you reach for the bottle.
How Naproxen Works
Naproxen belongs to a class of drugs called nonselective NSAIDs, which means it blocks two enzymes in your body (COX-1 and COX-2) that produce chemicals called prostaglandins. Prostaglandins are what trigger inflammation, pain, and fever. By cutting prostaglandin production, naproxen reduces swelling at the source and dulls the pain signals your body sends to your brain. This makes it particularly good for inflammatory pain, the kind that comes with redness, swelling, or stiffness, rather than nerve-related pain like sciatica.
How Fast It Works and How Long It Lasts
The naproxen sodium version (sold over the counter as Aleve) starts providing relief within about 30 minutes. Standard naproxen tablets take closer to an hour. Peak levels in your bloodstream arrive within one to two hours for naproxen sodium, or two to four hours for regular naproxen.
The real advantage is duration. A single dose of naproxen covers you for 8 to 12 hours, while ibuprofen wears off in four to six. If you’re managing all-day pain, that’s the difference between two pills and three or four. For overnight relief, especially with conditions like arthritis that cause morning stiffness, a dose before bed can still be working when you wake up.
Naproxen vs. Ibuprofen
In terms of raw pain-relieving power, the two are comparable. A 440 mg dose of naproxen is roughly equivalent to 400 mg of ibuprofen. The choice between them often comes down to convenience and your specific situation.

Naproxen works well for osteoarthritis, rheumatoid arthritis, menstrual cramps, tendinitis, bursitis, gout flares, headaches, dental pain, and general muscle aches. It’s a go-to for menstrual pain because cramps are driven by prostaglandins. Blocking those chemicals at the source often provides more complete relief than acetaminophen, which doesn’t address inflammation.
For arthritis in particular, naproxen’s long duration means consistent coverage throughout the day with minimal dosing. Many people with chronic joint pain prefer it over shorter-acting options for this reason.
Where naproxen is less helpful: nerve pain, fibromyalgia, and pain that doesn’t involve an inflammatory component. For those, different classes of medication tend to work better.
Stomach and Digestive Risks
The same mechanism that makes naproxen effective also creates its biggest drawback. The COX-1 enzyme it blocks plays a protective role in your stomach lining, helping maintain the mucus barrier that shields your gut from its own acid. When that protection drops, you’re more vulnerable to heartburn, nausea, stomach pain, and in more serious cases, ulcers or gastrointestinal bleeding.
These risks increase with long-term use. Taking naproxen for a few days during a flare-up is very different from taking it daily for months. Signs of a serious problem include vomiting blood or dark particles, black or tarry stools, or severe stomach pain that doesn’t go away.
Taking naproxen with food can reduce the odds of stomach irritation. It won’t eliminate the risk entirely, but it provides a buffer. If you have a history of stomach ulcers, naproxen is generally not the best choice. Acetaminophen is gentler on the stomach, and your doctor may recommend it as an alternative or prescribe a stomach-protecting medication to take alongside naproxen if you truly need it.
Kidney and Heart Considerations
All NSAIDs, including naproxen, can affect kidney function. Your kidneys rely on prostaglandins to maintain healthy blood flow, especially when your body is under stress from dehydration, heart failure, or liver disease. Blocking those prostaglandins with naproxen can reduce blood flow to the kidneys, potentially causing acute kidney injury or a gradual decline in kidney function over time.
For most healthy adults using naproxen occasionally, this isn’t a major concern. The risk rises with long-term daily use and in people who already have reduced kidney function, are dehydrated, or take blood pressure medications. Staying well-hydrated while using naproxen is a simple but important precaution.
On the cardiovascular side, the FDA warns that all NSAIDs may increase the risk of heart attack and stroke, particularly with long-term use. The data isn’t strong enough to say that any one NSAID is definitively safer or riskier than another for the heart, so this warning applies broadly across the class.
Important Drug Interactions
Naproxen can interact negatively with blood thinners, increasing the risk of bleeding. It can also reduce the effectiveness of blood pressure medications, which is a concern if you’re managing hypertension. If you take either of these drug types, your prescriber needs to know before you add naproxen to your routine, even the over-the-counter version.
Older adults tend to be more sensitive to naproxen’s effects on both the stomach and kidneys, and often need lower doses or closer monitoring.
Getting the Most From Naproxen
For short-term pain relief in otherwise healthy adults, naproxen is one of the most effective over-the-counter options available. Its long duration, solid anti-inflammatory action, and twice-daily dosing make it especially practical for pain that persists throughout the day. Take it with food to protect your stomach, don’t exceed the recommended dose on the package, and keep use as brief as the situation allows. For ongoing pain that lasts more than 10 days, the underlying cause is worth investigating rather than continuing to mask it with any pain reliever.
